In response to your renewal application for continued coverage under stormwater General Permit NCG060000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage (COC)
• A copy of General Permit NCG060000
• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://Portal.ncdenr.org/web/wq/ws/su/current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

for your specific General Permit carefully.

Some of the changes include:
• Part II:
• Section A: The Stormwater Pollution Prevention Plan (SPPP) section, if applicable, has been updated
to the most current language of our permits. Additional conditions for specific industry sectors have
been added to the SPPP requirements in some cases.
• Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring
that the permittee begin a monthly sampling scheme.
• Sections B, C: A lower TSS benchmark of SO mg/I for HQW, ORW, PNA and Tr Waters applies to these
more sensitive waters.
• Sections B, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter
Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other
analytical monitoring requirements.
Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in
separate on -site records if your General Permit does not require an SPPP. Adverse weather is defined
in the "Definitions" section of the permit.
• Sections B, C: The term "Representative Storm Event" has been replaced by "Measurable Storm
Event." A measurable storm event is defined in the permit.
• Section D: If the permittee fails to respond effectively to problems identified by qualitative
monitoring, DWQ may require the permittee to perform corrective action.
Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit,
including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and
Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms
of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on
the Stormwater Permitting Unit's website above. DMR forms must be delivered to the Division no later than
30 days from the date the facility receives the sampling results from the laboratory. Also note that existing
permittees do not need to submit a renewal request prior to expiration unless directed by the Division.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree. If you have any questions regarding this permit

10/31/2012. The Division staff is currently in the process of renewing this permit. When a draft is available for
notice and public comment, it will be posted on our website at http://portal.ncdenr.org/web/wq/ws/su/public-notices.
Please continue to check this website to review and comment on proposed changes to the permit. Once the permit
is reissued, your facility would be eligible for continued coverage under the reissued permit.
To assure consideration for continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit. Enclosed you will find a Permit Coverage Renewal Application Form. The
application must be completed and returned by May 4, 2012, Letters confirming our receipt of the completed
6pplication will not be sent.
Failure to request renewal within the time period specified may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $25,000 per day.
"No exposure exclusion" is now available to industrial facilities in any of the categories of "storm water discharges
associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of
"no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for
the no exposure exclusion. For additional information contact the Central Office stormwater Staff member listed
below or check the Stormwater Permitting Unit Web Site at http://portal.ncdenr.org/web/wq/ws/su/npdessw#tab-5.
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions can be found on the bottom of the form. You will be notified when
the rescission process has been completed.

Dear Mr. Cacciatore:
On June 14, 2011, 1, Hughie White, from the Fayetteville Regional Office of the Division of Water
Quality, conducted a site inspection at the Campbell Soup Supply Company facility located at 2120 NC 71 Hwy
North, Maxton, Robeson County, North Carolina. A copy of the Compliance Inspection Report is attached for
your review. Mrs. Hope Walters was also present during the inspection and her time and assistance is greatly
appreciated. Stormwater from this facility drains to the Lumber River, a Class B; Sw; HQW water located in the
Lumber River Basin. The site visit and file review revealed that the subject facility is covered by NPDES
Stormwater Permit-NCG060029.
Accordingly, the following observations were noted during the Division of Water Quality inspection:

Inspection Summary:
A stormwater pollution prevention plan has been developed and implemented. As noted in this inspection checklist, a
copy of a General Location (USGS) map needs to be added to the plan. The plan is being updated annually with the last
update being performed in November 2010.
This facility has been granted a variance on pH monitoring due to the naturally occurring acidic groundwater and surface
water that occurs in this region. The benchmark values in the permit for pH are within 6.0-9.0 standard units but samples
of area surface waters and the stormwater outfalls are consistenetly below the minimum 6.0 su. So for the pH parameter,
this facility currently does not have to enter into the Tier Two provisions. Also, there was one benchmark exceedance
during the last sampling period for total suspended solids at one outfall that receives flow from a dirt and gravel parking
lot. Options need to be looked at and action needs to be taken to insure the benchmark value is not exceeded two times
in a row. If that happens then analytical monitoring will have be performed monthly.
Qualitative and analytical monitoring is being performed semi-annually, as required. This facility does not meet the criteria
to perform analytical monitoring for vehicle maintenance areas.

