Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October

Some of the changes include:
• Part Il:
• Section A: The Stormwater Pollution Prevention Plan (SPPP).section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added_ to the SPPP requirements in some cases.
• Sections B, C. Failirre.to peryrform analytical stormwater monitoring may result in the Division requiring
that the,permittee begin a monthly sampling scherne.
• Sections B, C. A lower TSS benchmark of 50 mg/1 forHQW, ORW; PNA and Tr Waters applies to.these
more serisitive waters.
• Sections B, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases,.other

Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements. to dbtain other permits which may be required by DENR, nor does it
,relieve'the permitteeifrorn responsibi€i'ty for compliance with any other,applicable, federal, state; or local,law,

On May 20, 2011, Thomas Ascenzo of the Raleigh Regional Office (RRO) conducted a compliance inspection
(CEI) of The General Services Central Maintenance Facility stormwater pollution program. Your assistance was
appreciated as it facilitated the inspection process.
The General Service Central Maintenance facility discharges stormwater to receiving waters designated as
Pidgeon House Branch in the Neuse River Basin.
The following observations were made:
Review of your Stormwater Pollution Prevention Plan (SP3) revealed the following
• The plan contained the required general location map that included outfall location and drainage
areas.
• Employee training documents were included in SP3. Last training was 12/30/2010.
• List of responsible parties for Emergency. spill response notification- update accordingly
• No spills were noted for the 3 year period as noted in the documentation in the manual.
• Best Management Practices summary.
• Permit was included in SP3 manual- Old version noted, replace with renewed permit.
• Narrative description of practices dated 2003 needs updating.
• Spill prevention and response plan
2. Qualitative Monitoring has been conducted on 5/2010 & 6/1/2010. It should be noted that monitoring
should be spread out over a six month period. Semi-annual as per permit requirements.
3. Analytical Monitoring was conducted 6/1/2010 and 10/20/10 and were within benchmarks. None were
noted for 2011 at the time of inspection.

7. Suggestion to add a tab and insert analytical results in the SP3 manual at one location to ease
inspection and for easy access for the documents.
8. SP3 manual should be updated annually and as needed.
9. SP3 manual contained an old version of the permit, insert new version with manual.
10. Include a Stormwater Facility inspection Report in the SP3 manual.

In accordance with your application for discharge permit received on March 11. 2003, we are forwarding herewith
the subject certificate of coverage to discharge under the subject state - NPDES general permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between
North Carolina and the US Environmental Protection agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to request an individual pemut by submitting an individual permit application. Unless such demand is
made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of
coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division
of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

Request for rescinding of COG NCGO80027
Dear Mr. Albiston:
Thank you for the opportunity to meet with you at our facility bf March 26, 2004_ We
appreciated your input concerning our facility's stormwater permit and your
suggestion on improvements that need to be made to make ;our facility fully compliant
with our permit. Enclosed for your review are the tasks that need to be Implemented
under our Stormwater Pollution Prevention Plan (SWPP), and our best estimate of
their implementation or completion dates.
1. Non-Stormwater Certification to be signed and dated by April 9, 2004.
2. Second Nan-Stormwater Discharge Assessment rind Certification for
Outfalls 1 and 2 by April 23, 2004.
3. Testing and submittal of Stormwater Discharge Monitoring Report (DMR)
for each outfall by April 24, 2004.
4. Organization and identification of Pollution Prevention Team by April 1,
2004, `
5. Employee training activities to cover: Spill Prevehtion and Response,
Good Housekeeping, Material Management Practices, Environmental
Health Exposure Wmits, and Safety Equipment by June 30, 2004,
6. Preventive Maintenance and inspection activities, by July 26, 2004.
7. Implementation of Best Management Practices fir the 1) waste material
storage area (i.e., metals, cardboard, garbage and wooden pallets), 2)
runoff from car washing building ; and 3) runoff from fuel dispensing area
by April 15, 2004 and June-30, 2004.

' It is the facilJty's contention that this location does"not contribute to
stormwato►'runoff ofpollutants and will be lndMdualiy tested far verification
purposes.
Further investigation will be conducted for the upper parking deck, which is used for
storage of used and new vehicles for sale_ Information derived from this investigation
will be included in the SWPP and the stormwater site plan.
We are requesting that the previously existing stormwater permit for the facillty (i.e.,
COC NCGO80027) be rescinded and that we are allowed to operate under the newer
COC NCG080674.
