(COC) is available. This permit is reissued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.)

The General Permit authorizes discharges of stormwater, and it specifies your obligations with
respect to discharge controls, management, monitoring, and record keeping. Please review the
new permit to familiarize yourself with all changes in the reissued permit. Significant changes
to the General Permit are outlined in documents available on the website above. Your
facility has six months from receipt of the permit to update your Stormwater Pollution
Prevention Plan (SPPP) to reflect any new permit requirements.

How does the new General Pennit affect Tier Status?
If vehicle- maintenance activities (VMA) at your site trigger analytical, monitoring, the first
sample period of your permit began on June 1, 2018. Tier 3 actions in Part IV of your permit
are triggered by benchmark exceedances on four occasions beginning on the effective date of
this permit and do not count -prior exceedances. Likewise, Tier 2 actions are triggered by two
consecutive benchmark exceedances beginning on the effective date of this permit and do not
count prior exceedances. However, if your facility is.already in Tier 2 (monitoring stormwater
discharges monthly) or Tier 3; you must continue monthly monitoring until relieved through
conditions of the permit or by DEMLR staff approval.
When does electronic DMR reporting start?
We recognize that many permittees under this permit do not have activities that trigger analytical
monitoring. However, for those that do, note that permittees will receive notification when
our eDMR system is ready for registration to begin reporting monitoring.data

On February 26, 2018, 1, Mike Lawyer along with Jason Gill with the Fayetteville Regional Office of the
Division of Energy, Mineral and Land Resources (DEMLR) conducted a site inspection for the Cascades
Tissue Group - Rockingham facility located at 805 Midway Road, Rockingham, Richmond County, North
Carolina. A copy of the Compliance Inspection Report is enclosed for your review. Ms. Karen Bellamy,
Environmental Manager, was also present during the inspection and her time and assistance is greatly
appreciated. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater
General Permit NCG050000 under Certificate of Coverage NCG050153. Permit coverage authorizes the
discharge of stormwater from the facility to receiving waters designated as Hitchcock Creek (Midway Pond,
Steeles Mill Pond), a Class C waterbody in the Yadkin -Pee Dee River Basin.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG050000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to $25,000 per day for each violation. If you or your staff have any questions,

Inspection Summary:
Met with Karen Bellamy, Environmental Manager. Ms. Bellamy relayed that Mickey Lee has replaced Eric Taylor as the Mill
Manager. It was also relayed that several ASTs have been replaced/upgraded since the previous inspection. Reviewed
facility's Stormwater Pollution Prevention Plan (SPPP) and associated documentation of facility inspections, employee
training, and annual reviews/updates, Records of semi-annual Qualitative Monitoring from 2013 through 2017 for the facility's
five stormwater discharge outfalls were also reviewed. After the records review, a tour of the facility's operations was
conducted and then observations were made of site conditions including secondary containment areas, loading/off-loading
areas, storm drains and outfalls. At the time of inspection, facility determined to be in compliance with the conditions of
Stormwater General Permit NCG050000.

I phoned Larry Cobler about the status of the foam at the POTW. It was worst today than on Wednesday, but the
Effluent has not yet been affected (it still looks good). Larry talked With the SlU's, and they all say that they have not
changed anything. But, Cascades Tissue Group did say that they had starting adding a bonding agent that in orderto
increase the strength of their paper product for a client that specifically requested increased strength. At this time,
Cascades Tissue Group plans continue using the bonding agent, and then abruptly stop running the bonding agent on
July filth or 12th. They want to fill their customer's specifications request on the product order. Since the POTW Effluent
still looks good, Larry Cobler intends to continue evaluating the effect of the foam. If there is improvement at the
POTW after July 121h , then Larry will request that Cascade Tissue stop using the bonding agent for any future
products. At this time, Larry is using the sprinklers on the clarifiers and he started using a small amount of defoamer (I
think he's adding defoamer as the mixed liquor leaves the aeration basin). Larry has been using phosphoric acid to
increase the amount of phosphorus available. He sent me the MSDS for the bonding agent --- an alkyl glycol or
propylene glycol, and an alcohol --- n-octanol.
agree with Larry, that the use of the bonding agent coincides with the time period that the foam problem
appeared. According to it's MSDS, it decomposes to organic acids, and some sources say that organic acids can
encourage filamentous growth. 1 think that recent heavy rains may have washed out additional amounts of grease that
was adhering to the inside of the collection system. But, Larry said that he had not noticed an increase of grease at the
headworks. I think that the recent flow increases would also change the recommended ratio (100:5:1 --- BOD:N:P) in
the POTW for avoiding filamentous bacteria.
Larry said that he will keep FRO posted on the foam situation.