Dear Mr. Cacciatore:
On October 16, 2008, 1, Trent Allen from the Fayetteville Regional Office of the Division of
Water Quality, conducted a site inspection for the tract/project known as Campbell Soup Supply
Company — Plant Expansion 2008 located at 2120 NC Highway 71 North, Robeson County, North
Carolina. A copy of the Compliance Inspection Report is attached for your review. Bruce Grambois
was also present during the inspection and his time and assistance is greatly appreciated. The site visit
and file review revealed that the subject project has an approved Erosion and Sedimentation Control
Plan and is covered by a Construction Stormwater General Permit NCGO10000.
A Construction Stormwater General Permit (NCGO10000) is issued upon the approval of an
Erosion and Sedimentation Control Plan. This permit applies to projects that receive a Division of Land
Resources (DLR) (or local delegated program) Erosion and Sedimentation Control Plan for land
disturbance of l acre or greater. Specifically, the General Permit (NCGO10000) authorizes the discharge
of stop-nwater under the National Pollutant Discharge Elimination System (NPDES) in accordance with
Title 15A North Carolina Administrative Code 2H.0100.
Accordingly, the following observations were noted during the Division of Water Quality
inspection:

9. Buyer may reject either before or after delivery any materials which do not conform with all specifications and conditions of this Purchase On;ler or are
not delivered at the stated times, delivery times being of the essence. Delay in Seller's delivery and delay in Buyer's, acceptance and payment vhall be
excused to the extent Seller's production or shipment or Buyer's receipt or use of the materials Is prevented by strike or labor dispute, fire, or other
circumstance beyond control within Seller's establishment or within the location where the materials are Intended tc be received or used; except that
nothing herein shall be deemed to prevent Buyer from canceling undellvered materials which are so delayed. Buyer may cancel any portion of this
Purchase Order which remains unfilled after the beginning of any bankruptcy or insolvency proceeding by or against Seller or after the appointment of an
assignee For the benefit of Seller's creditors or of a receiver.

10. Seller shall maintain and observe the fallowing;

additional insured; 2) automobile liability; 3) workers' compensation (in the minimum amounts required by state law and employees liability at 31,000.000
per accident, per disease, and per employee); and 4) such other insurance policies as may reasonably be required by Buyer (including umbrella policies)
with minimum coverage as Buyer shall specify. Before work begins, Seller shall furnish Buyer with cartifrcate(s) of insurance evidencing such csverages.
All such policies shall be issued by insurer(s) with a Best's raring of at least A, Financial Glass Sae Vill, and in such amounts as are reasonably
acceptable to Buyer. Seiler shall provide Buyer with at least thirty (30) day's prior written notice of any material chenga In, or expiratlon or cancollation of
any Such policy. Without in any way limiting the remedies available to Buyer, Buyer shall have the right to immediFately terminate the Purchase Order upon
the failure of the Seller to comply with any of the provisions of this guarantee. Failure to deliver such certificates or -fa,lure of Buyer to demand evch
certificates shall not be considered a waiver nor excuse performance under this provision. Seller's liability as stated in the Purchase Order is not limited to
the scope or limits of any insurance policy or policies,
(h) Lions -For work performed on Buyer's premises, no lien shall be claimed or filed and complete releases of Ilan$ shall be furnished prior to final payment;

to fill this Purchase Order. Unless otherwise directed by Buyer, Seller agrees to return all such drawings, blueprints, samples and other Informeitlon to

13.5eller agrees to create accurate records, books and accounts of costs and charges to Buyer hereunder and to maintain them in accordance with
generally accepted accounting principles. Buyer shall have the right to review and inspect From time to time such records, books and accounts to confirm
that amounts paid or to be paid by it hereunder were correctly calculated in accordance with the terms of this Purct ase Order. Buyer shall have the right tc

