NPDES STORMWATER PERMIT NCG080659 Wake County Dear Mr. Wilkins: On May 2, 2014, Dave Parnell, of the Raleigh Regional Office of the North Carolina Division of Energy, Mineral and Land Resources (DEMLR), conducted a compliance evaluation inspection (CEI) at the above referenced facility. The site lies in the watershed of an unnamed tributary (UT) to Crabtree Creek (Lake Crabtree), Class B, NSW waters, which lies in the Neuse River Basin. The following observations were noted during the NCGO80659 DEMLR inspection. - This facility is a shipping and receiving parcel facility, which conducts vehicle maintenance on site. Thank you for your assistance during the inspection, it was appreciated. The inspection began in the lobby, where a thorough review of the Stormwater Pollution Prevention Plan; as well as, the qualitative and analytical monitoring results, was conducted. All aspects of the Stormwater Pollution Prevention Plan are being implemented. Analytical and Qualitative monitoring data were reviewed and found to be in order and compliant with the NPDES Stormwater Permit. Following the monitoring data review, all secondary containment, the oil -water separator and the Stormwater Discharge Outfall (SDO) were observed. The entire production site flows to one outfall at the outlet of the Stormwater pond. You are knowledgeable, with respect to the sample site and sampling techniques. Mr. Parnell discussed with you the sample location and the Division's preference that the analytical and qualitative monitoring be conducted at the outlet of the pond, as it discharge off site. Sullins Director May 23, 2011 Subject: Compliance Evaluation Inspection UPS Freight Certificate of Coverage NCGO80659 Wake County Dee Freeman Secretary On May 20, 2011, Thomas Ascenzo of the Raleigh Regional Office (RRO) conducted a compliance inspection (CEI) of UPS Freight stormwater pollution program. The assistance of yourself was appreciated as it facilitated the inspection process. The UPS Freight facility discharges stormwater to receiving waters designated as a unnamed tributary to Crabtree Creek in the Neuse River Basin. The following observations were made: Review of your Stormwater Pollution Prevention Plan (SP3) revealed the following : • The plan contained the required general location map that included outfall location and drainage areas. • Employee training documents were included in SP3. Last training was 1/2011. • List of responsible parties for Emergency spill response notification • No spills were noted for the 3 year period as noted in the documentation in the manual. • Plan included Stormwater Facility Inspection reports • Best Management Practices summary. • Permit was included in SP3 manual. • Narrative description of practices. • Spill prevention and response plan 2. Qualitative Monitoring has been conducted semi-annually on 1/3/2010, and 9/27/2011 as required by the permit. Analytical Monitoring was conducted 9/27/2010, 8/ 2008, 12/2008. No evidence of analytical monitoring conducted for period 1 of 2010 & none were observed for 2009 in addition to 2011 in the documents at the time of inspection. As per permit requirements samples must be conducted between Jan. 2011-June 30 2011 for period 1 and, July 1-Dec.31-2011 for period 2. Samples on 9/27/2010 & 2008 were within benchmarks. It was indicated that results could of have been located at another building within the complex. 4. Outfail #1 was observed to be dry with no signs of pollution.