Quality, conducted a site inspection at the Cascades Tissue Group - NC Inc. facility located at 805 Midway
Road in Richmond County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your

Inspection Summary:
Facility personnel have revised their SPPP to reflect the recent changes to the NCG050000 general permit. Monitoring is
conducted and documented according to the permit requirements and observations of facility conditions are also well
documented with pictures and other notations. Facility has addressed the secondary containment concern expressed
during the previous inspection and have provided containment structures for old equipment and scrap materials that were
previously exposed to the elements. A lot of proactive work has been done with more planned for the near future to
address specific concerns related to stormwater and the environment in general. Staff should be commended on these
efforts and encouraged to continue this proactive approach.

Stormwater Outfall Analysis

Dear Mr. Bennett,
Please see the enclosed Stormwater Analyses from our Outfall locations.
Outfall 1 and Outfall (2CS2D-Same location) showed TSS over our limit. We did some
corrective action and CSX will be doing work help with this outfall (2c&2D)
Upon testing within the time frame of the data received. We found Outfall 1, 2C, 2D to be fine,
but outfall 5 had a TSS limit over our limit We have not had an issue with this outfall in the
past. I have made a request for a settling basin to be put at this location in 2009.
We performed Capital Projects for improvements in 2008 and also are looking to perform
capital work in 2009. We don't perform vehicle maintenance at our location.

Water Quality, conducted a site inspection for the Cascades Tissue Group -NC Inc facility located at 805
Midway Road in Rockingham, Richmond County, North Carolina. A copy of the inspection report is
enclosed for your review. Stormwater from the facility drains to 1-litchcock Creek., a Class C water
located in the Pee Dee River Basin. The Site visit and file review revealed that. the subject facility is
covered by NPDES Stormwater General Permit-NCG050153.

ADDENDUM
The National Pollutant Discharge Elimination System (NPDES) program was established under the
federal Clean Water Act and then delegated to the Division of Water Quality for implementation in
North Carolina. The NPDES permitting program for stormwater discharges was established in 1990.
Phase I focuses on site and operations planning to reduce pollutant sources.
Through the NPDES permitting program industrial facilities that fall into one of the subject ten
categories are required to obtain permit coverage under a general permit or an individual pert -nit,
depending upon the facilities SiC code and the industrial activity occurring at the facility. There are
currently 19 active general stormwater permits in North Carolina. One condition that is applicable to
both the general permits and individual stormwater permits is the requirement to develop and implement
site -specific comprehensive stormwater pollution prevention plans (SPPP). These plans are required to
include a comprehensive evaluation of the site and operations to reduce pollutant sources and prevent
pollutant discharge.

monitoring forms, and a Notice of Intent [which is the application of for coverage under a NPDES
General Stormwater Permit] can be found at this web site.
The following items are a subset of permit requirements and are listed to assist you and help bring your
attention to certain permitting issues, process and condition requirements of your permit. That said;
please see the above -mentioned web site in order to obtain a copy of the permit such that you may fully
comply with all the conditions therein. Failures to properly comply with conditions of the permit,
like those bulleted below, constitutes violations and are subject to a civil penalty assessment of up
to $25,000 per day, per violation.
• Industrial facilities that fall into one of the subject ten categories are required to obtain permit
coverage under a general NPDES stormwater perrnit or an individual NPDES stormwater permit
• All persons desiring to have facilities covered by a General Permit must register with the
Division of Water Quality (DWQ) by the filing of a Notice of Intent (NOI) and Applicable Fees.
• This Notice of Intent (NOI) must be submitted and a Certificate of Coverage issued prior to any
discharge of stormwater associated with the industrial activity, process wastewater, and/or mine
dewatering.
• Once the Notice of Intent is received, DWQ will review and determine whether a Certificate of
Coverage (COC) may be issued. II this Certificate of Coverage is issued the facility is covered
by the General Permit. That is, the permittee is authorized to discharge stormwater, process
wastewater, and/or mine dewatering pursuant to the type of General Permit until the permit
expires or is modified or revoked.
• Any other point source discharge to surface water of the state is prohibited unless it is an
allowable non-stormwater discharge or is covered by another permit, authorization or approval.
• There are currently 19 active general permits. Permit conditions vary accordingly; however, most
of the permits have following condition requirements (_you must read your entire permit to ensure
full compliance):
o Development and compliance with the Stormwater Pollution Prevention Plan
o Collection, reporting, and recording of Analytical Monitoring