This letter concerns the spill that occurred at Campbell Soup on December 3, 2000. The letter
also concerns the fourth and final visit that I made to your facility on August 2, 2001, in which 1 met
with you, Lorraine Sampson (Environmental Coordinator), John McBryde (Area Manager -Power
House), and Brock Gettinger (Project Engineer). In order to prevent the reoccurrence of a spill, a large
dam with fifty cubic yards of cement has been constructed as a barrier for floating oil. It had three
pipes that extended eight inches below the surface to prevent any surface oil from flowing to the
receiving stream. Granite rock was hauled to the area near the dam to prevent soil erosion. There were
three large gate valves on the oil -water separator dam. A new containment area was built for the
unloading of fuel oil from trucks. The containment area drain connected to an oil sump. The sump led
to the ditch water before the dam (at the point where the December 3 oil spill had entered the ditch). In
the boiler house, there was a new float mechanism that sets off a sound alarm (and a small blinking red
light) when the float level was disturbed by a liquid (such as oil). The drain that originally went to the
sump (which caused the December 3 spill) was completely filled with cement. Also in the boiler house,
there was a new cement bump, similar to a speed bump, that prevented an oil spill from going further
into the boiler house. I was informed that these items had been in service for approximately three
months. It has been stressed to the operators that, when doing maintenance, the valves should to be
closed to prevent future oil spills, and that the operators should be aware of the possibility of a spill.
There was a new containment area in the boiler house around the place that the spill had originally
occurred. It had a detention time of approximately ten minutes, which gave the operator enough time to
answer the alarm and perform the appropriate response to the spill. The valve was the same (lever type-
--not spring loaded). After a review of the piping system for this plant by the plant engineers, there was
not any other piping or drains that were found that could cause a similar spill to occur in the future. On
August 20, Lorraine Sampson informed me that the sensor in the boiler house containment area was
connected to the control panel in the control room.

On December 8, 2000, Terry Stillman of EPA in Atlanta visited the spill site and made
recommendations. It appears that the items in your January 26, 2001 'letter to George Starr, EPA, have
been addressed. I visited the site on December 5 and 6, 2000, immediately after receiving the spill
report, oil April 4, 2001 after the clean up was complete, and again oil August 2, 2001 to view the new
constructions and improvements. It appears that the oil spill affected areas have been restored,
preventive measures taken, and closure to tills incident should be made. You and your staff are
commended for your work during this spill.

The following is a description of the oil spill incident that recently occurred at our
Sunday, December 3, 2000:
+ 4:30 a.m. The plant boiler house personnel started a change over from
natural gas to a blend of #2 & #6 fuel oil as part of our gas curtailment
procedures. An employee went on the first floor to clean the associated fuel
oil strainer.
+ 5:00 a.m. A Campbell Soup Company boiler house employee went behind
#4 Boiler and found fuel oil on the first floor. This spip was caused bysan
opened valve on the fuel oil strainer. He immediately ran to close the fire!
oil strainer valve and notified another employee there had been an oil spill
within the boiler louse. They proceeded to put mats on the floor and began
clean up efforts. The spilled oil from the floor was placed into 55-gallon
drums. At this time, boiler house employees considered the spill properly
contained within the boiler house building.
Monday, December 4, 2000:
+ 7:00 a.m. The Boiler House Supervisor was notifiedthata fuel oil spill -had
occurred on the previous day. He immediately investtgaW and determined
that the spill had been contained and cleaned up. }
Tuesday, December 5, 2000:
• 9:30 a.m. The pump in the sump pit used to discharge boiler blowdown,
water lost prime. -Campbell Soup employees opened up the pit doors and
saw fuel oil inside. The Boiler House Supervisor used a dipstick to see how
much oil was in the pit. The stick read between 6 and 8 inches. He then
inspected the cooling water ditch and determined that no oil had reached the
ditch.

