the Organization name on the COC is not affected, we will work through the legal owner name correction
independently.
Did the company submit a Name Change form sometime in the past to have Harborlite Corporation changed? If that
was never done, we might need you to submit that paperwork to us. There's a form for'Name/Ownership Change' on
our website. If that was already done, please let us know about when, and we'll investigate more as to why it wasn't
done on our end.

So, the new COC was correct, but the webpage information was not?
We're crunching on the several parts of our new on-line renewal procedures right now, and for the next couple of
days. We will be able to make these changes at the end of that activity. I'm not sure why the name change didn't
take. Give us a couple of weeks to sort this out.

Hello Ken,
I participated in the online stormwater permit renewal service this year and printed off the COC today. Before printing the
COC, it asked me to verify the information which is when I noticed that it contained our old legal name "Harborlite
Corporation". This, field was locked and it would not allow me to change it. I've attached the fetter of correspondence
regarding our legal name change from July of last year for your reference. It appears the name change didn't cant' over
to the new permit?

On. December 12, 2008, I, Dave Parnell of the Raleigh Regional Office of the Division of Water Quality,
performed a compliance evaluation inspection at your perlite producing company. Brandon Hedgecock,
Plant Manager, was available for the inspection. His assistance was appreciated. This inspection was
conducted to determine Harborlite Corporation's compliance with its NPDES stormwater permit.
Observations during the inspection are as follows:
1. The current permit was available on site. It was issued June 1, 2008 and expires on May 31, 2013.
2. Harborlite Corporation operates under standard industrial classification code (SIC) 3295 (Minerals
and Earths, Ground or Otherwise Treated) and manufactures expanded perlite products.
3. Stormwater discharges from the site to receiving waters designated as an un-named tributary to
Smith Creek and Richlands Creek, a class C, nutrient sensitive water (NSW), in the Neuse River
Basin.
4. All five stormwater discharge outfalls (SDO) were identified. Little or no flow was observed
during the inspection. A series of sediment ponds and check dams are in place to settle out any
solids that may escape from the property. None of the perlite material had made its way into the
waters of the state. Care should be taken to ensure that none of the product leaves the property
through the stormwater discharge outfalls.
5. The stormwater pollution prevention plan (SPPP) for the site was reviewed and was complete. A
general location map and site map identifying all SDO, a record of spills, training rosters, and
emergency contact information were on file. Records of internal and corporate audits of the SPPP

Mr. Chad Coburn and Ms. Vicki Webb of the Division of Water Quality, Raleigh Regional Office,
conducted an compliance evaluation inspection of the Youngsville plant for coverage under the NPDES permit
NCG070149 on July 6, 2006. All assistance during the inspection was greatly appreciated. Findings during the
inspection are as follows:
1) The current permit was issued May 1, 2003, and expires on April 3, 2008.
2) Harborlite Corporation operates under standard industrial classification code (SIC) 3295 (Minerals
and Earths, Ground or Otherwise Treated) and manufactures expanded perlite products.
3) Stormwater discharges from the site to receiving waters designated as an un-named tributary to
Smith Creek and Richlands Creek, a class C, nutrient sensitive water (NSW), in the Neuse River
Basin.
4) All five stormwater discharge outfalls (SDO's) were identified and discharge flow was observed
during the inspection. A series of sediment ponds and check dams are in place to settle out any -
solids that escape from the property. There was a large amount of the perlite product on the grounds
of the site, and in the first sediment basin going to the third outfall. Also, a small amount of perlite
was observed in the next sediment basin behind the check dam. While the perlite product is
basically inert in nature and doesn't appear to present any biological risk to wildlife (according to the
material safety data sheets), it is still considered a pollutant if it enters surface waters. Care needs to
be taken to ensure that the perlite product doesn't leave the property via the stormwater discharge
outfalls.
5) The stormwater pollution prevention plan (SPPP) for the site was reviewed and found to be in
excellent order. A general location map and site map (both to scale) identifying all SDO's, a copy of
the general permit, records of spills, training rosters, emergency contact information, certification

potential pollutants. In addition, records of internal and corporate audits of the SPPP were on file.

Your facility is currently covered for stormwater discharge under General Permit NCG070000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result.in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 11 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