o Collection, reporting and recording ol'Qualitative Monitoring

o Collection, reporting and recording of Vehicle Maintenance Monitoring
o Records — Visual monitoring shall be documented and records maintained at the facility
along with the Stormwater Pollution Prevention Plan. Copies of the analytical monitoring
results shall also be maintainer) on -site. The Permittee shall retain records of all
monitoring information, including all calibration and maintenance records and all original
strip chart recordings for continuous monitoring instrumentation, and copies of all reports
required by this general permit for a period of least 5 years for the date of the sample
measurement, report or application.
o Expiration — The permittee is not authorized to discharge after the expiration date of the
general permit. In order to receive automatic authorization to discharge beyond the
expiration date, the permittee shall submit forms and fees as are required by the agency
authorized to issue permits no later than 180-days prior to expiration date. Any permittee
that has not requested renewal at least 180 days prior to expiration, or any permit:tee that
does not have a permit after the expiration and has not requested renewal at least 180
days prior to expiration, will be subject to enforcement. Failure to renew your permit is
a violation.
o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is
not transferable to any person expect after notice to and approval by the Director. The
Director may require modification or revocation and reissuance of the Certificate of
Coverage to change the name and incorporate such other requirements as may be
necessary under the Clean Water Act. The Permit is NOT transferable.
o Name Change, Closure or Facility Ownership change — Permittee is required to notify the
Division in writing in the event the permitted facility is sold or closed. The Division of
Water Quality views changes of name or ownership as a modification that requires the
Director's approval and reissuance of the permit. Name and ownership changes required
You to complete a Namc/ownership Change Form from the Division and mail it back to
the .Division of Water Quality along with all appropriate information.
o Proper Operation and Maintenance is a requirement of the permit.

Inspection Summary:
Facility has a well -developed Stormwater Pollution Prevention Plan that is detailed yet easy to follow, Although analytical
monitoring is not a requirement under stormwater general permit NCG050000, analytical monitoring has been conducted
at each of the facility's outfalls throughout each year of permit coverage. As discussed during the inspection, this
monitoring may be eliminated at the discretion of facility personnel. As required by the general permit, semi-annual
qualitative monitoring has been conducted at each of the facility's outfalls and records were available at the time of
inspection for every year of permit coverage. Some of the facility's five current outfalls receive only stormwater runoff
from roof drains and parking areas. It was relayed during the inspection that these particular outfalls could be removed
from the monitoring program as they do not meet the intent of the general permit, which covers point source discharges
from associated industrial activities. Although secondary containment is provided around all but one tank (Gray Water
Tank) at the facility, it appeared at the time of inspection that there may be inadequate storage capacity around some
areas should a major leak develop, accidental damage occur or possible vandalism. Plans for addressing this area of
concern should be discussed between facility personnel and an engineer/consultant.

In response to your renewal application for continued coverage under the subject permit, the Division
of Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit
is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency
dated December 6, 1983.
The following information is included with your permit package:
■ A copy of the stormwater general permit.
■ A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit. This form must be completed and returned to the Division within 30 days of receipt of
this letter. DO NOT send the SPPP with the signed form.
■ Five copies of Analytical Monitoring forms.
m Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program which outlines program components
and addresses frequently asked questions.
■ A corrected Certificate of Coverage if you indicated a name or address change on the Renewal
Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
