On May 29, 2014, Dave Parnell; of the Raleigh Regional Office of the North _Carolina _Division of
Energy, Mineral and Land Resources (DEMLR), conducted a compliance evaluation inspection
(CEI) at the above referenced facility. The site lies in the watershed of an unnamed tributary
(UT) to Stirrup Iron Creek, Class C, NSW waters, in the Neuse River Basin.
The following observations were noted during the NCGO80060.1 DEMLR inspection:
This facility is a vehicle maintenance operation that services and parks transit busses for the
Triangle Transit Authority. There is one Stormwater Discharge Outfall (SDO) and there is an
Oil Water Separator, which discharges waste to the Sanitary Sewer. The used oil is stored and
removed from the site by Noble Oil Company. Thank you for your assistance during the
inspection, it was appreciated. The inspection began in a conference room, where a thorough
review of the Stormwater Pollution Prevention Plan; as well as the qualitative and analytical
monitoring results, was conducted. The Stormwater Pollution Prevention Plan has been
implemented and was found to be in compliance with your NPDES Stormwater Permit.

Qualitative and Analytical monitoring data were reviewed and found to be in order and in
compliance, as well. The SDO was observed, as well as the secondary containment for
hazardous materials. As you and Mr. Parnell discussed, the sample location at the SDO is in
the correct location, and the secondary containment is adequate.

WET CHEMISTRY DATA REPORTING QUALIFIERS
On the appropriate reporting form, under the column labeled "Q" for qualifier, each result is flagged with
the specific data reporting qualifiers listed below, as appropriate. The qualifiers used are:
U : This flag indicates the compound was analyzed for, not detected and is reported as less than the
Method Detection Limit (MDL) (or as defined by the client). The Reporting Limit (RL), or Limit
of Quantitation (LOQ), and the MDL will be adjusted to reflect any dilution or concentration of
the sample and, for soils, the percent moisture.
This flag indicates the reported result is an estimated value. The flag is used when an analyte is
detected and the result is less than the adjusted RL/LOQ but equal to or greater than the MDL.
Q : This flag denotes that one or more quality control criteria have failed (e.g., LCS recovery,
Continuing Calibration Verification, or CCV) and reanalyses can't be perfonned. The Q flag is
applied to all specific analyte(s) in all samples associated with the failed quality control criteria.
B : This flag is used when the analyte is found in the associated method or calibration blank as well
as in the sample. It indicates probable blank contamination and warns the data user to take
appropriate action. The combination of flags BU or UB is not an allowable policy. Blank
contaminants are flagged B only when they are detected in the sample.
• D: This flag is applied to an analyte when the reported result is based on a dilution.
X/Y/Z : Other specific flags may be required to property define the results. If used, the flags will be fully
described in the SDG Narrative. The laboratory -defined flags are limited to X, Y, and Z.

On April 19, 2011 Mr. Myrl Nisely of the Raleigh Regional Office made an inspection of the bus
maintenance facility at 5201 Nelson Road, Morrisville. Your help was greatly appreciated. During the
inspection documents such as the Stormwater Pollution Prevention Plan (SP3}, stormwater monitoring
reports, and employee training records could not be located. Therefore, the site was out of compliance
with the above permit, issued originally in December 1997 and most recently reissued in November
2007. Since the inspection, you report that the SP3 has been located and that you will have a consultant
review and update the stormwater program.
Comments from a tour of the site are these:
The area around the building is paved with asphalt, and slopes toward the back and left side of
the building when viewed from the front. Numerous drop inlets constructed in the curbs carry
rainwater out to several different outfalls, all of which appear to enter vegetated land. The site
map needs to show each outfall location, as well as where the flows ultimately join a stream or
pond.
2. Most of the pavement was clean, the exception being an evident petroleum spill just upstream of
one inlet. That release occurred when a full tank on a bus was left in the sun and expansion
caused a small overflow. It was noted that a container of spill cleanup equipment was next to the
inlet. You reported that since that incident a Best Management Practice is now in place to avoid
filling the tanks absolutely full.

The SP3 is intended to be a living document into which documents are added as activities are
accomplished. Qualitative and quantitative monitoring reports, employee training attendance sheets, and
evidence of annual reviews of the SP3 are, for example, some of the kinds of information to store there
for future inspections.

Inspection Summary:
This site has an oil/water separator, the area around which was clean with no evidence of oil releases. The tank inside
the building had been cleaned two days earlier. Clear water was observed flowing within the unit.
The site is neatly landscaped. Most of the surface slopes significantly away from the back of the maintenance building.
There are numerous drop inlets serving the paved area around the building. One inlet showed evidence of an old
petroleum release, said to be from filling a bus tank completely full and then parking it in the sun,.causing expansion of
the fuel. Spill response equipment was stationed on the site. Above the building is a parking lot, the runoff from which
goes to a stormwater retention pond.
A follow-up inspection is planned after the Stormwater Pollution Prevention Plan (SP3 )has been updated, and possibly
after the required monitoring has been done. The absence of the SP3 and any evidence of its required activities on the
day of the inspection is the reason for this inspection being a Notice of Violation

maintenance facility at 5201 Nelson Road, Morrisville. The site visit confirmed that Triangle Transit
hired a consultant to revise the Stormwater Pollution Prevention Plan (SP3) and provide guidance. This
inspection consisted of a review of the SP3 and discussion of how the program is being implemented.
The help you and Mr. Jim Gleason provided was greatly appreciated. The well written SP3 contains all
the required components. The company has been actively providing employee training to a large
number of workers, as well as sampling storrawater and making qualitative observations of the facility
outfall. Triangle Transit's response and efforts to achieve compliance with the general permit for
Transit and Transportation is commendable and appreciated.

In accordance with your application for discharge permit received on October 10,
1997, we are forwarding herewith the subject certificate of coverage to discharge under the
subject state - NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between
North Carolina and the US Environmental Protection agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this
permit are unacceptable to you, you have the right to request an individual permit by
submitting an individual permit application. Unless such demand is made, this certificate of
coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice
to the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may
be required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other Federal or Local governmental permit
that may be required.

to receiving waters designated as an unnamed tributary to Stirrup Iron Creek, a Class C NSW stream in the Neuse

in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III
and IV of General Permit No. NCGO80000 as attached.
This certificate of coverage shall become effective December 19, 1997,
