On January 23, 2018, Mike Lawyer, Environmental Program Consultant from the Fayetteville Regional Office
of the Division of Energy, Mineral and Land Resources (DEMLR), conducted a site inspection for the
Novipax, LLC facility located at 112 Sonoco Paper Mill Road, Rockingham, Richmond County, North
Carolina. A copy of the Compliance Inspection Report is enclosed for your review. Mr. William Allred,
Safety/Maintenance Coordinator, was also present during the inspection and his time and assistance is
greatly appreciated. The site visit and file review revealed that the subject facility is covered by NPDES
_ Stormwater General Permit NCG050000 under Certificate of Coverage NCG050355. Permit coverage
authorizes.the discharge of stormwater from the facility to receiving waters designated as Hitchcock Creek
(Midway Pond, Steeles Mill Pond), a Class C. waterbody in the Yadkin -Pee Dee River Basin.
As a result of the site inspection, the following deficiencies are noted:
1) Stormwater Pollution Prevention Plan (SPPP)
Facility's Stormwater Pollution Prevention Plan (SPPP) has not been reviewed/updated on an annual basis
per Part II, Section A, item 8 of the NCG050000 permit.
2) Qualitative Monitoring
- -Qualitative monitoring -has not been conducted and recorded in.accordance with permit requirements.

Inspection Summary:
Met with Michael Hardy, Plant Manager, and William Allred, Safety/Maintenance Coordinator. Reviewed facility's Stormwater
Pollution Prevention Plan'(SPPP), which appeared to contain all required components with the exception of a documented
annual review/update per Part II, Section A, item B of the NCGO50000 General Permit. Mr. Allred relayed that they are
actively working with a consultant to revise the SPPP. Available Qualitative Monitoring records were reviewed from 2013 to
present. Monitoring of stormwater discharges was not conducted/recorded for the second half of 2016 or first half of 2017.
The issuance of a Notice of Deficiency for failure to document an annual review/update of the SPPP and failure to
conduct/record monitoring for two periods was relayed. After the records review, observations were made at the stormwater
discharge outfalls. Previously, five outfalls had been identified, however Mr. Allred pointed out that what had been identified
as outfall 1 and outfall 2 actually convey stormwater to outfall 3 prior to discharging off the property. Therefore, the facility
currently has a total of three stormwater discharge outfalls instead of five. Since the previous inspection, the facility's only
above -ground storage, tank has been removed from the site. At the time of inspection, facility appeared to be well operated
and maintained. Other than an outside area for storage of used pallets, there appeared to be no other products/materials
stored outside exposed to stormwater. The possibility that the facility could qualify for a No Exposure Certification was
disca�ssed.

This COC is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection
Agency, dated October 15, 2007 (or as subsequently amended.)
The General Permit authorizes discharges of stormwater and specifies your obligations for discharge controls,
management, monitoring, reporting, and record keeping. Please review the new permit to familiarize yourself
with all of the changes. Parts III and IV contain the Standard Conditions, including Compliance and Liability,
Reporting, Monitoring and Records requirements; Operation and Maintenance obligations; and Definitions.
Your facility has six months from receipt of the COC to update its Stormwater Pollution Prevention Plan
(SPPP) to comply with changes in SPPP requirements. Other changes are effective immediately. Please note
that Tier 3 Actions (if applicable) are triggered by four benchmark exceedances beginning on the effective
date of the renewal permit and do not count prior exceedances. Please visit our website above to review the
new General Permit carefully.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit'
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree.

On September 25, 2008, I, Michael Lawyer from the Fayetteville Regional Office of the Division
of Water Quality; conducted a site inspection at the Sealed Air Corporation facility located at 112
Sonoco Road in Rockingham, Richmond County, North Carolina. A copy of the Compliance Inspection
Report is enclosed for your review. Your time and assistance during the inspection is greatly
appreciated. Stormxvater from this facility drains to Hitchcock Creek, a Class C stream located in the
Yadkin -Pee Dee River Basin. The site visit and file review revealed that the subject facility is covered

Inspection Summary:
Inspection conducted with Mr. Ken Long, Plant Quality Manager. A review was conducted of the facility's Stormwater
Pollution Prevention Plan (SPPP) and Qualitative Monitoring records. Some minor revisions to the SPPP were suggested
concerning the new Qualitative Monitoring requirements in the current general permit and documentation of an annual
review and/or update as well as a certification of the absence of any non-Stormwater discharges not covered by other
permits. Records of Qualitative Monitoring from initial permit coverage to the date of the inspection were available and
complete. A tour of the facility including manufacturing areas and the grounds and outfalls was conducted. At the time of
inspection, the facility appeared to be well -maintained and the outfalls did not show the presence of any obvious
pollution.

Bill,
Conducted a site visit for this facility on 12/4. Met with Mr. Ken Long, Plant
Quality Manager. Facility developed an SPPP back in 1996 when they were a textile
Mill. Facility stores pallets outside and their waste dumpster/compactor is
uncovered and very close to an open pipe that collects and transports rainwater to
one of their two outfalls. However, there were no signs of trash or materials
getting into the pipe at the time of my visit. They have a rather large fuel oil
tank outside, which does have secondary containment and they are keeping a log book
of visual inspections of rain water collected in the containment area before
releasing onto the ground. The Fayetteville Regional Office recommends issuing
coverage under the NCGO50000 general permit for this facility.

forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general pen -nit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this pen -nit are
unacceptable to you; you have the right to request an individual permit by submitting 'an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or pen -nits required by the
Division of Land Resources, Coastal Area Management Act or any other federal or local governmental
permit that may be required.

Bill,
Conducted a site visit for this facility on 12/4. Met with Mr. Ken Long, Plant
Quality Manager. Facility developed an SPPP back in 1996 when they were a textile
mill. Facility stores pallets outside and their waste dumpster/compactor is
uncovered and very close to an open pipe that collects and transports rainwater to
one of their two outfalls. However, there were no signs of trash or materials
getting into the pipe at the time of my visit. They have a rather large fuel oil
tank outside, which does have secondary containment and they are keeping a log book
of visual inspections of rain water collected in the containment area before
releasing onto the ground.` The Fayetteville Regional Office recommends issuing
lcoverage under the NCG050000 general permit for this facility./
