On August 12, 2015, Melissa Joyner from the Fayetteville Regional Office of the Division of Energy, Mineral
and Land Resources conducted a site inspection for the Unilever — Raeford Plant facility located on 100
Faberge Blvd., Raeford, Hoke County, North Carolina. A copy of the Compliance Inspection Report is
enclosed for your review, Amy Dorney, Health, Safety & Environmental Manager, Madison Mercer,
Environmental Engineer and Donna Locklear, Document Control were also present during the inspection and
their time and assistance is greatly appreciated. The site visit and file review revealed that the subject facility
is covered by NPDES Stormwater General Permit-NCG060000, Certificate of Coverage-NCG060114 Permit
coverage authorizes the discharge of stormwater from the facility to receiving waters designated as Peddlers
Branch, a class C stream, in the Cape Fear River Basin.
As a result of the inspection, the facility was found to be fully in compliance with the conditions of the
NCG060000 permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.

Inspection Summary;
The renewed General Permit for the facility became effective on December 1, 2012. The inspection was conducted with the
assistance of Amy Dorney, Safety, Health and Environmental Manager, Madison Mercer, Environmental Engineer and
Donna Locklear, Document Control. The facility had applied for and obtained representative outfall status in June 2006,
Outfall #2 is the representative outfall where monitoring/sampling is occurring. The Stormwater Pollution Prevention Plan
(SPPP) was reviewed and found to contain all of the necessary components. Analytical and Qualitative monitoring forms
were also reviewed. Monitoring frequency is being done in accordance with the General Permit. The facility grounds were
observed, including Outfall #2 and all ASTs to ensure that there was proper secondary containment around the ASTs.
Everything appeared in order on the grounds of the facility.

10/31/2012. The Division staff is currently in the process of renewing this permit. When a draft is available for
notice and public comment, it will be posted on our website at http://portal.ncdenr.org/web/wq/ws/su/public-notices.
Please continue to check this website to review and comment on proposed changes to the permit. Once the permit
is reissued, your facility would be eligible for continued coverage under the reissued permit.
To assure consideration for continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit. Enclosed you will find a Permit Coverage Renewal Application Form. The
application must be completed and returned by May 4,-2012. Letters confirming our receipt of the completed
application will not be sent.
Failure to request renewal within the time period specified may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $25,000 per day.
"No exposure exclusion" is now available to industrial facilities in any of the categories of "storm water discharges
associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of
"no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for
the no exposure exclusion. For additional information contact the Central Office Stormwater Staff member listed
below or check the Stormwater Permitting Unit Web Site at http://portal.ncdenr.org/web/wci/ws/su/npdessw#tab-5.
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed

In response to your renewal application for continued coverage under stormwater General Permit NCG060000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG060000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://Portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments /Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

Some of the changes include:
• Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections 8, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C: The monitoring parameter Oil & Grease (0&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections 8, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections 8, C: The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.org/web/wg/­wslsu/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

Some of the changes include:
Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C: The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

The Fayetteville Regional Office of the Division of Water Quality has reviewed your request
dated June 12, 2008 and received on June 16, 2008 for a determination that Stormwater discharge outfall
(SDO) #2 he granted representative outfall status for stormwater outfalls #1 and #2. Based on the site
visit conducted on May 7, 2008 and the information provided in your letter, we are approving this
request. In accordance with 40 CFR § 122.21(g)(7), you are authorized to sample outfall #2 as a
representative outfall. This approval is effective with the next sampling event. Please be reminded that
the permit requires Qualitative Monitoring to be performed at all stormwater discharge outfalls,
regardless of representative outfall status.
Please remember that any actions you initiate in response to benchmark exceedances as directed
in the tiered response provisions of your permit must address all drainage areas represented by SDO #2,
where appropriate.
Please append this letter to your Stormwater Pollution Prevention Plan (SPPP) to document that
representative outfall status has been approved. If changes in drainage areas, structures, processes,
storage practices, or other activities occur that significantly alter the basis of this approval,
representative outfall status is no longer valid. In that case, you should resume sampling at all SDOs, or
reapply to this office for representative outfall status based on updated information. If you have any

ADDENDUM
The National Pollutant Discharge Elimination System (NPDES) program was established under the
federal Clean Water Act and then delegated to the Division of Water Quality for implementation in
North Carolina. The NPDES permitting program for stormwater discharges was established in 1990.
Phase I focuses on site and operations planning to reduce pollutant sources.
Through the NPDES permitting program industrial facilities that fall into one of the subject ten
categories are required to obtain permit coverage under a general permit or an individual permit,
depending upon the facilities SIC code and the industrial activity occurring at the facility. There are
currently 19 active general stormwater permits in North Carolina. One condition that is applicable to
both the general permits and individual stOnriwater permits is the requirement to develop and implement
site -specific comprehensive stormwater pollution prevention plans (SPPP). These plans are required to
include a comprehensive evaluation of the site and operations to reduce pollutant sources and prevent
pollutant discharge.

