On April 04, 2012, I, Mitch Hayes, conducted a compliance evaluation inspection on the subject
facility. The assistance of Steve Simmons, Service Center Manager, with this inspection was greatly
appreciated. Below is a list of findings and recommendations developed from the inspection:
The Certificate of Coverage under General Permit NCGO800000 was issued November 01, 2007
and will expire October 31, 2012. Request for permit renewal and paid associated fees must be
made 180 days prior to expiration to receive automatic authorization to discharge stormwater
beyond the expiration date.
2. The facility receives freight via tractor trailers. Freight is unloaded, resorted, and reloaded onto
tractor trailers for shipment. Tractor trailers are refueled and oil topped off if needed. A private
vendor is hired for truck washing. All wash water is captured and contained and taken to the
vendor's facility for treatment and disposing or recycling.
3. The covered fuel depot consists of a 12,000 gallon above ground storage tank with secondary
containment wall. Absorbent pads are placed next to the pumps in case of spillage. Any spillage
would flow on the concrete pad and onto the asphalt lot that slopes to a drainage outlet at the
southeast corner of the lot. This drainage way can be closed with a gate. Used and new oil are
stored in a storage building adjacent to the covered fuel depot. Less than 55 gallons of new oil is
used a month when average over a year; therefore analytical monitoring is not required.
4. There are three (3) Stormwater Discharge Outfalls (SDO) for the facility: SDO 001 receives flow
from the fuel depot and oil storage building area; SDO 002 receives flow from the vehicle
washing area by the mobile vendor; SDO #3 receives flow from the employee park that drains to
the Municipal Separate Stormwater Sewer System (MS4) operated by the City of Wilson.
Discharges for SDO #3 are not associated or comingle with industrial activities on site and
therefore do not require monitoring. You may remove SDO 003 from your permit monitoring
requirements. Please update your Stormwater Pollution Prevention Plan (SWPPP) to reflect this
change.
5. The current stormwater permit was available for review. All elements of the SPPP are being
implemented. Qualitative monitoring results were available for review. There was no discharge at

Inspection Summary:

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.

A copy of a Technical Bulletin for the general permit
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.
