The TSS level of 104 mg/L exceeds the benchmark value by 4 mg/L, triggering a Tier I response by GSK Zebulon. 24Apr2017 — Site Stormwater management inspection performed by David Schaefer, EHS Engineer, as required by Tier I response. Following trouble areas identified for action: • Two storm drains within pavement east of main site energy center show that one leading to other has high liquid level, seeming to indicate blockage. It was found later that this drain has a sump that is lower than the connecting pipe (probably to easily capture dry weather spills ). • Hill directly west of dust collector building shows large areas of exposed soil. Stormwater from hill eventually leads to storm drain system within gravel area near production building. ZrI • Storm water outfalls west of MDPI building (series of rip -rap depressions with steel pipes) are mostly filled with sediment. **Corrective actions identified and approved —see below.** • Representative Outfall was very clear (unlike date of sample 31Mar17). It was identified that the date of the sample, a nearby project to install a new manhole had caused trackout of soil into the roadways, very close to the representative outfall. While the roads had been cleaned since that date and may have allowed future samples to be within benchmark limits, it was decided to still move forward with other stormwater management correction projects. 01May17 — Meeting held to share results of stormwater management inspection with contractors to develop project plan. 07May17 — First item completed -- Edwards Inc. repaired erosion control fencing near South Admin emergency generator project and added additional straw bales. (Complete landscaping solution identified for hillside to be completed before end of year, after completion of project.) ***Delays for remaining projects.*** This site has complex vendor management and quote approval processes. Multiple quotes were requested, the approval process is slow, and then the vendors would schedule far out in advance or had to wait for other vendors (i.e. the storm drain cleaning service). However, we were always in talks regarding projects and drove to have them all completed. y 1� I V Week of 31Ju117 — Landcare adds riprap to storm drain entrance to system near gravel yard. Also, gravel bag filters are added to storm drains within gravel yard area and contractor gravel area to prevent sediment from entering stormwater system. 11 21Aug17 — Updated training in powerpoint format has been distributed to engineering staff, including references to erosion control. Site is also currently arranging a site visit by Tetratech for an erosion evaluation of the entire site. The sum of these actions should greatly reduce the total suspended solids in storm water outfalls. In addition, the site has much better awareness and is working harder to identify and control potential erosion issues, particularly those related to construction projects. FQrany q�es ons related to this Tier I response, please contact me at (919) 269-1966 0: David Schaefer, CHMM EHS Engineer GSK 1011 N Arendell Ave Zebulon, NC 27697 Email david.x.schaefer@gsk.com Non significant dye spill — Report dated 04May2017 On 01May17, it was discovered that a pink liquid was inside of the storm drain east of the main site energy center, just to the north of the rollup door entrance to the Chiller room. This was documented with a photograph. As EHS was aware of a pink dye being used on site to search for water leaks in the chill water closed loop, contact was made with CBRE to determine how the dye made its way into the storm drain. That evening, CBRE utility technicians used a pump to remove 55 gallons of the dyed water from the drain, finding it to not change the level much. The CBRE maintenance manager discovered the next morning that a chill water leak was occurring in Air Handler 90, which is positioned on the roof of the building. The air handler allows ambient condensate to enter the storm drains on the roof, which is acceptable. However, this chill water leak from the cooling coils was entering this very storm drain, and sending dyed water into the storm drain on a constant basis. CBRE contacted site based contractors for an expedited repair, and lined up Edwards Inc. to repair the leak the next morning, 03May2017. The leak in AH-90 was repaired at approximately 12:30 pm on 03May17, which halted the leak of pink liquid into the storm drain. The stormwater drain in question, and the stormwater drain just north of the drain outside of the rollup door (located in the alleyway between energy center and SD Mfg) both have deep pits, possibly to collect material like this before sending forward towards the outfall. The drain outside of the EC rollup door has a horizontal pipe near the top of the pit that will drain south towards the series of pipes and drains that enter site outfall #5, which is GSK's representative outfall, discharging to wetlands on GSK property and flowing towards Worth Hinton Road. As the pits must be full to a high level before sending water south toward the outfall, it is likely water from these pits only overfills the pits and flows toward the outfall during rain events or massive spills / leaks. From 01May17 to 04May17, no water flowed towards the outfall from this pits. The leak was collected into drums by CBRE using a sump pump 02May-04May17. The afternoon of 02May2017, Dave Schaefer EHS investigated the environmental impact of the leak. CBRE provided an SDS of the dye used (Kingscote Chemicals — Bright Dyes Industrial Red Liquid). This SDS showed that the dye contain 5-10% Basic Violet 10. This dye is non-biodegradeable, per an SDS of the pure substance sourced from Sigma Chemical. However, it is also not a marine pollutant. It was found that over the course of a year, approximately 1 gallon of this dye had been poured into the chill water closed loop system at various times — and much of it may have entered the storm water system from the time the leak from AH-90 had begun. After the discovery of the pink water in the storm water drain outside of the energy center 01May17, all efforts had been made to collect the dyed water present without discharging any more to the environment. However, prior to 01May17, the leak was not known, and dyed water is known to have entered the stormwater drains and pipes leading to outfall #5, and passing through the outfall into the wetlands. A gemba of the wetlands was conducted on 02May17 by David Schaefer EHS. He viewed the water in the shallow stream leading from the outfall, and saw a faint tinge of pink to the water. Three photos are attached in this file, and the pink tinge is not easily noticeable without historical reference. The surrounding soil and plant life did not show signs of staining. The level of the dye at this location is estimated to be 5 ppm or less, conservatively. The dyed water in the storm drain was determined to be - 50 ppm by CBRE utility technicians. The dilution of this water prior to entering outfall #5 is estimated to be at least 10x during a measureable storm event, which is likely conservative as 3 major branches of storm drain feed this outfall. This stream from outfall #5 combines with several other stormwater streams before finally leaving GSK property near Worth Hinton Road. This stormwater system will eventually join the Little River. As there are several dilutions prior to leaving GSK property, it was also estimated that the level of dye in water leaving the GSK property is much less than 1 ppm, and is likely in a lower concentration in ppb. This dye is shown to not be a marine pollutant. While it is not biodegradeable, it is not especially toxic in the environment in low concentration per SDS. It is also not a hazardous substance, not a reportable quantity under section 311 of the Clean Water Act, nor is it in section 102 of CERCLA. While the dye also contains a small concentration of acetic acid, the acetic acid found in this spill will be on the same order as the dye concentration. Even if all of the dye poured exited the site at once, it would total less than 1 lb of acetic acid, much less than the reportable quantity of 5000 lb. For these reasons, GSK Zebulon EHS determined this spill to not meet the definition of a "Significant Spill" or "24 hour reporting" required notification to the NC Dept of Water Quality as described in section E of the NCG060000 general wastewater permit. This event does meet the definition of "other non-compliance" as defined in section E of the wastewater permit, and GSK Zebulon will report this instance during the next monitoring period of July - December 2017.
