Dear Mr, Olsen:
On June 4, 2018, a site inspection was conducted for the Graphic Packaging International, Inc., Lumberton
facility located at 2801 Kenny Biggs Road, in Robeson County, North Carolina. A copy of the Compliance
Inspection Report is enclosed for your review. Mr. Jonathan Brooks, EIS&S Manager and Glenda Hunt,
EH&S Coordinator were also present during the inspection and their time and assistance is greatly
appreciated. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater
General Permit NCG050000 under Certificate of Coverage NCG050168. Permit coverage authorizes the
discharge of stormwater from the facility to receiving waters designated as Gum Branch Canal, a Class C
SW waterbody in the Lumber River Basin.
As a result of the inspection, the facility was found be in compliance with the conditions of the NCG050000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.

Inspection Summary:
Melissa Joyner met with Mr. Jonathan Brooks, EH&S Manager and Glenda Hunt, EH&S Coordinator at the Graphic
Packaging International, Inc. facility in Lumberton, NG. The Stormwater Pollution Prevention Plan (SPPP) was reviewed. It
appeared well -organized and contained the information required by General Permit NCG050000. The Site Map was detailed
but should indicate topography and the percentage of each drainage area that is impervious. The SPPP has not been
annually reviewed and updated since 2015. Under the Spill Team and Plant Emergency Contacts, the Plant Engineer name
should be changed to the current Plant Engineer, Jonathan Brooks.
Outfalls #146 were inspected and appeared in good condition. It may be possible to reduce the number of outfalls which are
currently being monitored if there are outfails which share the same drainage area boundary and total contributing area.

if a facility is required to sample multiple discharge locations with very similar starmwater discharges, the
permittee may petition the Director for Representative Outfali Status (ROS). DWQ may grant Representative
Outfall Status if stormwater discharges from a single outfall are representative of discharges from multiple
outfalls. Approved ROS will reduce the number of outfalls where analytical sampling requirements apply.
If Representative Outfall Status is granted, AU. outfalls are still subject to the ualitotive monitoring
requirements of the facility's permit —unless otherwise allowed by the permit (such as NCGO20000) and DWQ
approval. The approval letter from D WQ must be kept on site with the facility's Stormwa ter Pollution
Prevention Plan. The facility must notify DWQ in writing if any changes affect representative status.

North Carolina General Statute 143-215.6 B(i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record,
report, plan, or other document filed or required to be maintained under this Article or a rule implementing this
Article; or who knowingly makes a false statement of a material fact in a rulemaking proceeding or contested case
under this Article; or who falsifies, tampers with, or knowingly renders inaccurate any recording or monitoring device
or method required to be operated or maintained under this Article or rules'of the [Environmental Management]
Commission implementing this Article shall be guilty of a Class 2 misdemeanor which may include a fine not to exceed
ten thousand dollars ($10,000).
I hereby request Representative Outfall Status for my NPDES Permit. I understand that ALL outfalls are still
subject to the qualitative monitoring requirements of the permit, unless otherwise allowed by the permit
and regional office approval. I must notify DWQ in writing if any changes to the facility or its operations
take place after ROS is granted that may affect this status. If ROS no longer applies, I understand 1 must
resume monitoring of all outfalls as specified in my NPDES permit.

The easiest thing to do is to go to this website:

Run the Permit Contact Summary report for your COC (NCG050168), and then let us know what needs to be changed.
You can either make changes on that that same page, scan it, and send it back to us, or you can simply e-mail us the
contact information details that need updates, We can make changes to Permit Contact, Facility Contact, and Owner
Contact that way. However, another person in our section (Deborah Reese) is the only person that can make any
changes to Billing Contact (at the end of that report). Also, any changes to the Owner Affiliation (i.e., the person legally
responsible for the permit) need to be submitted with a separate signed form. I can send you that if you need it; just let
me know. (The forms, depending on what the change is, are also on that website up there.)
Please email the requested updates to Laura Alexander in our group - she is copied here. Let me know if you have any
other questions!

