SUPPLEMENTAL INFORMATION REQUIRED FOR RENEWAL OF INDIVIDUAL
NPDES STORMWATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application
to be considered complete:

(Do not submit the site Stormwater Pollution Prevention Plan)
I. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall,
building locations and impervious surfaces should be clearly noted.
2. A summary of Analytical Monitoring results during the term of the existing permit
(if your permit required analytical sampling). Do not submit individual lab reports.
The summary can consist of table including such items as outfall number,
parameters sampled, lab results, date sampled, and storm event data.
3. A summary of the Visual Monitoring results. Do not submit individual monitoring
reports. The summary can consist of a table including such items as outfall number,
parameters surveyed, observations, and date monitoring conducted.
4. A summary of the Best Management Practices utilized at the permitted facility.
Summary should consist of a short narrative description of each BMP's in place at
the facility. If the implementation of any BMP's is planned, please include
information on these BMP's.
5. A short narrative describing any significant changes in industrial activities at the
permitted facility. Significant changes could include the addition or deletion of
work processes, changes in material handling practices, changes in material storage
practices, and/or changes in the raw materials used by the facility.

If the final year analytical monitoring of the existing permit term has not been completed

Title: Storm Water Management
1.0 Purpose
The purpose of this Best Management Practice (BMP) is to provide basic techniques and
environmental guidelines for the protection and management of storm water to prevent
contamination of storm water from occurring at this industrial site.
2.0 Application
This BMP applies to those individuals who have the potential to contribute to or prevent storm
water contamination from occurring at the industrial site.

4.0 Procedure
4.1 Discussion
If not properly managed, storm water can be a vehicle for pollutants to be transported from
industrial sites to sensitive environmental receptors down gradient. It is important that all
associates be aware of the potential impacts that an industrial site can have on storm water and
that they operate in a fashion to minimize the site's potential impact to storm water.
This BMP establishes minimal Best Management Practices that will help protect the
environment from storm water pollution and the negative environmental impacts thereof.
It should be noted that some facilities are required to attain some type of Industrial Storm
Water Permit. In these cases, this BMP is only designed to supplement the requirements of that
permit and does not replace any requirement set forth in the permit.
4.2 Operational Controls
The following practices will be utilized at the site to prevent or minimize storm water pollution:
a. No grading, soil disturbance or construction activity can take place prior to
notification and approval by the Environmental Manager (EM) responsible for
oversight at the facility. This is to ensure that the proper permits (if required) are
attained and that plans are in place to protect storm water from becoming
contaminated as a result of the proposed activity.
b. No industrial materials, including; ingredients, raw materials, waste products and
loose packaging will be located in such a way as to allow it to come in contact with
rainwater or storm water runoff. If such materials must be temporally exposed, all
efforts must be made to protect it from storm water as soon as practical.
c. Decommissioned equipment stored outside will be cleaned and drained of any
lubricants and fluids prior to placing outside.
d. Equipment which is permanently installed outside will be inspected routinely to
ensure that they are properly operated and maintained and no leaks have
developed which could result in storm water contamination.
e. Exposed materials that could come in contact with storm water due to spills and
leaks must be cleaned up immediately...
f. Solid waste receptacles will be managed in a way as to prevent storm water
contamination from occurring. This might include; covering dumpsters, closing
dumpster lids, routine cleanup around dumpsters and routine haul -off of solid
waste.
g. Routine sweeping of outside production areas should be conducted to prevent
accumulation of potential storm water contaminants.
h. All outside washing activities will be contained as to prevent dry weather flows,
except as allowed by the sites storm water permit (if applicable).
i. No dry weather flows from the process or any equipment will be allowed at the site,
except as allowed by the site's storm water permit (if applicable).
j. Good housekeeping practices will be employed at all times.
4.3 Verifications and Corrective Actions
Periodic verifications will occur which provide assurance that the above mentioned practices are
being adhered to and that the practices are adequate to protect against storm water pollution.
These verifications will occur at a frequency deemed adequate to assure compliance with this BMP;
however, the frequency will never be less than quarterly.
When items are identified as deficient or found to be inadequate to protect against storm water
pollution, follow-up actions must be identified and completed on an established schedule and the
corrective actions documented in the site's EMS program files.
4.4 Documentation