pump and began -pumping the material out of the pit. This process was
completed around 2:15 p.m. All material was placed ini6'55-gallon drums.
• 2:30 p.m. The Boiler House Supervisor noticed that there were visible signs
of oil in the drainage ditch. He then informed theiplant "Aw' ronmental
Coordinator of the situation. Immediately plant management wai infoined.
• A visual inspection of the Cooling Water Discharge ditcland the Lumber
River was performed. This visual inspection confirised'tiiat no material`
reached the Lumber River.
• Campbell Soup Company boiler house employees continued to contain the
spill by placing oil absorbent material in various locations in the drainage
ditch, including in front of and behind the Parshall flume used to monitor
flow in the ditch as part of our existing NPDES peinnit.'W-booth was pli3oed
on the backside of the flume.
• There was a total of 27, 55-gallon drums collected on site'SAll of this•'
material was either removed from the sump pit or from the floor in the moiler
house. At this time it was estimated 1400 gallons of fuel oil was spillled.and
contained on the property.
• 3:00 p.m. Plant Services Manager called Brian Septic Tank Service for
assistance with the clean up of the sump pit. They stated that a truck would
be sent out immediately.
• 3:30 p.m. Maintenance Manager called the Plant Services Manager
requesting a second truck from Brian Septic Tank Service. Mr. David
Wallwork (Brian)'informed Campbell Soup Company the truck would be at
our facility within' 15 minutes.
• At 4:00 m., per Campbell Environmental Policy, the oaf.. i-S O�

Company, World Headquarters Environmental Engineering Department;was
notified of the spill and given a description of the situation, as it was down
at that time.
• The National Response Center was notified at 4:35 p.m. of the spill. We
spoke with Ms. Johnson and were given Report #550007.
• The North Carolina Department Natural Resources (Water Quality Section)
was notified at 4:50 p.m. In talking with Belinda Hinson, she informed.us
she would like a follow up phone call the next day giving an update of t#c
situation.
• 5:10 p.m. Belinda Hinson called informing Camp liell•Soup'Con'p" any that<a
w.
representative from the NCDNR would be in that night to conduct an
inspection.
• 330 p.m. Thomas Ammons with Environmental'HI ydirb'ge'("
Consultants was,cOed requesting assistance with an ' emergency response
spill. At 5.45 p.m. Mr. Ammons'arrived on site.
The two Response Teams (Brian Septic Tank Service and Environmental
Hydrogeological Consultants) that arrived on site on Tuesday, December 5, 2000, remain
on -site as part of our cleanup effort. They will continue to work with us until all work is
complete.
This description was compiled from interviews with plant employees after the contact
was made to the National Response Center on December 5, 2000. All-6f th6i�61detiah

Dear Mr. Starr:
Per your conversation with Diane Leslie of Campbell Soup Company on Thursday January 25, 2001, we
are unable to sign the Statement of Correction certifying that all the violations listed in your letter dated
December 12, 2000 have been corrected. We are proposing the following plan to address the
deficiencies found during the inspection. This plan will be completed by June 1, 2001. Upon completion
we will submit the signed Statement of Correction.
The SPCC plan will be implemented as required by 40 CFR § 112.3(a) in accordance with the
guidelines for plan implementation at 40 CFR § 112.7 when all deficiencies have been corrected.
2. The fuel oil truck unloading area will be expanded and a curb will be installed to provide an
impervious surface to contain and direct any oil from a spill or a ruptured tanker into the oil water
separator system. The curb will encompass the existing transfer connections.
3. A dike will be installed around the fuel oil pump skid located in the boiler house.
A secondary containment system will be constructed utilizing the existing drainage ditch by installing
an oiftater separator dam and a discharge pipe. This system will provide sufficient containment for
any potential spills that might occur within the boiler house and the fuel transfer station.
Appropriate secondary containment and diversionary structures will be provided at all oil, liquid
ingredient and chemical storage areas.
6. Facility drainage from all storage areas will be directed to the wastewater treatment system.
7. Bulk storage requirements will meet the regulatory requirements as highlighted in your letter.