Hi Pete,
Thanks for your call yesterday. Attached is the e-mail confirmation that went to your predecessor Evan Davenport in
May 2013. Although it shows the previous permit's effective and expiration date, it is in fact a confirmation of coverage
effective in 2013. Your facility does have active permit coverage. Also attached is the full text of the General Permit
that you should follow. You can also access the permit and all monitoring forms
here: http://portal.ncdenr.org/web/Ir/npdes-stormwater (just click on the "General Permits" tab or "Monitoring
Forms" tab)
Finally, attached is a copy of the COC for this facility that I created this morning. Unfortunately the website we used for
electronic COC renewals now prints the effective date of the COC as the current date, rather than June 1, 2013, because
we are so far past that renewal window. I can't change that. We would have to manually generate a COC, but we are
not even in the same Division that we were in 2013 (which also makes today's copy inconsistent because it still says
Division of Water Quality). I would recommend keeping this correspondence with your records to verify that your COC
coverage was in fact effective on June 1, 2013 —the same day as General Permit NCG050000. In fact, if you go to our
Active Permits Map, you can search for your permit number and verify that. Here's the website, and below that is a

This COC is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection
Agency, dated October 15, 2007 (or as subsequently amended.)
The General Permit authorizes discharges of stormwater and specifies your obligations for discharge controls,
management, monitoring, reporting, and record keeping. Please review the new permit to familiarize yourself
with all of the changes. Parts III and IV contain the Standard Conditions, including Compliance and Liability,
Reporting, Monitoring and Records requirements; Operation and Maintenance obligations; and Definitions.
Your facility has six months from receipt of the COC to update its Stormwater Pollution Prevention Plan

In response to your renewal application for continued coverage under stormwater General Permit NCG050000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG050000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since the your last Certificate of Coverage include the following:
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part II Section A —Several minor expansions and clarifications of the required content of the Stormwater Pollution
Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part II Section B — Qualitative monitoring must now be conducted during a representative storm event,
• Part II Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part II Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require that discharges from the vehicle maintenance areas be sampled twice per year, and that the
sampling results be compared to new numerical benchmark values. (The previous cutoff concentrations have been
eliminated.)
• Part II Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events
show no benchmark exceedences.
• Part II Section C —This part contains a new provision that four exceedences of any particular benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct the
permittee to apply for an individual permit, or may direct the implementation or installation of specific stormwater control
measures.

Water Quality, conducted a site inspection at the Graphic Packaging International Inc - Lumberton

ADDENDUM
The National Pollutant Discharge Elimination System (NPDES) program was established under the
federal Clean Water Act and then delegated to the Division of Water Quality for implementation in
North Carolina. The NPDES permitting program for stormwater discharges was established in 1990.
Phase 1 focuses on site and operations planning to reduce pollutant sources.
Through the NPDES permitting program industrial facilities that fall into one of the subject ten
categories are required to obtain permit coverage under a general pennit or an individual pennit,
depending; upon the facilities SIC code and the industrial activity occurring at the facility. There are
currently 19 active general stonnwater permits in North Carolina. One condition that is applicable to
both the general permits and individual stonnwater pen -nits is the requirement to develop and implement
site -specific comprehensive stonnwater pollution prevention plans (SPPP). These plans are required to
include a comprehensive evaluation of the site and operations to reduce pollutant sources and prevent
pollutant discharge.

