we are forwarding herewith the subject certificate of coverage (COC) to discharge under the
subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended). This COC #NCG050421 replaces your existing coverage under
NCGNE0747 which is rescinded effective immediately.
Please take notice that this certificate of coverage is not transferable except upon
approval of the Division of Energy, Mineral, and Land Resources. The Division of Energy,
Mineral, and Land Resources may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other, permits which may be
required by the Division of Energy, Mineral, and Land Resources, the Coastal Area
Management Act, or any other federal or local government permit that may be required.

Bradley,
.lust spoke with Evan Davenport with Quickie Manufacturing. He said there have been no operational changes, however
he still wants to obtain permit coverage for peace of mind and in case site conditions change. He came from another
facility that had permit coverage and is familiar with the requirements to develop an SPPP and conduct monitoring. The
FRO has no objections to issuance of permit coverage.
He also mentioned that the facility is currently collecting air compressor condensate for monthly pick up by Safety Kleen.
In order to save associated costs, he asked about discharging the condensate as well. I advised him that the general
stormwater permit did not cover this activity, but I would send him a copy of the application for coverage under the
NCG500000 general wastewater permit to be submitted to DWR.

Belinda this notification is a follow up to the incident at the site today in regards to the forklift battery acid going into
the drain. Approximately 1-2 gallons, worst case scenario, went into the drain. As stated, the solution was below the
drain tile exiting the area which assured that containment did occur. Porter Scientific (Certified Environmental
Specialist) arrived at 2:30 pm and removed the solution from the drain and will discard per there protocols. Let me
know if you have any other questions or concerns and thanks again for your support.

Dear Ms. Henson
We thank you kindly for your recommendations for our Cooling Tower and Chiller Discharges. We would like
this letter to serve as notice of our intentions. We are submitting this letter giving you a written Plan of Action
pertaining to these discharges. As outlined below these two options will be our plan of action;
We have begun the process of submitting internal documentation to our corporate office of our intent
to re-route the two discharge points into the existing sump that goes directly to the local POTW. We
have obtained permission from the local POTW as you recommended. We anticipate that within 60
days of this letter we will have the funds to begin this project. Once the project is completed we will
send you written documentation as confirmation that we have completed this task.
2. However, if the corporate office chooses not to re-route these discharges, and chooses to continue
discharging into the storm drain, we will file the application NCG500000. This will be done within 60
days of this letter should this be the route that corporate chooses to take. In this case we will contact
you in writing to let you know of our intent to file the application.

visit. The information provided by your office has been very helpful and we thank you for your time.

On February 1, 2011, Mike Lawyer with the Fayetteville Regional Office of the Division of Water Quality
conducted a site visit at the Quickie Manufacturing facility located at 2880 Kenny Biggs Road, Lumberton,
Robeson County, NC. The purpose of the site visit was in response to the submittal of a No -Exposure
Certification for Exclusion from stormwater permitting application and to determine the validity of the
information contained in the application. Mr. Lawyer met with Ms. Michelle Springer who provided a tour of
the facility's operational areas as well as outside storage and loading/unloading areas. Approval of the No -
Exposure Certification has been granted under separate letter.
During the site visit, observations were also made of the facility's cooling tower and closed -loop chiller. As
observed and discussed with facility personnel, each piece of equipment has a discharge that enters a storm
drain located within the adjacent loading dock. The purpose of this letter is to inform you that discharges of this
nature are considered as a wastewater and that permit coverage under an NPDES General Wastewater Permit
must be obtained for such discharges to continue. As an alternative, these discharges could be connected to a
sanitary sewer collection system provided that notification and prior approval is obtained from the local utility
that owns and operates the system and/or receiving wastewater treatment facility. This office strongly
encourages this alternative as it would eliminate an additional point source discharge of potential pollutants to
the environment/natural resource, which is the ultimate goal of the NPDES Program.
Within 30 calendar days from receipt of this letter, you are asked to respond by either, completing and
submitting the enclosed Notice of Intent application form for coverage under the applicable NPDES General
Wastewater Permit, or provide a written Plan of Action pertaining to the elimination of these discharges.
Failure to comply with one of the aforementioned options could result in the issuance of a Notice of
Violation, which is subject to the assessment of civil penalties up to $25,000 per day, per violation.

The Division has reviewed your submittal of the No -Exposure Certification for Exclusion from NPDES Stormwater
Permitting form. Based on your submittal and signed certification of no exposure at the above referenced facility as
well as observations made during a site visit conducted by Mike Lawyer with the Fayetteville Regional Cffice on
February 1, 2011, the Division is granting your conditional exclusion from permitting as provided for under 40 CFR
122.26(g), which is incorporated by reference in North Carolina regulations.
Please note that by our acceptance of your no -exposure certification, you are obligated to maintain no -exposure
conditions at your facility. If conditions change such that your facility can no longer qualify for a no -exposure
exclusion, you are obligated to immediately obtain NPDES permit coverage for your stormwater discharge.
Otherwise, the discharge becomes subject to enforcement as an un-permitted discharge. You must self re -certify
annually using the enclosed form, or obtain NPDES permit coverage for any stormwater discharges from your
facility. The annual self re -certification does not need to be submitted to DWQ unless requested.
Your conditional exclusion from permitting does not affect your facility's legal requirements to obtain environmental
permits that may be required under other federal, state, or local regulations or ordinances.

Submission of this No Exposure Certification constitutes notification that your facility does not require a
permit for stormwater discharges associated with industrial activity in the State of North Carolina because
it qualifies for a no exposure exclusion. A condition of no exposure at an industrial facility means all
industrial materials and activities are protected by a storm resistant shelter (with some exceptions) to
prevent exposure to rain, snow, snowmelt, andlor runoff.
For permitted facilities in North Carolina, DWQ must approve your application for No
Exposure Certification before this exclusion is effective. Until you are issued a No
Exposure Certification and your NPDES permit is rescinded, your facility must continue
to abide by the terms and conditions of the current permit.
Industrial materials or activities include, but are not limited to: material handling equipment or activities,
industrial machinery, raw materials, intermediate products, by-products, final products, or waste products.
Material handling activities include the storage, loading and unloading, transportation, or conveyance of
any raw material, intermediate product, final product or waste product. A storm resistant shelter is not
required for the following industrial materials and activities: drums, barrels, tanks, and similar containers
that are tightly sealed, provided those containers are not deteriorated and do not leak. "Sealed" means
banded or otherwise secured and with locked or non -operational taps or valves; adequately maintained
vehicles used in material handling; and final products, other than products that would be mobilized in
storm water discharges (e.g., rock salt).
A No Exposure Certification must be provided for each facility qualifying for the no exposure exclusion. In
addition, the exclusion from NPDES permitting is available on a facility -wide basis only —not for individual
outfalls. If any industrial activities or materials are, or will be, exposed to precipitation, the facility is not
eligible for the no exposure exclusion. By signing and submitting this No Exposure Certification form, you
certify that a condition of no exposure exists at this facility or site and are obligated to comply with the
terms and conditions of 40 CFR 122.26(g).
You are required to self re -certify your No Exposure Exclusion annually.

DWQ or the operator of the local municipal separate storm sewer system (MS4) into which the facility discharges (where

NCDENR Regional Office prior to issuance. The Regional Office may inspect your facility for
compliance with no exposure conditions prior to that approval. The Regional Office may also
inspect your facility at anytime in the future for compliance with the No Exposure Exclusion.
North Carolina General Statute 143-215.6 B(i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record, report, plan, or other

statement of a material fact in a rulemaking proceeding of contested case under this Article; or who falsifies, tampers with, or knowingly