Dear Mr. Imhoff:
In accordance with an authorization from you on December 5, 2000, concerning the clean up of
a No. 6 fuel spill at the above -mentioned site, the following is submitted for your consideration.
Environmental Hydrogeological Consultants, Inc. (EHC, Inc.) were contacted on December 5,
2000 to respond to a No. 6 fuel spill (approximately 1,400 gallons) resulting from a shut off valve not
being closed during the switching of fuel for a boiler. Upon the arrival(approximately 6:00 p.m.-EHC
contacted 5:30 p.m.) of El-1C, Inc., it was discovered that the fuel impacted the Campbell Soup
Company's drain lines in the boiler house and subsequently entered a "wet well" outside that
discharges to a ditch along the northern portion of subject property and which leads into the Lumber
River. Approximately 5,000 feet of the ditch/canal was impacted with various levels of oil
contamination(none to free product present). Campbell Soup had already contacted the Fayetteville
Regional Office of the North Carolina Division of Water Quality and Mr. Dale Lopez was enroute to
subject site. The National Response Center, which is operated by the US Coast Guard, was contacted
due to the impact to surface water.
Upon our arrival on -site, Campbell Soup had already obtained Brian's Waste Recycling of
Lumberton, N.C., to remove free product from the "wet well" and they were performing this task with
three vacuum trucks. EHC determined that due to the concerns of the oil getting to the Lumber River,
EfJC placed 250 ft. of 18 inch hard boom at the junction of the ditch(near railroad trestle) at the
Lumber River and a point downstream, approximately 100 feet, in the river to prevent possible impact
to the river. Campbell Soup employees prior to our arrival also constructed a straw dam to help stop
the flow of the oil near the junction of "hot water discharge". Due to the large scope of work,

It was determined that Brian's Waste Recycling would concentrate on skimming and pumping the oil
from the "wet well" and coordinate with EHC on oil recovery from the ditch. EHC would provide
general spill cleanup overview and concentrate on cleanup operations along the ditch.
EHC environmental technicians concentrated on using oil only absorbents (5", 8" booms & 17"
X 19" pads) in combination with skimming operations, but it was Necessary to physically remove
contaminated debris (pine straw, limbs, etc) from the lower half of the ditch/canal to enhance our
cleanup effort. It was our opinion that complete ditch/canal excavation was not necessary due to water
depth (1-2 feet) was greater in this area of the ditch/canal and oil contamination was restricted in the
debris on top of the surface water. The area between the railroad and river(lowland) was inspected on
the morning and afternoon of 12/6/00 and no evidence of free product or oil sheen was present on the
water.
On December 6, 2000, two stream/river samples (Stream Sample 1 and Downstream) were
collected and split sampled with Mr. Dale Lopez with NCDENR-DWQ. Samples were collected and
transported to our office where same was prepared for Federal Express pickup and delivery to
Environmental Science Corp., Mt. Juliet, TN(Cert# ENV375) to be laboratory analyzed for oil and
grease using EPA Method 9071. Both samples were reported, BDL-Below Detection Limit_
Absorbent booms and pads were used extensively during the first two(2) weeks of the operation
and were reduced, but inspected almost daily until mid January. A&D Environmental provided waste
disposal for these used materials. The bard booms were used in the Lumber River until 12/23/00 and
during this time no evidence of a sheen or free product was observed. Hard boom was utilized to
ensure that if any free product was released due to rainfall event, etc., impact to the river,
downgradient, would be limited.
EHC and Brian's Waste Recycling continued the clean up the next five days on about a 24 hour
status due to oily water still being observed in the `vet well". A&D Environmental and Industrial
Services, High Point, N.C., were contacted to help with the pumping and disposal of fuellwater and
two(2) 22,000 gallon portable tanks were brought to the site to assist in oil/water storage. A total of
80,645 gallons of oil/water was disposed of by A&D Environmental during December, 2000. Brian's
Waste Recycling was utilized between 12/5 — 12/13/00. TEMCO, from Charlotte, NC was brought
in to utilize their high pressure wash system(40,000 psi) to assist in cleaning and jetting the lines leading
to and from the `vet well", with special effort on the "wet well".
Cleanup effort of the ditch involved skimming, placement of oil pads and absorbents and general
ditch "cleanup". The worst area impacted was the "first 1000 feet" of the ditch and soil excavation
from the ditch was concentrated. Contaminated soil was excavated along the ditch bank and stockpiled
on site for disposal on January 15, 2001 to load/haul seventeen(17) loads (431.84 tons) of petroleum
contaminated soil to Oak Hill Farms (Permit # SR 0600039) in Autryville, NC for disposal. Two soil
pile samples (Soil Sample A, Soil Sample B) were collected and laboratory analyzed using analytical
methods EPA 3550/5030 (Diesel Range Organics / Gasoline Range Organics), 9071 oil and grease, and
TCLP Metals as required by the disposal facility (see attached lab results).

