Resources, performed a stormwater Compliance Evaluation Inspection CCEI) for Cormetech,
Incorporated, on July 7, 2014. Your assistance and cooperation was helpful and appreciated.
The inspection checklist is enclosed for your review. During the inspection the following
items were noted:
1. The current permit was available on site. It was issued June 1, 2013 and will expire on
May 31, 2018.
2. Cormetech, Incorporated operates under standard industrial classification code (SIC)
3299 manufacturing catalytic filters, for use in natural gas power plants.
3. Stormwater discharges from the site to receiving waters designated as an un-named
tributary to the Little River, a class WS-IV, nutrient sensitive water CNSW) stream in
the Neuse River basin.
4. The current two stormwater discharge outfalls (SDO) were identified and observed.

No flow was evident during the inspection.
5. Secondary containment was observed during the site visit and was compliant with
permit requirements.
6. The stormwater pollution plan (SPPP) for the site was reviewed. The plan met all
requirements of the permit.
The implementation of your stormwater program is commendable, and the Department is

Dear Mr. Pamell:
This letter is in response to the findings during your Compliance Evaluation Inspection (CEI) on January
9, 2009. A description of the actions taken and the planned schedule to correct the deficiencies stated
in item #5 are included.
A Stormwater Facility Inspection Plan for the semi-annual qualitative monitoring was written on
1/20/09 and added to the Stormwater Pollution Prevention Plan (SWPPP). The inspection plan
includes a description of how and where to inspect the plant stormwater discharge outfalls
(SDO). Specific references are made to find the paragraphs in the permit where the
requirements are located. All of the required inspections have been completed to date. This
item is complete.
2. After checking with the personnel in charge of the Stormwater Pollution Prevention Plan in 2006
and 2007 it was verified there were no significant spills or leaks of pollutants during those years.
A list for each of those years was added to the SWPPP on 1/14/09. A list for 2008 was already
in the SWPPP. There is now a list showing no significant spills or leaks for the last three years
in the SWPPP. A new list will be updated each subsequent year. This item is now complete.
3. The Non-Stormwater Discharge Assessment and Certification form was completed, signed and
added to the SWPPP on 1 /28/09. This form will be completed annually. These observations
were being made and noted during the qualitative monitoring inspections in the past. Since the
inspections now must be completed during a rain event, a separate inspection will be made
during a dry period to observe any non-stormwater discharges. This item is now complete.

I, Dave Parnell of the Raleigh Regional Office of the Division of Water Quality, performed a
stormwater Compliance Evaluation Inspection (CEI) for Cormetech, Incorporated, on January 9,
2009. The assistance and cooperation of Mr. Dick Harlow, Environmental Engineer, was helpful
and appreciated. The inspection checklist is enclosed for your review. During the inspection the
following items were noted:
1. The current permit was available on site. It was issued June 1, 2008 and will expire on
May 31, 2013.
2. Cormetech, Incorporated operates under standard industrial classification code (SIC) 3299
manufacturing catalytic filters, for use in coal power plants.
3. Stormwater discharges from the site to receiving waters designated as an un-named
tributary to the Little River, a class WS-IV, nutrient sensitive water (NSW) stream in the
Neuse River basin.
4. All three stormwater discharge outfalls (SDO) were identified. Little or no flow was
observed during the inspection. The flow for SDO#2 will be diverted through an existing
stormwater structure and monitored at the outlet of the device. SDO#1 and SDO#3 are
monitored at a point where the drainage ditch daylights to the woods.
5. The stormwater pollution plan (SPPP) for the site was reviewed. The plan contained
most of the requirements of the permit. However, the following deficiencies of the
permit were discussed with Mr. Harlow: A more complete list of significant spills,
consisting of the last three years, should be place in the SPPP. A Stormwater Facility
Inspection Plan will need to be developed and place in the SPPP for implementation.

Your facility is currently covered for stormwater discharge under.Generai Permit NCG070000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 200_ in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirminL
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215A
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial ,
facilities in any of the I 1 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed

to receiving waters designated as the Little River, a class WS-IV NSW stream in the Neuse River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, H, III
and IV of General Permit No. NCGO70000 as attached.
This Certificate of Coverage shall become effective August 11, 1995.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

An IMS error has been identified during the Stormwater units current effort to renew our 07 general
permits. An 03 general permit that was changed to an 07 permit back in August of 1995 was not
removed from IMS (See attached letter). Could you remove NCG030217 from IMS and notify me
when the deletion is completed, so that I can dose this matter out ?

Due to Cormetech's SIC code change from 3829 to 3299, as stated in your August 4,
1994 letter, the type of general permit required by the facility has changed. In accordance with
this change we are forwarding herewith the modified Certificate of Coverage page along with the
new stormwater permit for the subject facility. This Certificate of Coverage is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U. S. Environmental Protection Agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this
permit are unacceptable to you, you have the right to request an' individual permit by
submitting an individual permit application. Unless such demand is made, this certificate of
coverage shall be final and binding. Please take notice that this certificate of coverage is not
transferable except after notice to the Division of Environmental Management. The Division of
Environmental Management may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Environmental Management or permits required by the Division of
Land Resources, Coastal Area Management Act or any other Federal or Local governmental
permit that may be required. If you have any questions concerning this permit, please contact
