state NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice
to the Division of Energy, Mineral and Land Resources. The Division may require

This is the facility that I spoke with Ken about in regards to whether or not they would be agriculturally exempt. I think
he discussed with you and it was determined that they would be subject to permitting. I spoke with the owner in early
May about the stormwater permitting requirement, SWPPP development and monitoring. I e-mailed him a copy of the
NO1 with instructions to complete and submit within 30 days. Glad to see he followed through. Please move forward
with issuance of permit coverage and please provide our office with a copy of the issuance letter and COC.

Thanks for speaking with me today regarding the need for stormwater permit coverage for the Southeastern Grain
Company facility located on Westover Road in Clinton. I have attached a copy of the Notice of Intent application form to
obtain coverage under the NCG060000 General Stormwater Permit based on your facility's suspected Standard
Industrial Classification (SIC) Code of 4221. Please submit your completed application within 30 days. Failure to obtain
permit coverage for stormwater discharges could result in the issuance of a Notice of Violation, which is subject to the
assessment of a civil penalty of up to $25,000 per day, per violation.

I am forwarding you an e-mail sent tome on Friday from the DOT Division 3 DEO, Stoney Mathis.__Apparently, workers
on the� _ NC 24 project, noticed a foul stench in the drainage system rthe',inter eection,of NC. 24.and.USA2�1'M ar a hog
truck washing facility. I'm not going to be able to make up there this week because of other commitments, so f thought
if you had someone in the area that is familiar with this operation, that they might want to take a look. This is all of the

pdf shows the location of the report. '

Hey Mason,
Ken reported to me yesterday and I left you a voicemail yesterday 4/16/15 in this regard about apparent discharge of
pollutants to a drainage system at the R-2303D project.
Ken said that the folks were working on/around drainage structure 3321 and it had a terrible stench. Please see the
attached marked up plan sheet with some pertinent information marked up in red. This is just off of the service road to
the southwest of Hwy 701/421 in the area where the large grain bins and agricultural businesses are located; this is
Sheet 33 from the plans. This is just south of NC 24. Ken said that the easement to the south/southwest of the
referenced drainage structure 3321 goes back to a hog truck washing facility. Ken said that some of the workers (on our
DOT job) said that the pipe/box had been flowing about half full earlier in the day (4/16/15) with the apparent hog wash
water. This drainage network leads to a JS to the South/SE of the area; see the attached screen shot clip taken from
Sheet 34 which matches to sheet 33; the JS can be seen at the outfall of the pipe drain system.