EHC and A&D environmental returned to the scene on January 2, 2001 to pump out the
portable on -site tanks and properly clean the interior of these two(2) tanks. On this date, A&D
Environmental pumped and disposed of 20,450 gallons of fuel/water from the portable tanks. Due to
icing inside the tanks, EHC, Inc. returned to Campbell Soup Co. between the dates of 1 /17/01-1 /19/01
to clean the interior of the tanks. A&D Environmental was also on site during these dates to vacuum
and dispose of 4,691 gallons of remaining oil/water.
A complete receptor survey was not conducted of the area. A hand held Garmin GPS 12
(Global Positioning Satellite) was used to mark the position or location of the spill area (see subject).
We hope the information provided and actions taken are sufficient in addressing this incident.

permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or

Mike, per Ruth's conversation about outfall #5 (located at point A) is the location on where we exceeded
the limits on TSS (Total Suspended Solids). As she stated last year we exceeded this limit and did some
work what we thought would correct the issue. However it has not. I would like to correct the issue
think is taking place. Currently, during a rain event the water leaves from Old drop lot (point B) and
travels thru a rock bed (point C). Then it travels underground to point A and that is where we get a
sample to test. At point A we have created a sedimentation area which will continue to drive this
number above the limits. With our Stormwater Discharge Permit (NCG060029) Under Part 11, Section B,
Table 3, Tier One we have to do the following.
1) Conduct a stormwater management inspection of the facility within two weeks of receiving
sampling results.
i) Done. Did a second sample at the same point and came up with the TSS (1.87mg/L)
2) Identify and evaluate possible causes of the benchmark value exceedence.
i) Done. Complete on 3/25/2013
3) Identify potential, and select the specific: source controls, operational controls, or physical
improvements to reduce the concentrations of the parameters of concern, or to bring
concentrations within the benchmark range.
i) Done. Designed with BMCO grading cost plant order ($13,500).
4) Implement the selected actions within 2 months of the inspection.
i) Inspection was on February 13, 2013; Have till April 13, 2013 to correct. Can easily do in 2
days, I just need to get the plant order for signatures -on Monday (4/01/2013).
The next lay out is what I propose we need to do to correct the TSS high limit.

Take pipe that we had left over from the EQ tank and create a catch basin to send all the water correctly
to the sediment pond. This is what should have been done when the new drop lot was created. The
water will dump into a small sediment pond (point D). Settle out some before dumping into the larger
pond (point E). Once the larger pond retains enough water, then and only then will it spill over thru a
spill way (point F) and go out to the outfall 5. The cost is 13,500. 1 will write the plant order and get it
out for signatures.
I will also talk with Michael Lawyer (Division of Water Quality) in the Raleigh before submitting my Tier
One Plan. To see if this is on the right path.
