On October 15, 2013, Dave Parnell and Gabi Jones of the Raleigh Regional Office of Land Quality
— Stormwater Program, performed a compliance evaluation inspection (CEI), at the referenced
facility. Your and Jerry Frank's assistance was helpful and appreciated. This inspection was
conducted to determine your compliance with the NPDES stormwater permit referenced above.
The following observations were noted during the inspection:
SITE DESCRIPTION
This facility is a glass recycling plant. Used glass materials are brought to the site, sorted, then
ground down to a fine dust -like product that is used in the striping of highways. There are three
stormwater discharge outfalls (SDO) at the facility, which collect stormwater from the access and
production areas of the site. The SDO discharge to an unnamed tributary of Middle Creek, in the
Neuse River Basin. The SDO are located on the northwestern side of the property and discharge
after significant rainfall events. Vehicle maintenance is not conducted at this facility; and therefore,
no analytical monitoring is required by the permit at this plant.

INSPECTION SUMMARY
The NPDES General Stormwater Permit was available during the inspection. The permit expires
on May 31, 2018.
2. The Stormwater Pollution Prevention Plan (SPPP) was available, reviewed and found to be
compliant with the requirements of the permit. Please provide a USGS location map within the
plan. The facility updates their SPPP annually and includes a list of significant spills and employee
training within the plan.
The SDO and secondary containment areas were visited during the inspection and were found to be
adequate and compliant with the permit.. Qualitative Monitoring has been conducted as required by
the permit

Inspection Summary:
This facility was in compliance with the requirements of the permit. The facility is ISO 14001 certified and maintains
excellent records and provides excellent environmental and safety training to employees.
The facility has a pretreatment permit with Apex WWTP and sends all process wastewater to the WWTP. The facilty
provides filtration and pH adjustment prior to discharging to the WWTP and a composite sampler is located on -site to
meet the quarterly sampling requirements of the WWTP.
An update is needed for the facility owner and personnel as follows: Please complete the forms attached and return to
DWQ for update.
The owner is Peak Investment, and the owner listed in BIMS permit is currently Jeffery Franks, which I understand should
be Jerry Franks,
The Vice President of Operations at the home office is Jim Haywood, 610-651 -4700.

Comment: There are three stormwater outfalls at the facility. Outfall 001 receives rain
water from the parking lot and roof drains. There was no discharge observed the day of
the inspection. Outfall 002 receives runoff from the warehouse and product storage
area. This outfall was flowing slightly, the discahrge was clear. Flows through
conveyance ditch approx. 75 feet to the creek. The ditch is cleaned periodically and is
in need of cleaning now. The sediment should be removed from the ditch to prevent
sediment carryover to the creek. Outfall 003 was difficult to reach, it requires travel up
the the creek, and due to water levels in the creek the day of the inspection was not
observed. It was recommended that land access be considered from the back of the
property through the wooded area that could be cleaned enough to provide a path to the
outfall. If this is feasible, it would provide a safer and more convenient access to the
outfall for inspections.

Your facility is currently covered for storrwater discharge under General Permit NCG070000. This permit expires
on March 31, 2003. The Division stall is currently in the process of rewriting this permit and is. scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility -would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you im
advance that your permit coverage will he expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2. 2002 in ot-der to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of'
the completed application will not he sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed

is herby authorized to discharge stormwater from a facility located at:

WAKE COUNTY
to receiving waters designated as Middle Creek in the Neuse River Basin
in accordance with the effluent Limitations, monitoring requirements and other conditions set forth in Parts I,
11, 1II and IV of General Permit No. NCG070000 as attached.
This Certificate of Coverage shall become effective May 1, 1998.
This Certificate of Coverage shall remain in effect for the duration of the General Permit_

In accordance with your application for discharge permit received on September 25, 1992, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
