A hard copy original has also been mailed out. I have also attached a Stormwater Permit Owner Affiliation Designation Form to be completed and submitted to our Central Office in Raleigh. According to our permit database, we do not have a named affiliated owner (person legally responsible for the permit). This should be an officer of the company (Vice President or higher) or their designee. In reviewing the facility's SPPP, there is a document signed by the Vice President of the company designating the Plant Manager as having signature authority and responsibility for the stormwater permit. I understand that Chris Miller is the current Plant Manager therefore he should be listed on the owner affiliation designation form. Our permit database does list R. Phillip Waters as the Facility Contact Person and Permit Contact Person (see attached screenshot from our database). I appreciate everyone's time and assistance. A copy of the Compliance Inspection Report is enclosed for your review. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit NCG070000 under Certificate of Coverage NCG070007. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated as Shoe Heel Creek (Big Shoe Heel Creek)(Maxton Pond), a Class C;Sw waterbody in the Lumber River Basin. As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG070000 permit. Please refer to the -enclosed Compliance Inspection Report for additional comments and observations made during the inspection. Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil penalty assessment of up to $25,000 per day for each violation. Mr. Smith provided the facility's Stormwater Pollution Prevention Plan (SPPP), detailed site map and monitoring records for review. Facility's SPPP appeared to contain all permit -required components and is reviewed and updated annually. Records of semi-annual Qualitative (visual) Monitoring from 2013 through 2017 were reviewed and contained all permit -required information. Facility has identified and monitors ten stormwater discharge outfells. After the records review, observations were made of site conditions including storage areas and the stormwater discharge outfalls with the assistance of Mr. Barber, At the time of inspection, facility appeared to be well operated and maintained and in compliance with the NCG070000 permit. The NPDES permitting program for stormwater discharges was established in 1990. Phase I focuses on site and operations planning to reduce pollutant sources. Through the NPDES permitting program industrial facilities that fall into one of the subject ten categories are required to obtain permit coverage under a general permit or an individual permit, depending upon the facilities SIC code and the industrial activity occurring at the facility. There are currently 19 active general stormwater permits in North Carolina. General Stormwater Permit Certificate of Coverage No. NCG070007 Non-Stormwater Dischar a Notification Dear Belinda: DENR-FRO JAN 2 6 2005 This letter is a written follow-up to a phone conversation with you on January 19, 2005 concerning the non-stormwater connections found in the 006 stormwater outfall and a phone conversation with you on January 21, 2004 regarding the chromate release that occurred on January 20, 2004. Chromate Release At about 3:45 pm on Thursday, January 20, 2004, a ruptured chromate cooling water supply line was detected on the North side of the plant from the non -operational Line #1 air pollution control system. Personnel and contractors on -site worked very quickly and had the leak stopped and the spill contained within 10 minutes. The cooling water was leaking at a rate.of about 100 gallons/minute (visually estimated). There is a stormwater catch basin in that area that discharges to stormwater outfall S002. Site personnel sealed off the catch basin within about 5 minutes of detecting the spill. It is estimated that a total of about 1000 gallons of cooling water was released with about half of that (500 gallons) reaching the stormwater catch basin. The cooling water contains about 400 ppm of sodium chromate. The total amount of sodium chromate released was about 3.336 lbs. The estimated amount of sodium chromate that drained into the stormwater catch basin is about 1.67 lbs total. The portion of the release that did not go into the catch basin was contained on the concrete below the cooling water pipe. The spill on the concrete was absorbed with clay absorbent and shoveled into drums for proper disposal. The remaining liquid in the cooling water pipe was drained, captured and returned to the cooling water hot well system. The pipe will remain empty since the system it was servicing is shut down. Box 969 — Laurinburg, North Carolina 28353 Tel (910) 276-5630 Non-Stormwater Connections to S006 During an internal inspection of the stormwater outfalls, it was noted that there was dry weather flow in Stormwater outfall S006. PNA retained the services of a contractor with a camera to detect the source of the flow in the 48" concrete pipe. The camera picked up apparent gourndwater leakage around the joints of the pipe and around the outside of incoming sewer connections. Based on the amount of the leakage observed, it appears that it accounts for the majority of the flow seen at the discharge end of the pipe. During the course of this investigation it was further found that there were some connections to the sewer pipe that were not depicted on the drawings. These connections were traced and they appear to be coming from the drains in the battery storage room, the discharge from the oil/water separator that services the truck wash pad, two hand washing sinks, and two basement sumps. Battery washing operations in the battery room were stopped immediately and alternative methods of cleaning the batteries are being used. PNA has gotten proposals to disconnect and re-route the processes described above to the wastewater treatment pond. Re-routing of these sewers involves the installation of a new lift station. PNA plans to begin the sewer construction activities as soon as the lift station pumps are received. The target date for completion of these activities is March 31, 2005. Due to the unexpected findings as described above, PNA is verifying routes for all other processes on the south side of the plant to ensure they are connected to the proper sewers. At about 2:15 pm on Thursday, December 4, 2003, A truck driver ran over a jack stand, causing the truck's diesel tank to rupture onto the truck pad. Upon detection of the release a can was placed under the leak to prevent further release onto the truck pad. Due to heavy rains occurring at the time, some of the diesel fuel was washed into a storm drain which disharges via stormwater Outfall 002 to an unnamed tributary to Big Shoe Heel Creek, An outside cleanup company was called in to assist with the cleanup of the remaining diesel fuel. A total of about five gallons of diesel fuel was released. The tank was patched and the leak stopped by 4:30 pm. PNA has initiated a detailed accident investigation to determine