As you requested during our conversation this week, please accept this email as a 5 day report in connection with an approximately 5 gallon diesel fuel spill at
our Pfizer Rocky Mount facility in Rocky Mount, NC.
Event Summary:
A fuel oil spill was discovered Monday, 9/11/17, on the east section of our property, in a gravel drive space, adjacent to our shipping warehouse. The
source of the spill is a 5 gallon fuel tank, used to refuel a rough terrain fork truck used by our roofing contractor. The fuel tank fell from the contractor's
pickup truck, spilling fuel in the process. There are no waterways or drains located near the spill location.
Event Response:
Upon discovery, site personnel requested service from our environmental cleanup contractor. The contractor arrived onsite, excavated the top 6 inches
(approximate) of gravel and soil, containerizing in drums for disposal. Due to the heavy compaction of gravel and soil, additional excavating equipment
is required to complete the work. Due to the need to locate utilities (the impacted area is over a fire main) and periodic rain, the completion of the work
has had to be extended to early next week. During all rain, the area has been covered with plastic sheeting, and access to the area restricted until work
is completed.
Actions Remaining:
We anticipate the work being completed early next week, including backfilling the area with new soil and gravel.

Comments: The sample was originally analyzed on 6-15-09 at 200m1. The found mg/L exceeded the acceptable limits. The result
was 2812mg/L, The sample needed to be rerun at a lower dilution. The sample was rerun at 50ml on 6-17-09 which was I day past
the holding time. The result on the second run was 492mg/L. Due to the large difference in results there was a possible contamination
during the initial run. The result found on 6-17-09 is the one being reported.
Corrective Action: Precautions will be taken to ensure there are no issues with contamination.

On 2/8/2013, the Division of Water Quality received your request to rescind your organization's
stormwater permit coverage under Certificate of Coverage Number NCG020774. In accordance with your
request, Certificate of Coverage Number NCG060035 is rescinded effective immediately.
In your letter of April 20, you also requested that your certificate of coverage for NCG060034 be
renewed. NCG060034 was renewed as noted in our letter to Marty Nealey on December 4, 2012.
Unfortunately, the renewal was in a routinely -scheduled renewal process and was not coordinated with
your request to cover plants R I and R2 under the same permit. Therefore, I have attached a revised
"Certificate of Coverage" for your facility that makes it clear that plants R I and R2 are both covered
under the same permit. We appreciate the efforts you have made to cover both facilities under one
Stormwater Pollution Prevention Plan.

plants in NCG060034 on March 8, 2013.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
