On April 23, 2014, a site inspection was conducted for the AAA Cooper Transportation - Cumberland facility
located on Chicken Foot Road, Cumberland County, North Carolina. A copy of the Compliance Inspection
Report is enclosed for your review. John Ryals, Manager and Jimmy Fulghum, Assistant Manager were also
present during the inspection and their assistance is greatly appreciated. Permit coverage authorizes the
discharge of stormwater from the facility to receiving waters designated as Gray's Creek in the Cape Fear
River Basin. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater
General Permit-NCG080000, Certificate of Coverage-NCG080478.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG080000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to $25,000 per day for each violation. If you or your staff has any questions,

Inspection Summary:
Melissa Joyner met with Facility Manager, John Ryals. The Stormwater Pollution Prevention Plan (SPPP) was available
for review. It will need to be annually updated including the information on page four of the permit, SPPP Amendment and
Annual Update, #7., Items a-d. Qualitative Monitoring is being done on a quarterly basis with the company's form. It is
recommended that the state issued Qualitative Monitoring Form also be utilized. The Quantitative Monitoring does not
need to be done because less than 55 gls. of oil is used on average per month. There is also no oil and water separator.
The stormwater is not connected to a sewer line. There is one-outfall (a concrete Swale leading to a diversion ditch) on the
property which is adjacent to the public road. Vehicle maintenance is done on -site. It is noted that the oil, antifreeze and
fuel is located internally in a building or if these products are outside, they are sealed up, under a roof and placed on
pallets. The fuel tanks located under a roof have a leak detection syatem and haz mat materials nearby in case of leaks.

will be sending a compliance inspection report to your facility this week. During the time of the inspection, I had
mentioned to Mr. Ryals (and also to you verbally on the phone) the possibility of the facility receiving a Notice of
Deficiency or Notice of Violation. Neither will be sent to this facility since you do have a Stormwater Pollution Prevention
Plan, which does need to be continually updated annually and you have been doing the Qualitative Monitoring using
your company form. It is recommended that you use the state issued Qualitative Monitoring form which I had sent as an
attachment in an earlier email.

Thank you for sending me the recent NCDENR's response to your permit application.
I have attached the forms I had discussed with you on the phone. If the company is using more than 55 gls. of
oil on average over a 12 month (1 year) period than they need to fill out the Semi-annual Stormwater Discharge
Monitoring Report and the Stormwater Discharge Annual Summary Data Monitoring Report. If less than 55 gls.
of oil on average over a 12 month (1 year) period is used only the Stormwater Discharge Outfall Qualitative
Monitoring Report needs to be completed. This form is not submitted, but remains on site with the SPPP. Please
let me know how much oil is used on average over a 12 month period. Thanks.
In reviewing your new permit, if there have been changes, these need to be updated. In particular, please refer to
Pg. 1: d and e, Pg. 3:, #3, 94 and #5 (John did have a recent record of Employee Training and the personnel's
signatures), and Pg. 4: #7, #8 and #9.

In accordance with your application for discharge permit received on September 4, 1996, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management
Act or any other Federal or Local governmental permit that may be required.
