MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."

A "measurable storm event" is a sionn event that results in an actual discharge from the pennitted site
outfall. The previous measurable storm event must have been at least 72 hours prior. The 72-hour storm
interval does not apply if the permittee is able to document that a shorter interval is representative for
local storm events during the sampling period, and the permittee obtains approval from the local DEMLR
Regional Office.

Dear Mr. Odum,
Enclosed is the Stormwater Discharge Outfall (SDO) Monitoring Report form for the 2018 first
semi-annual monitoring period as required by the North Carolina Department of Environmental Quality.
The SDO forms must be submitted even if a sample want llected during an ary 1 - June 30, 2018.
• If you failed to collect,a sample due to lack of a qualifying rain event, write "No Discharge"
in the Total Rainfall, inches column. Otherwise, please give a brief explanation as to why no
sample was taken.
• Sign and date the bottom of the second page of the SDO Monitoring Report where indicated
by tabs.
• Mail the signed, original and one signed copy of the SDO Monitoring Report in the
enclosed envelope with the enclosed cover letter via certified mail (this will require a
postmark from the post office and cannot be mailed from postal drop box or automated postal
machine).
• Complete and sign the Qualitative Monitoring Report (QMR). This form is just an
observation of water quality. You are required to complete the QMR forms twice a year for
each permitted outfall, regardless of analytical reporting for the duration of the permit. These
forms are not due to the State, but should be kept on site in your SWPPP. _
Keep the provided copies of the signed SDO Monitoring Report along with the QMRs, with your
Stormwater Pollution Prevention Plan for your records.

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."
