On June 18, 2012, Cheng Zhang of the Raleigh Regional Office (RRO) conducted a Stormwater
Compliance Evaluation Inspection at the subject facility. The assistance provided -by Mr. Mike
Owens, the zone operation manager, was greatly appreciated. The inspection report is attached.
Findings during the inspection were as follows:
1. Certificate of Coverage (COC) No. NCG080749 of NPDES general permit .
NCG080000 was issued Henderson Bins to discharge stormwater runoff to Devil's
Cradle Creek, a Class-B NSW stream, in the Tar -Pamlico River Basin.
2. The facility has developed and implemented a Stormwater Pollution Prevention Plan
(SPPP). A review of the plan indicated that all required components were included
and required activities well documented.
3. Qualitative monitoring was conducted at the two stormwater discharge outfalls semi-
annually. Analytical monitoring is not required because most vehicle maintenance is
conducted off site.
4. Both stormwater outfalls were observed during the inspection and adequately
maintained.

In accordance with your application for a discharge permit received on October 5, 2009, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state — NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection Agency dated October 15, 2007 (or as subsequently amended).
Please note that 1) the adjacent transfer station is not covered under this permit; 2) exposed
areas of the transfer station are to be addressed such that sotrmwater does not commingle with
leachate or residuals from offloading and truck traffic; and 3) if concerns at the transfer station are not
or cannot be addressed to the Division of Water Quality's satisfaction, additional permitting may be
required.
This certificate of coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources, Coastal
Area Management Act or any other federal or local governmental permit that may be required.

1. On 24 January 2007, Ron Boone, of the NC Division of Water Quality (DWQ), Raleigh Regional Office (RRO),
conducted a compliance evaluation inspection at the subject site to determine your compliance with the subject
permit. Your assistance and cooperation was helpful and appreciated. An inspection report is attached for your
records and inspection findings are listed below. It should be noted thatthis permit was issued in March of 2006.
Prior to that date the site was operated without a permit.
2. There is a stormwater pollution prevention plan (SP3) on site. The SP3 contained most of the required
components, however one item that remains in question is the number of stormwater discharge outfalls (SDO) at
the site. Although the SP3 identifies two SDOs, a walk around the premises during the inspection revealed that
there are at least three separate outfalls, and likely more. The actual number of outfalls is in question because of
the runoff of stormwater at multiple points along the northern, western and southern edges of the property. Each
SDO must be positively identified and then documented on the general location map component of the SP3 with
accurate latitudes/longitudes. The outfalls are the points at which both analytical and qualitative monitoring must
be accomplished. Please take special note of the definition of "point source discharge of stormwater" in Part VI,
paragraph 19, while making the determination of how many SDOs exist at your site.
3. As of the date of this inspection, analytical monitoring had not been completed for the year 03/2006 through
03/2007. Please note that the initial analytical monitoring must be accomplished within the first year (12 months)
of permit issuance and that all analytical testing must be done by a certified laboratory. The only exception to this
is the pH test; this test may be run on site to avoid conflicts with sample holding times. Refer to Part II, Section B
of your permit. Please also note that all documentation for analytical monitoring must be maintained on site.
4. As of the date of this inspection, no qualitative monitoring had been accomplished for the year 03/2006 through
03/2007. Please note that qualitative monitoring must be accomplished twice per year, once in the spring and once
in the fall. Refer to Part II, Section C of your permit. Please also note that all documentation for qualitative
monitoring must be maintained on site.

5. As of the date of this inspection, no facility inspections had been accomplished for the year 03/2006 through
03/2007. Please note that, according to Part II, Section A, Paragraph 8 of your permit, such inspections must be
accomplished twice per year, once in the spring and once in the fall. Please also note that all documentation for
this inspection program must be maintained on site.
6. The best management practices (BMP) for the handling of automotive wastes such as gasoline, brake fluid, engine
oil, transmission fluid, batteries, etc., were not clearly understood by the inspector. Several BMPs were identified
in the SP3 as existing or proposed (feasibility study), but these BMPs were either not yet implemented (proposed
only) or the conditions at the site did not match what the BMPs indicated would be done. Additionally, your
verbal. explanation of how gasoline and batteries are handled did not match site conditions either. Although only a
few batteries were seen around the yard, there were several piles of vehicle gas tanks that had already been
removed from vehicles in preparation for crushing. Several of these tanks had fluids in them that smelled very
strongly of gasoline. The way the tanks are piled up around the site, it would be very easy for the gasoline to leak
out onto the ground. Please note that every time such a spill of gasoline, or other concerned automotive fluids
occurs, constitutes a violation of NC general statute 143-215.75, and is punishable by a fine of up to $5,000 per
day per violation. We strongly encourage you, as the owner of Young's Auto Center and Salvage, to read through
your permit very thoroughly and feel free to ask our inspectors to clear up any questions you may have. Your
complete familiarity with, and understanding of the permit is imperative to best assure compliance.
7. There were several proposed BMPs in the SP3. It is also highly suggested that you seriously consider
implementing these BMPs as well.
8. Please be aware that the above -mentioned issues constitute violations of your stormwater discharge permit and that
such violations may result in the assessment of civil penalties in an amount of up to $25,000 per day per violation.
Please provide a response to the issues identified above within 30 days of your receipt of this letter. Your response
must include a narrative as well as a tentative schedule for you to correct the identified issues. If you or your staff