5. Others as required by the site's Storm Water Permit (if applicable)
4.5 Training
All associates who have the potential to contribute to or prevent storm water pollution will be
trained in the following:
1. This BMP
2. General Knowledge of Storm Water Pollution Prevention
3. Storm Water Permit Requirements (if applicable)
4.6 Emergency Response
If a spill were to occur which has the potential to impact storm water runoff or receiving waters,
quick action must be taken to protect people, wildlife and the environment. Immediate
notification to the Site Manager and also to the EM responsible for the site must be made. The
site's EM will assist site management on the proper notification and cleanup activities which
should occur. The site's EM will also be responsible for ensuring that the proper follow-up
activities, such as documentation and reporting occur as required by governing regulations.

Short Narrative Describing Any Significant Changes
The Candor Feed Mill has completed a few upgrades at the facility during the past five (5)
year permit cycle. A 500,000 bushel galvanized corn storage bin was constructed in 2012. In
addition, a rail pit and corn receiving area was added. In 2014, number 6 fuel oil was removed
from the site. Compressed natural gas was installed on site and is used as the primary fuel
source. Number 2 diesel fuel is the backup fuel source.

In response to your renewal application for continued coverage under stormwater General Permit NCG060000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG060000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http:/Iportal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

for your specific General Permit carefully.

Some of the changes include:
Part 11:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections 8, C: A lower TSS benchmark of 50 mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C. The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
• Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections 8, C: The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative

The following information is submitted in response to your letter dated July 27, 2012, requesting additional
information regarding the July 24, 2012 site visit by Mr. Hughie White. Outfall 002 is located on the North side
of the Perdue property next to the railroad tracks (please see attached map). According to Mr. White, 002 is a
direct point discharge which requires qualitative and analytical monitoring. It should be noted that due to the
small collection area that contributes to this outfall and the sandy soil in the ditch, history has shown that runoff
from this area does'not reach waters of the state but simply percolates into the ground. However, to avoid any
confusion in the future, we have decided to seal 002 outfall to eliminate any potential point source discharge.
Perdue is committed' to be good stewards of the environment and this misinterpretation did not have any
discernable impact to the area.

Quality, conducted a site inspection for the Candor Feedmill facility located at 351 Perdue Way, in
Candor, Moore County, North Carolina. A copy of the inspection report is attached for your review.
Your assistance, as well as the assistance of Mr. Tom Brinson and Mr. Aaron McCaskill was greatly
appreciated during this inspection. Stormwater from the facility drains to Drowning Creek, a Class WS-
I1; Sw, HQW water located in the Lumber River Basin. The site visit and file review revealed that the
subject facility is covered by NPDES Stormwater General Permit-NCG0601.02.

Inspection Summary:
The stormwater pollution prevention plan has been developed and implemented. This facility has two designated outfalls;
however, qualitative monitoring and analytical monitoring is only being performed at outfall 001 and not at outfall 002. As
discussed during this inspection, even though outfall 002 only collects stormwater from a very minute footprint of the
facility, it still has a direct point discharge that needs to be monitored. Also, currently this facility is in the Tier Two
monitoring requirement outlined in the stormwater permit. This facility exceeded the benchmark for Total Suspended
Solids (TSS)for the last two sampling events in December 2011 and May 2012 at outfall 001, so currently the facility is
monitoring the outfall monthly.' It was also suggested during this inspection that oufall 001 could be moved further
downstream from the current location. The stormwater where outfall 001 is located flows further downstream into a
sedimentation basin, then stormwater overflows the basin and on to the receiving stream. Since the sedimentation basin
is still on Perdue property and the property line is actually closer to the receiving stream; it was recommended to move
outfall 001 downstream of the basin closer to the property line. That way the basin will help with stormwater treatment
and should solve the problem -for the TSS exceedances and also stormwater samples should be taken as close as
possible to where the water leaves the property. This facility does not meet the requirement to perform monitoring from
vehicle maintenance areas. Also, both outfalls were observed during this inspection.

■ A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit. This form must be completed and returned to the Division within 30 days of receipt of
this letter. DO NOT send the SPPP with the signed form.
■ Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program with outlines program components
and addresses frequently asked questions.
■ A corrected Certificate of Coverage if you indicated a name or address change on the Renewal
Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
