On September 30, 2014, Dave Parnell, of the Raleigh Regional Office of the North Carolina
Division of Energy, Mineral and Land Resources (DEMLR), conducted a compliance evaluation
inspection (CEI) at the above referenced facility. The site lies in the watershed of an unnamed
tributary (UT) to Little White Oak Creek, Class C waterbody, which lies in the Cape Fear River
Basin.
The following observations were noted during the NCG080156 DEMLR inspection:
This facility is a firefighting training facility for the county and municipalities with in the
county. Thank you for the assistance of Robert Gron, PE, Ricky Dorsey & Joseph Knurowski of
your staff and Guido Carrara, PE of G. C. Environmental. It was appreciated. The inspection
began in the conference room, where a thorough review of the Stormwater Pollution
Prevention Plan (SPPP); as well as, the qualitative and analytical monitoring results, was
conducted. The SPPP is being implemented. To further comply with the intent of the permit,
documentation of three years of significant spills must be developed and kept within the
SPPP. Analytical and Qualitative monitoring data were reviewed and found to be in order
and compliant with the NPDES Stormwater Permit. Stormwater Discharge Outfall (SDO) # 2
has replaced SDO # 1, since the last inspection in 2010. The size of the pond, and the fact that
the pond water is used during training exercises ,has resulted in some sample periods being
reported as "no flow".

Following the monitoring data review, secondary containment and the SDO were observed.
Access to both SDO was well maintained and accessible. The staff was knowledgeable, with
respect to the sample site and sampling techniques.

In response to your renewal application for continued coverage under stormwater General Permit NCG080000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCGO80000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DM R) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of Stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part It of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http1/portal, _ncdenr.org(weblwg/ws(su(current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

for your specific General Permit carefully.

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical Stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme_
• Sections 8, C: A lower TSS benchmark of 50 mg/i for HQW, ORW, PNA and Tr Waters applies to these
more se ns itive waters.
• Sections 8, C: The monitoring parameter Oil & Grease (0&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C• The term "Representative Storm Event' has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's webs ite above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ, This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

inspection conducted at the Wake County Fire Training Center on June 11, 2010.

Dear Mr. Nisely:
Item 1.- Records of employee training, qualitative observations and analytical results will
be placed in a binder to be available during future inspections.
Item 2.- Outfall 002, identified as the outfall of the large pond was tested on 6/30/10.
Item 6.- The ash contaminated Riprap lined swaie has been cleaned as well as physically
possible by the Wake County Department of Fire and Emergency Management.
See enclosed photos of subject area. In the future the waste ash and nails will be
transferred to a rain resistant metal dumpster which as shown in the enclosed photo
will be located between the burn building and drill tower. The dumpster will be
monitored daily by the on -site Deputy Fire Marshall. The dumpster will be
plugged to prevent liquids from leaving the unit.
Item 7.- The burned cars outside of the burn pits have been removed and in the future all
burning will be done from within the burn pit rings.

From our discussion on the phone yesterday, I think that most of the ash and nail deposit
that was a threat to move into the pond has been taken out. The little remaining is not
serious enough to require digging into the swale, moving the riprap, etc to get it all. The
important point of that discovery was that debris like that cannot just be deposited any old
place - it needs to be given thought and discarded in a manner protective of the
environment. For the future, I would assume that what is shoveled out of the burn building
can be put into a container and taken to a landfill.
So, unless.I am missing an important issue here, I don't see the need for me to attend a
meeting.

On June 11, 20101 made an inspection of the stormwater program at the Fire Training facility with Dale
Montgomery and Guido Carrara. Their help was greatly appreciated. Attached is the inspection
checklist. The site was in compliance, but there are some topics to discuss and changes to make.
I was asked whether the facility should be deemed permitted and exempt from a formal permit because
of a clause in NCAC 15A 2H.0106
"(f) The following discharges are deemed to be permitted pursuant to G.S. 143-215.1(c) provided that no water
quality standards are contravened, or expected to be contravened, and it shall not be necessary for the Division to
issue separate permits for these activities: (10) flows from fire fighting"
The intent of this rule is to exempt water flows from emergency fire fighting in situations where it is
impractical or impossible to set up controls to contain or treat runoff from fighting an undesired fire.
Because the fire fighting at the Training Center is planned, intentional, and in a facility designed to
handle the expected runoff, the existing stormwater permit is appropriate.
A review of Stormwater Pollution Prevention Plan records and a tour of the site revealed the following:
1. DWQ requests that records of employee training, qualitative observations and analytical results
be placed in the binder to be available during future inspections.
Analytical testing is needed for this year (2010). This permit covers all stormwater for the site in
addition to the releases from the separator, so the focus of DWQ is on the water quality leaving
the property. Accordingly, the anal3iical.samples. should be taken from the discharge pipe for
the last pond in the series of three ponds. The point labeled Outfall 001 on the map tests the
effectiveness of the Oil/Water separator and the secondary pond following it, but constitutes an
internal point within the entire system. Therefore, please take future samples from the discharge
pipe of the large pond.
3. The plan has been reviewed and updated each year as required.
4. There is no Vehicle Maintenance at the site.