Inspection Summary:
Permittee does have a SP3 now with the required components but not all components have been fully implemented.
Permittee has identified 2 outfalls from the site when there are definitely at least three outfalls, and possibly more.
Permittee has not yet conducted qualitative monitoring as required by the permit. Several practices are still in question,
such as how the permittee handles fuel tanks on cars received and the leftover fuel in them. Written procedures and what
permittee says they do does not mesh well with appearances at the yard. Permittee must write all procedures for each
waste and/or potential waste, incorporate the written procedures into the SP3, train all employees on them and then
ensure each is fully implemented.

In accordance with your application for a discharge permit received on March 29, 2007
we are forwarding herewith the subject certificate of coverage (COC) to discharge under the
subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended).
It was noted in both your application and a site visit by Mr. Ron Boone, of the Raleigh
Regional Office, that truck washing activities currently occur onsite. It is the Division's
understanding that the current vendor for this activity collects the washwater and disposes of it
appropriately offsite. This wastewater discharge will need to continue to be disposed of
appropriately.
If any parts, measurement frequencies, or sampling requirements contained in this permit
are unacceptable to you, you have the right to request an individual permit by submitting an
individual permit application. Unless such demand is made, this certificate of coverage shall be
final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal

On September 9, 2014, Dave Parnell, of the Raleigh Regional Office of the North Carolina
Division of Energy, Mineral and Land Resources (DEMLR), conducted a compliance evaluation
inspection (CEI) at the above referenced facility. The site lies in the watershed of an unnamed
tributary (UT) to Little Buffalo Creek, Class C waters, which lies in the Cape Fear River Basin.
The following observations were noted during the NCG080668 DEMLR inspection:
This facility is a vehicle maintenance facility for the City of Sanford government vehicles.
Thank you for the assistance of Randy Paschal and Michael Lamping during the inspection, it
was appreciated. The inspection began in the Randy's office, where a thorough review of the
Stormwater Pollution Prevention Plan; as well as, the qualitative and analytical monitoring
results, was conducted. All aspects of the Stormwater Pollution Prevention Plan are being
implemented. Analytical and Qualitative monitoring data were reviewed and found to be in
order and compliant with the NPDES Stormwater Permit.
Following the monitoring data review, secondary containment and the Stormwater Discharge
Outfalls (SDO) were observed. SDO #A has the potential of having elevated levels of total
suspended solids, due to areas of exposed soil, which stormwater passes over - prior to .
concentrating at the sample point. The staff was knowledgeable, with respect to the sample
site and sampling techniques.

On March 1, 2011, Thomas Ascenzo of the Raleigh Regional Office (RRO), conducted a compliance inspection of
the City of Sanford's Public Works Service Vehicle Maintenance Facility. The help of Randy Paschal and Michael
A. Lamping was greatly appreciated.
The following observations were made:
1. The Public Works Service Vehicle Maintenance Facility discharges stormwater through two outfalls into
an unnamed tributary of the Little Buffalo Creek within the Cape Fear River Basin.
2. Review of the facilities Stormwater Pollution Prevention (SP3) revealed the following:
• The plan contained the required general location map that included outfall locations and
drainage area, narrative description of practices, spill response plan.
• Employee training documents were included in the plan. However no evidence of annual
training is being conducted for spill response. Advised to conduct employee annual employee
training and document attendants on the form and include it in the notebook.
• A sheet is included for spill reporting. If no spills are noted through the year, add a line on the
reporting sheet to state the dates that no spills were reported ie: From the period of Jan 2010-
Dec- 2010 No spills reported date and sign when entered.
• A list of responsible parties for Emergency spill response notification is included in the plan
however it needs to be updated annually.
• Essentially all the components of the plan were included in the notebook however no evidence
that the plan is reviewed and updated annually. Suggestion to add Analytical and Qualitative
documents to the SP3 manual instead of being located in Town Hall.
Outfall A was observed to be dry.
4. Outfall B was observed to be clear and free from pollution.
5. Suggestion to add a signs to mark the locations of the outfalls.