the root cause of the incident and will be following up with the trucking company concerning operating practices. Safety glasses are to have rigid side shields attached to the frame. Approved eyewear that will fit over prescription glasses will be provided to visitors upon request. Tinted glasses or sunglasses are not to be worn inside the plant. Foot Protection Impact and compression resistance shoes (i.e., steel -toe) which meet ANSI Z41-1991 standards are required to be worn when on production floor. Visitors and contractors who do not wear ANSI approved foot protection will be required to remain in "approved" walkways. Head Protection Hard hats which meet ANSI Z89.1-1986 standards are required to be worn in areas where glass is stored, below cranes, and any other "designated areas." Hearing Protection Hearing protection is required in designated areas, such as: boiler room, wareroom, and lehr. Designated hearing protection areas are marked with appropriate signs. Hair While on the production floor, hair longer than the top of the shoulder must be secured in a manner that keeps the hair above the shoulder (i.e., pinned up or beneath a cap). Pony tails/braids must also be secured. Beards greater than 2 inches in length must be bound close to the head. Jewelry The wearing of jewelry is prohibited while on the production floor. The only exceptions are medical alert bracelets when walking in "approved' walkways. Body Protection Appropriate work clothing shall be worn. Tank tops, shorts, or sleeveless shirts/blouses, loose sleeves, tails, ties, lapels, or other loose clothing which can be entangled in moving machinery shall not be worn. Fall Protection Contractors are required to wear and secure full body safety harnesses while working in aerial lifts, or when working from unprotected work surfaces over 6 feet high. Lockout/Tagout Contractors are required to follow Pilkington's LOTO program. Direct questions to the project manager. Hot Work Contractors are required to follow Pilkington's hot work program. Certain portions of the facility require a permit. Questions should be directed to the project manager. Confined Space Contractors are required to follow OSHA's confined space safety program. Questions should be directed to the project manager. Smoking Smoking limited to designated areas only outside facility. Severe Weather • If severe weather alarm sounds, proceed to nearest shelter area as identified severe weather signs. Signage will typically be located at stairs going down into tunnels or locker rooms. • Wait for a headcount to be taken Emergency Evacuation • Proceed to nearest evacuation point located on map (identified by white signs that say assembly point) or designated by your respective contracting firm. • Wait for a headcount to betaken Medical/Fire Emergencies • In the event of a true emergency, contact the bath control room at 2270 or main guardhouse at 277-2237 from a Plant phone and the ERT will be dispatched to the emergency location. (Remember minor injuries are not emergencies and should be reported to your immediate supervisor.) Aluminum Products Aluminum products including aluminum soda cans, food cans, aluminum foil are prohibited from the premises of the facility. Aluminum contamination is a very serious issue to Pilkington and violators will be asked to leave the worksite immediately. Radios Radios or stereos are NOT allowed in areas where hearing protection is required (above 85 dBA). "Walkman" type headsets are not allowed anywhere within the facility. Personal Protection Equipment (PRE) PPE guidelines are posted throughout the facility. Kevlar cuffs are to be worn when handling untempered glass. Gloves that have been designated by the department are to be worn as well. The proper use of PPE is the responsibility of every visitor. Questions should be directed to your host. Hi-Vis is also a requirement throughout the facility. Accidents, near misses, and first aid Visitors, contractors, and truck drivers are required to report all accidents, near misses, and first aid incidents to the area supervisor. The area supervisor will coordinate an incident investigation if necessary. Asbestos This facility has asbestos containing materials (ACM). ACM is labeled and is not to be disturbed without approval. Safety Signs & Traffic Rules All safety signs must be observed. Obey all traffic signs including speed limits. Do not block fire suppression equipment such as fire hydrants. Yield to pedestrians. Housekeeping Good housekeeping is expected to be practiced by all visitors. All trash is to be placed in proper waste receptacles and not in glass cullet bins. Cell Phones Cell phones are not to be used in or on any moving vehicles, fork lift trucks, personnel carriers (including bicycles). Cell phone use is not allowed anywhere within the facility except for break rooms and offices. Cell phone use within the perimeter of the fence is limited to designated smoking and break areas. NSG Group Energy Policy Glass manufacture is an energy intensive process and results in the emission of greenhouse gases (mainly COZ). Many of the products manufactured by NSG are specifically intended to either reduce energy consumption by their use or are components of energy generation systems. Our policy is to maximize the efficient use of energy whilst minimizing polluting emissions. It is designed to support our commitment to continually minimize environmental impact whilst ensuring sustainable economic operation. Energy efficiency is an integral part of our product program both from a manufacturing and product performance viewpoint. We will utilize internal NSG and/or externally verified energy management systems to ensure that we are delivering against the policy objectives and performance targets. • As part of our own R&D and design engineering program we will actively evaluate economically competitive alternative energy sources and technologies to reduce dependence on fossil fuel resources. • We will work with supply chain partners to innovate our logistics arrangements to improve the environmental impact of our products. • We will promote the use of alternative communication strategies to limit the impact of energy consumption associated with business travel. • We will support our customer's efforts to reduce the overall energy impact of their own processes and improve the energy performance of their own products. • We will work with our suppliers to encourage them to improve energy efficiency, and the cost of the energy consumed by products purchased shall be considered over the expected operating lifetime during the technical tender evaluation. • We will comply to all relevant energy and emission legislation and will work with trade associations and public bodies to actively support the development of future legislation. • We will support our employees and contractors with appropriate training and necessary information to enable them to appreciate the significance of their involvement and also assist our suppliers and customers to understand their own contributions.