monitoring forms, and a Notice of Intent [which is the application of for coverage under a NPDES
General Stormwater Pen -nit] can be found at this web site.
The following items are a subset of pennit requirements and are listed to assist you and help bring your
attention to certain permitting issues, process and condition requirements of your permit. That said;
please see the above -mentioned web site in order to obtain a copy of the pennit such that you may fully
comply with all the conditions therein. Failure to properly comply with conditions of the permit, like
those bulleted below, constitutes violations and is subject civil penalty assessment of up to
$25,000.00 per day.
• Industrial facilities that fall into one of the subject ten categories are required to obtain permit
coverage under a general NPDES stonnwater permit or an individual NPDES stonnwater permit
• All persons desiring to have facilities covered by a General Pen -nit must register with the
Division of Water Quality (DWQ) by the filing of a Notice of Intent (NOI) and Applicable Fees.
• This Notice of Intent (NOI) must be submitted and a Certificate of Coverage issued prior to any
discharge of stonnwater associated with the industrial activity, process wastewater, and/or mine
dewatering.
• Once the Notice of [ntent is received, DWQ will review and determine whether a Certificate of
Coverage (COC) may be issued. If this Certificate of Coverage is issued the facility is covered
by the General Permit. That is, the permittee is authorized to discharge stormwater, process
wastewater, and/or mine dewatering pursuant to the type of General Pennit until the pennit
expires or is modified or revoked.
• Any other point source discharge to surface water of the state is prohibited unless it is an
allowable non-stonnwater discharge or is covered by another pennit, authorization or approval.
• There are currently 19 active general permits. Pen -nit conditions vary accordingly; however, most
of the pennits have following condition requirements (you must read your entire pennit to ensure
full compliance):
o Development and compliance with the Stormwater Pollution Prevention Plan
o Collection, reporting, and recording of Analytical Monitoring
o Collection, reporting and recording of Qualitative Monitoring
o Collection, reporting and recording of Vehicle Maintenance Monitoring
o Records — Visual monitoring shall be documented and records maintained at the facility
along with the Stormwater Pollution Prevention Plan. Copies of the analytical monitoring
results shall also be maintained on -site. The Permittee shall retain records of all
monitoring information, including all calibration and maintenance records and all original
strip chart recordings for continuous monitoring instrumentation, and copies of all reports
required by this general permit for a period of least 5 years for the date of the sample
measurement, report or application.
o Expiration — The pernittee is not authorized to discharge after the expiration date of the
general permit. In order to receive automatic authorization to discharge beyond the
expiration date, the permittee shall submit forms and fees as are required by the agency
authorized to issue permits no later than 180-days prior to expiration date. Any permittee
that has not requested renewal at least 180 days prior to expiration, or any permittee that
does not have a pennit after the expiration and has not requested renewal at least 180
days prior to expiration, will be subject to enforcement. Failure to renew ,your permit is
a violation.
o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is
not transferable to any person expect after notice to and approval by the Director. The
Director may require modification or revocation and reissuance of the Certificate of
Coverage to change the name and incorporate such other requirements as may be
necessary under the Clean Water Act. The Permit is NOT transferable.
o Name Change, Closure or facility Ownership change— Permittec is required to notify the
Division in writing in the event the pennitted facility is sold or closed. The Division of
Water Quality views changes of name or ownership as a modification that requires the
Director's approval and reissuance of the permit. Name and ownership changes required
you to complete a Name/ownership Change form from the Division and mail it back to
the Division of Water Quality along with all appropriate information.
o Proper Operation and Maintenance is a requirement of the permit.

Inspection Summary:
Inspection conducted with time and assistance provided by Mr, Mitch Cummings, EH&S Coordinator, Facility
manufactures thin cardboard boxes and cartons for food products such as microwaveable meals, cereals, etc. Production
as well as storage of materials and chemicals all occur inside. Outside the facility, used pallets are stored and there are
several large, open -top dumpsters used for scrap metal and other wastes. Facility personnel have identified seven
outfalls, however some of these are not true outfalls and others receive only roof drainage from areas of the roof where no
facility operations occur. For example, the facility has a stormwater basin with two inlets on either side that have been
identified as seperate outfalls when the outlet of the basin should be what is monitored as it would convey stormwater off
the property. Also, there are four outlets to the roadside ditch that receive flow only from the roof, however only one of
these receives flow from an area where there are exhaust stacks from the printing area that may pick up particulates from
the stacks. Mr. Cummings was advised that he could reduce the number of outfalls he monitors from seven to three,
which would consist of the outlets from the facility's two stormwater basins located at the rear and side of the property and
the roof drain outlet that receives flow from the printer exhaust stacks area of the roof. The facility's Stormwater Pollution
Prevention Plan was reviewed and found to be very detailed and comprehensive. Visual monitoring reports for the seven
identified outfalls were available for the current and previous years of permit coverage. At the time of inspection, the
facility appeared to be very well maintained both inside and out.

Your facility is currently covered for stormwater discharge under General Permit NCG050000. This permit expires on
April 30, 2008. The Division staff is currently in the process of renewing this permit. When a draft is available for
notice and public comment, it will be posted on our website at http://h2o.enr.state.nc.us/su/publicnotice,htm. Please
continue to check this website to review and comment on proposed changes to the permit. Once the permit is
reissued, your facility would be eligible for continued coverage under the reissued permit.
To assure consideration for continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit. Enclosed you will find a Permit Coverage Renewal Application Form. The
application must be completed and returned by November 16. 2007. Letters confirming our receipt of the
completed application will not be sent.
Failure to request renewal within the time period specified may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $25,000 per day.
"No exposure exclusion" is now available to industrial facilities in any of the categories of "storm water discharges
associated with industrial activity," (except construction activities). If you feel your facility can certify a condition of
.,no exposure", i.e. the facility industrial materials and operations are not exposed to stormwater, you can apply for
the no exposure exclusion. For additional information contact the Central Office Stormwater Staff member listed
below or check the Stormwater Permitting Unit Web Site at http://h2o.enr.state.nc.us/su/Noexpexempt.htm.
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions can be found on the bottom of the form. You will be notified when
the rescission process has been completed.