General Stormwater Permit] can be found at this web site.
The following items are a subset of permit requirements and are listed to assist you and help bring your
attention to certain permitting issues, process and condition requirements of your permit. That said;
please see the above -mentioned web site in order to obtain a copy of the permit such that you may fully
comply with all the conditions therein. Failure to properly comply with conditions of the permit, like
those bulleted below, constitutes violations and is subject civil penalty assessment of up to
$25,000.00 per day.
• Industrial facilities that fall into one of the subject ten categories are required to obtain permit
coverage under a general NPDES stormwater permit or an individual NPDES stormwater permit
• All persons desiring to have facilities covered by a General Permit must register with the
Division of Water Quality (DWQ) by the filing of a Notice of Intent (NOI) and Applicable Fees.
• This Notice of Intent (NO]) must be submitted and a Certificate of Coverage issued prior to any
discharge of stornwater associated with the industrial activity, process wastewater, and/or mine
dewatering. .
• Once the Notice of Intent is received, DWQ will review and determine whether a Certificate of
Coverage (COC) may be issued. If this Certificate of Coverage is issued the facility is covered
by the General Permit. That is, the permittee is authorized to discharge stormwater, process
wastewater, and/or mine dewatering pursuant to the type of General Permit until the permit
expires or is modified or revoked.
• Any other point source discharge to surface water of the state is prohibited unless it is an
allowable non-storni'water discharge or is covered by another permit, authorization or approval.
• There are currently 19 active general permits. Permit conditions vary accordingly; however, most
of the permits have following condition requirements (you must read your entire permit to ensure
full compliance):
o Development and compliance with the Storm,,vater Pollution Prevention Plan
o Collection, reporting, and recording of Analytical Monitoring

o Collection and recording of Qualitative Monitoring
o Collection, reporting and recording of Vehicle Maintenance Monitoring
o Records — Visual monitoring shall be documented and records maintained at the facilitv
along with the Stormwater Pollution Prevention Plan. Copies of the analytical monitoring
results shall also be maintained on -site. The Permittee shall retain records of all
monitoring information, including all calibration and maintenance records and ail original
strip chart recordings for continuous monitoring instrumentation, and copies of all reports
required by this general permit for a period of least 5 years for the date of the sample
measurement, report or application.
o Expiration— The permittee is not authorized to discharge after the expiration date of the
general permit. In order to receive automatic authorization to discharge beyond the
expiration date, the permittee shall submit forms and fees as are required by the agency
authorized to issue permits no later than 180-days prior to expiration date. Any permittee
that has not requested renewal at least 180 days prior to expiration, or any permittee that
does not have a permit after the expiration and has not requested renewal at least 180
days prior to expiration, will be subject to enforcement. Failure to renew your permit is
a violation.
o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is
not transferable to any person expect after notice to and approval by the Director. The
Director may require modification or revocation and reissuance of the Certificate of
Coverage to change the name and incorporate such other requirements as may be
necessary under the Clean Water Act. The Permit is NOT transferable.
o Name Change, Closure or Facility Ownership change — Permittee is required to notify the
Division in writing in the event the permitted facility is sold or closed. The Division of
Water Quality views changes of name or ownership as a modification that requires the
Director's approval and reissuance of the permit. Name and ownership changes required
you to complete a Name/ownership Change .Form from the Division and mail it back to
the Division of Water Quality along with all appropriate information.
o Proper Operation and Maintenance is a requirement of the permit.

Inspection Summary:
Inspection conducted with the assistance of Mr. William Saylor, Senior Environmental Engineer. A review of the facility's
Stormwater Pollution Prevention Plan (SPPP) and records of Qualitative and Analytical Monitoring was conducted. The
SPPP was found to contain all necessary components with a few minor revisions to be made concerning the detailed site
map and changes made to the general permit pertaining to.monitoring frequency. It was noted in the facility's SPPP that
Representative Outfall Status had been granted back in 1991 for the outfall at the stormwater pond, however proper
documentation from DWQ showing approval could not be located at the time of inspection. If found at a later time, a copy
of an approval letter from DWQ should be submitted to the Fayetteville Regional Office. If not, a letter requesting
Representative Outfall Status needs to be submitted as soon as possible for review and approval. After review of the
SPPP, a tour of the grounds of the facility was conducted to ensure appropriate secondary containment around all ASTs
and observations were made pertaining to the outfalls.

Comment: Facility personnel had identified three outfails, however one of those is
simply a storm drain that leads into the facility's stormwater pond. Other than the outlet
of the pond, there is one other outfall located on the property giving the facility two
outfalls to be monitored per the requirements of the stormwater general permit. Facility's
SPPP contains language concerning Representative Outfall Status and Analytical
Monitoring has only been conducted at the outlet of the pond, however proper
documentation could not be located at the time of inspection.

We have received your letter of October 30, 1995 in the Fayetteville
Regional Office. You have indicated that this project involves clearing an
800'by 30' corridor outside the fence at the Raeford Chesebrough-Pond's
facility. of this area, an estimated 75, span includes wetlands through which
storm water drains from this facility. According to Mr. Michael Hosey of the
Regulatory Branch, U.S. Army Corps of Engineers, this site is above headwaters
and therefore subject to Nationwide Permit 26. Since the wetland impacts of
this project are less than one-third acre (approx. .05 acre), the Division of
Environmental Management does not require written notification nor approval of
this project. Also, since the entire project as described does not exceed one
acre of land disturbance, no sedimentation and erosion control plan is
required. This has been confirmed with the Division of Land Resources, Land
Quality Section.