5. One storage room holding small quantities of gasoline, motor oil, and fogging glycols has a floor
drain that is thought to discharge into a low point among some trees and a carpet of leaves a few
feet from the building. That area showed no sign of the fluids and the vegetation showed no
stress.
6. One of the riprap lined swales within a few feet of the final pond contained ash, pieces of
partially burned pallets and straw and nails. These were placed there while they were hot,
removed from the burn building after training exercises. This location is unacceptable because
of the possibility of transfer into the pond and out the nearby discharge pipe. It is recommended
that a holding device be established in one of the fire rings so that any runoff will be carried to
the separator pond.
7. There were burned cars between the rings. Please emphasize to training event planners that all
burning is to be done inside the rings so that fire water from training is carried to the oil/water
separator.
Please respond to this inspection letter within 30 days of receipt with a photo of the ash -contaminated
swale showing that the material has been removed, and another photo of the new location where hot
coals can be dumped (item 6).

been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 21-1.0105, under
the authority of North Carolina General Statutes 143-215.3(a)(1), (1 a) and (1 b). Because this fee was not fully paid within

The Division of Environmental Management has recently evaluated all existing individual permits for potential
coverage under general permits currently issued by the Division. 15A N.C.A.C. 2H .0127 allows the Division to
evaluate groups of permits having similar discharge activities for coverage under general permits and issue
coverage where the Division finds control of the discharges more appropriate in this manner. The Division has
determined that the subject discharge qualifies for such coverage. Therefore, the Division is hereby issuing the
subject Certificate of Coverage under the state-NPDES general permit no. NCG080000 which shall void NPDES
Permit NC0052051. This Certificate of Coverage is issued pursuant to the requirements of North Carolina and the
US Environmental Protection Agency Memorandum of Agreement dated December 6, 1983 and as subsequently
amended.
If any parts, measurement frequencies or sampling requirements contained in this general permit are unacceptable to
you, you have the right to submit an individual permit application, associated processing fee and letter requesting
coverage under an individual permit. Unless such demand is made, this decision shall be final and binding. Please
take notice this Certificate of Coverage is not transferable. Part 11, EA. addresses the requirements to be followed
in case of change of ownership or control of this discharge.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the
Permittee shall take immediate corrective action, including those as may be required by this Division, such as the
construction of additional or replacement wastewater treatment or disposal facilities. Construction of any
wastewater treatment facilities will require issuance of an Authorization to Construct from this Division.
Failure to abide by the requirements contained in this Certificate of Coverage and respective general permit may
subject the Permittee to an enforcement action by the Division of Environmental Management in accordance with
North Carolina General Statute 143-215.6A to 143-215.6C. Please note that the general permit does require
monitoring in accordance with federal law. The monitoring data is required to be submitted to the Division no later
than January 31 for the previous year in which sampling was required to be performed.

The issuance of this Certificate of Coverage is an administrative action initiated by the Division of
Environmental Management and therefore, no fees are due at this time. In accordance with current rules, there
are no annual administrative and compliance monitoring fees for coverage under general permits. The only fee
you will be responsible for is a renewal fee at the time of renewal. The current permit expires August 31, 1997.
This coverage will remain valid through the duration of the attached general permit. The Division will be
responsible for the reissuance of the general permit and at such time, you will be notified of the procedures to
follow to continue coverage under the reissued permit. Unless you fail to follow the procedures for continued
coverage, you will continue to be permitted to discharge in accordance with the attached general permit.
The issuance of this Certificate of Coverage does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be required by the Division of Environmental Management