6. Review of analytical data revealed on July 13, 2009 a PH reading was below benchmark for Outfall A.
Please conduct Tier 1 or Tier 2 investigation to determine the cause of the reading to be out of range.
These investigations must be documented and added to the SP3 manual.
7. Suggestion to include analytical data along with chain of custody in a separate tab in the SP3 manual.
8. Qualitative Monitoring is being conducted as observed with the documents reviewed during the
inspection. Add a tab in the SP3 manual.
9. Assign a person to update and review the Stormwater Pollution Prevention Plan (SP3) annually

On November 18, 2005 I conducted a stormwater compliance evaluation inspection of the City of Sanford's Public Works Service
Vehicle Maintenance Facility. Randy Paschal, Larry Bridges, Brent Johnson, and Trevor Hirt represented the City during the
inspection. Their aid was greatly appreciated.
The Public Works Service Vehicle Maintenance Facility discharges stormwater through two outfalls into an unnamed tributary of the
Little Buffalo Creek within the Cape Fear River Basin. At the time of the inspection, no flow was observed at either outfall. Both
outfalls appeared to be well maintained.
The facility's site map, stormwater management plan, and spill prevention and response plan were present and complete. I remind
you that these documents should be internally reviewed annually to insure that they are up to date with current facility conditions.
According to Mr. Paschal, employee spill response training occurs during the monthly safety meetings. All training should be
documented and maintained with other permit records for at least five years.
Semiannual qualitative inspection records were available for review and appeared complete. The inspection for fall 2005 had not been
conducted at the time of the inspection.
All vehicle maintenance is performed within the service bays. Wash water from the carwash area is directed through a sediment trap
prior to discharge. The facility has a vehicle fueling area that contains two storm drains. I recommend that the City purchase storm
drain mats to cover the drains during the filling of the underground storage tanks in order to prevent any spill from reaching surface
waters. These mats can be purchased from several companies including Fisher Scientific, Lab Safety Supply, Upright Inc., Interstate
Products Inc., and New Pig Corporation. I caution that the this list of suppliers is incomplete and does not imply an endorsement of
these companies by either DWQ or myself.
The facility was found to be compliant with the stormwater permit. If you have any questions or comments concerning the

Mitch Hayes of the Division of Water Quality conducted the subject inspection on May 31, 2011. Your time
and assistance with this inspection was greatly appreciated. A Basinwide Information Management System check
list of the inspection is attached to this letter. Below is a summary of findings and recommendations developed
from the inspection.
1. The subject permit was reissued and became effective November 01, 2007 and will expire October 31,
2012. You stated that you mailed your permit renewal this month. Please continue to operate under the
last issued permit until a new permit is issued.
2. The facility dismantles automobiles for scrap and parts. Scrap cars are spread out over an
undetermined amount of acres. All cars that are brought in are drained of fluids, batteries and tires
removed. Used oil is stored in drums and burned in a clean burn heater. Other fluids are stored in
barrels and taken to be recycled. Batteries are sold, traded as core for the purchase of new batteries.
Safety - Kleen recycles all fluids.
3. The site plan lists 2 Stormwater Discharge Outfalls (SDO's). SDO # 1 is located at the northeast corner
of the property; SDO # 2 is at the southwest corner of the property. It appears that SDO #1 is a
driveway drainage ditch that drains stormwater off the main road. This outfall does not have anything in
common with activities covered in this permit. You may delete this outfall and update your site map to
reflect this change.
4. The Stormwater Pollution Prevention Plan (SPPP) was reviewed for compliance. The SPPP is well
written and contain all permit requirements. Analytical and qualitative monitoring is currently being
conducted at SDO # 2. Analytical and qualitative results were up to date. Analytical results were below
the benchmark values. All other parameters in the SPPP are updated and documented.

On August 17, 2012, Mitch Hayes with the Division of Water Quality conducted a
compliance evaluation inspection on the subject facility. The assistance of James Batchelor,
dispatcher and Tony Taylor with this inspection was greatly appreciated. Below is a list of findings
and recommendations developed from the inspection:
The Certificate of Coverage under General Permit NCG0800000 was issued November
01, 2007 and will expire October 31, 2012. Renewal applications were mailed out on
March 05, 2012.
2. The facility consists of an office, truck shop, truck maintenance area and various storage
buildings. The parking lot is gravel with several storm drains located though out the lot. It
appears that all storm drains in the parking lot flow into the City of Wilson Municipal
Separate Stormwater Sewer System (MS4). According to Mr. Taylor, the facility uses
more than 55 gallons of new motor oil per month when averaged over a year. Mr. Taylor
stated that all used oil is burned for heat during cold months and is supplemented with
kerosene. Used motor oil and kerosene are kept in separate above ground storage
tanks inside a concrete block secondary containment area. The secondary containment
area has a gate valve that drains water out as needed. No staining was noted. Used
antifreeze is kept in drums and recycled by Safety Kleen as need.
3. The stormwater permit NCG080287 could not be found for the inspection. According to
Mr. Batchelor, no one has any knowledge of the stormwater permit or the requirements
to monitor stormwater discharge(s) from the property. A Stormwater Pollution Prevention

Plan (SPPP) was not available. This is a violation of the permit NCG080287 Part II
Section A, for failure to develop and implement a SPPP. The facility must develop and
implement a SPPP within 90 days receipt of this letter. The facility is in violation of
permit NCG080287 Part II, Section B for failure to conduct analytical and qualitative
monitoring at all identifiable discharge outfalls. Monitoring must be conducted before
October 31, 2012.
Permit noncompliance constitutes a violation of the Clean Water Act and is subject to a
daily civil penalty of not more than twenty-five thousand dollars ($25,000) per violation. Please
respond in writing to this office addressing the items noted in item 3 within 30 days receipt of this