This letter is submitted as a follow-up notification to the phone
call you received on June 6th,' 1995 reporting a spill of *18 rap
*shampoo" (off specification shampoo/conditioner) from the Raeford
facility of Chesebrough-Pond's USA (CPUSA) into a storm water
conveyance and the Fire Protection Lagoon at the facility A
brief description of the spill follows:
On June 5th, at. approximately 6:30 pm, some foam was noted at the
south end of the Fire Protection Lagoon. The foam was coming
from a storm drainage pipe. To prevent the foam from spreading
out into the lagoon and possibly reaching Outfall #002, oil. booms
were tied together and placed around the foam at the point where
the piped enters the lagoon. No foam reached outfall #002, and
the foam was contained at approximately 9:00 pm. At
approximately 9:30 pm, samples were caught from the lagoon and
the outfall. An analysis of the water in the lagoon, at the
point of entry of the storm pipe, showed 200 ppm COD. An
analysis of the water flowing through outfall #002.showed 10 ppm
COD. By 7:30 cam on June 6th, all of the. Loam i;i the storm
drainage pipe and the lagoon had dissipated.
Upon investigation, the origin of the -foam was found to be a Non -
Hazardous storage area used to store scrap shampoo. A number of
55 gallon drums of scrap shampoo were stored in an outside diked
area. Due to heavy rains, shampoo residue was washed off the
drums onto the floor of the diked area. i�lthough the drain valve
on the diked area was closed, the heavy r�iin.s caused the diked
area to overflow into an adjace.it storm water -Ir_ain, causing
foaming in the drain pipe. The residue flowed down the storm
drain to t-he Fire Protection Lagoon. As m.:ntioned above, . sol7Le
foaming was also noted where= the drain pipe empties into the
lagoon. It. is CPUSA's belief that only a small amount of scrap
shampoo entered the storm drainage pipe and lagoon.
Unfortunately, it only takes a small amount of shampoo to create

a large amount of foam.
To prevent re -occurrence of this incident, the Non -Hazardous
storage area mentioned above will be eliminated. Scrap shampoo
will be stored inside the Main Plant building.

Your request of October 5, 1994, to sample the stormwater discharge outfall(s) for purposes of
NPDES stormwater general permit monitoring requirements has been reviewed by members of the Water
Quality Section staff. Your request described three (3) outfalls at the plant S#l, S#2 and S#3. According
to the site drainage map, S#2 receives the majority of the storm drainage with S#1 discharging stormwater
mainly from a gravel and grass area. Analytical monitoring at S#3 is unnecessary due to stormwater
discharges from areas in and around S#3 are represented in S#2.
Your letter mentions a non-stormwater discharge at S#3. Our staff is currently working to process
your application (Form 2E) that was submitted for permitting of this non-stormwater discharge.
Based upon staff review of your request, and attached site map, we have concluded that the
requested determination is warranted and in accordance with with Part I, section B4 of the general permit..
We authorize you to sample at the outfall S#2 at the facility as the stormwater discharge representative of
the runoff from the Chesebrough-Pond's USA facility for your NPDES stormwater general permit
monitoring requirements.

On January 23, 1992, the Division of Environmental Management received your individual
application form and application fee for, an NPDES Permit for the continued discharge of stormwater
runoff from the subject industrial activity. We have reviewed your individual application and find that
your industrial activity stormwater discharge(s) qualifies for coverage under General NPDES
NCG060000. A copy of the General Permit is attached for your information. This Division developed a
number of General NPDES Permits for discharges of stormwater runoff from specific industrial activities.
Each of these General Permits are tailored to the specific types of industrial activities for which the permit
provides coverage. All of these General Permits follow the same basic requirements: 1. development of a
Stormwater Pollution Prevention Plan by the permitted facility; and 2. conduct routine self -monitoring of
the stormwater discharge. All of the permits require visual monitoring, some also require periodic
sampling and analyses.
Since your discharge(s) qualifies for General Permit coverage and individual stormwater permits
will essentially have the same conditions and requirements as the General Permits for the same type of
industrial activity, we propose to grant you General Permit coverage under NCG060000 in response to
your individual permit application. The application processing fees are the same for individual stormwater
permits and General Permits ($400.00); however, the individual permits have an additional annual
administering and compliance monitoring fee of $600.00 (reduced to $450.00 if compliance is
maintained), the General Permits do not have any annual fee. Your application processing fee which you
have already paid will be transferred to the General Permit and no additional fee will be required if you go
into coverage under the General Permit. One other difference in the individual permit and General Permit
coverage is that when we process an individual NPDES permit we are required to give public notice of the
draft permit and solicit public comments. If the public comments are considered to be significant, the
Director may order that a public meeting be held concerning the draft permit prior to any final action. With
the General Permit the public comments were entertained during the issuance of the General Permit, there
is no public notice required to provide coverage under the General Permits for individual facilities.
