Dear Mr. Chavis:
On January 9, 2018, a site inspection was conducted for The Plastek Group -Hamlet Manufacturing Division
facility located at 1015 County Home Road, Hamlet, Richmond County, North Carolina. A copy of the
Compliance Inspection Report is enclosed for your review. Your time and assistance during the inspection is
greatly appreciated. The site visit and file review revealed that the subject facility is covered by NPDES
Stormwater General Permit NCGO50000 under Certificate of Coverage NCG050002. Permit coverage
authorizes the discharge of stormwater from the facility to receiving waters designated as Falling Creek
(McDonalds Pond), a Class WS-III waterbody in the Yadkin -Pee Dee River Basin.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG050000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to $25,000 per day for each violation. If you or your staff have any questions,

Inspection Summary
Met with Anthony Chavis, Facility Engineer. Facility ownership has changed from Rexam Packaging Systems, Inc. to The
Plastek Group -Hamlet Manufacturing Division. Facility's industrial activity type has not changed. A Stormwater Permit
Name/Ownership Change Form was provided to Mr. Chavis to be completed and submitted to our Central Office in order to
update our files, Reviewed the facility's Stormwater Pollution Prevention Plan (SPPP), which is up-to-date and complete with
all permit -required components. Some minor revisions are needed to the detailed site map in order to contain all items as
listed in the permit. Qualitative monitoring records from 2013 to 2017 for the facility's two stormwater discharge outfalls were
available for review. No onsite vehicle maintenance isconducted, therefore analytical monitoring is not required for this
facility. After the records review, observations were made of the loading docks and stormwater outfalls as well as storage
areas for scrap metal and drums/totes. Facility appeared at the time of inspection to be well operated and maintained.

On November 9, 2006, I, Michael Lawyer along with Dale Lopez from the Fayetteville Regional
Office of the Division of Water Quality, conducted a site inspection for the Owens Illinois Inc -Hamlet
facility located at 1015 County Home Road in Richmond County, North Carolina. A copy of the Water
Compliance Inspection Report is enclosed for your review. Mr, Greg Germain, Maintenance Systems
Mana(,er, was also present during the inspection and his time and assistance is greatly appreciated.
Stormwater from the facility drains to Falling Creek (McDonalds Pond), a Class WS-ill water located in
the Yadkin River Basin. The site visit and file review revealed that the subject facility is covered by
NPDES Stormwater General Perm it-N CCJ05 0002.
Accordingly, the following observations were noted during the Division of Water Quality
inspection:

Other Observations:
The Safety Kleen Corporation established the Stormwater Pollution Prevention Plan (SPPP) in 2004.
While the SPPP is well written and quite detailed, it is not apparent through proper documentation that
an annual review and update is being conducted. Please be reminded that Part II, Section A, Item Tof
the NCG050000 General Permit states in part: "The Stormwater Pollution Prevention Plan shall be
reviewed and updated on an annual basis." During the inspection, a second outfall was identified
consisting of a swale that leads to a culvert underneath the main railroad line adjacent to the rail spur
where raw material is transported in and stored for use in the manufacturing process. This additional
outfall should be added to all pertinent documentation within the SPPP and monitored as required by the
NCG050000 General Permit. Please refer to the inspection summary located on page 2 of the enclosed
Water Compliance Inspection Report for additional comments.
Please be advised that violations of the NPDES General Permit are subject to a civil
penalty assessment of up to $25,000 per clay for each violation.. If you or your staff has any

Facili#y appeared at the time of inspection to be very well operated and maintained both inside the
manu acturing warehouse as well as the grounds. The Stormwater Pollution Prevention Plan (SPPP) is
well -written and easy to follow, however there is a lack of documentation concerning the permit requirement
of an annual review and update. The area where new and used oil is kept is in very good order and not only
has secondary containment, but is also equipped with a shelter. A black mat has been placed between the
rails of the rail spur area where raw product is tranported in and stored. This allows for an easy visual of
product spillage and any necessary cleanup as the product is white in color, which would obviously contrast
with the black mat. Adjacent to the rail spur, a second stormwater outfall was identified, which would
convey stormwater through a grass swale and into a culvert leading off of the property underneath the main
railroad line. The identification of this second outfall should be incorporated into the SPPP and monitored
per the conditions of the permit. This facility has taken a proactive approach to eliminating stormwater
runoff contamination.

permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection
Agency, dated December 6, 1983.
