The Systems Logistics-Vertique ("Systems Logistics") facility located in Arden, North Carolina is
submitting the following information to the Division of Water Quality ("Division") in regards to the
facility general industrial permit NCG030636. Please note, Systems Logistics was formerly named
ITW Warehouse Automation.
The current monitoring period is recognized as January 1, 2018 through June 30, 2018. The sample
analysis attached was collected from outfall 002. The results were reported within 30 days of the date
of collection. For 2018, facility personnel are scheduled to be trained to the SPPP and best
management practices. Also, specific personnel were identified to properly collect samples and
report out as needed.
Attached is the discharge monitoring report required for monitoring period year I -period 1. The
sampling requirements per Section D of the General Permit are below:

d. Environmental Science Corporation (ESC)

The benchmark level for pollutants has not been exceeded this monitoring period. A pH meter will
be purchased and kept onsite for testing the stormwater once it is collected to verify the result is in
compliance with permit NCG030636.
Appalachian Environmental Resources, Inc. has been contracted to handle the water quality program
at the facility. The Stormwater Pollution Prevention Plan (SPPP) has been developed and all
information pertaining to the NPDES permit for Metal Fabrication has been completed.

In accordance with your application for a discharge permit received on May 10, 2012 we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject
state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal.or local governmental permit that may be required.
Please note that the attached permit expires on October 31, 2012. Your COC will be
automatically renewed to cover the full 5 year term of the soon to be reissued permit. In the
interim you must abide by the terms of the expiring permit. You can review the proposed

I had met with Mr. Glenn Clifton who is in charge of environmental compliance for Prestage Foods back in April in
response to a flooding complaint from a neighbor at the 421 Feed Mill in Clinton. As usual, there was nothing I could do
about the flooding complaint, but realized that the facility was without permit coverage so I provided Mr. Clifton a copy
of the NOL He relayed that they had another feed mill facility in Moultonville. Glad to see they applied for coverage for
both facilities. The completed NOI's look good to me.

Section E: Assessment of Best Management Practices (BMPS) for facilities that use or
process animal fats/byproducts.
Prestage Farms, Inc. 421 Feed Mill is a facility where feed is manufactured along with
grain purchasing for pork production. Support services include a warehouse, lab, garage
and maintenance department. This facility does use processed animal fats/byproducts in
the manufacture of feed.
Animal fat is delivered to the 421 Feed Mill by tanker trucks to a delivery point where
animal fat is pumped to. two aboveground storage tanks with secondary containment.
This secondary containment wall also includes other tanks with liquid feed ingredients.
Potential spills could occur at the site to include the delivery point between tanker truck
connection and the receiving pipe system. Care must be taken by vendor and Prestage
Farms to insure that the connections are properly secured before transfer has begun.
Inside the boiler room a small Containment wall is used to contain any spill from any
leaks or equipment failure. Absorbent material is available at site for clean up and
disposed of properly. A Contractor is called if further clean up is warranted above what
we can handle here as indicated in our stormwater permit.
We do not feel that reduced exposure can be achieved at our site due to original
construction of feed mill. All systems are covered except the aboveground storage tanks
and the delivery point. At our site there has been incidence of fat spills mostly due to
accidents from equipment failure inside the boiler room. They were contained on site
and cleaned up.
We feel that there is no indication of presence of an on our site that could
cause a concentration of fecal coliform. Activity and noise around the feed mill could
keep animals/wildlife from entering our site. We also have a good rat and mice
eradication program at the feed mill. Birds do come on site to eat corn and other grains
that may have spilled during the unloading process.
To further reduce the potential for a fat spill, we plan to install a fat/grease separator
system. This system will separate fat from blow down water in case there is some
fugitive material coming from our boiler room. Plans are to complete this system
installation by January 30th 2014.

On July 2, 2013, I, Hughie White, from the Fayetteville Regional Office of the Division of Water
Quality, conducted a site inspection for the 421 Feed Mill facility located on US 421, Clinton, Sampson
County, North Carolina. A copy of the inspection report is attached for your review. I would also like
to thank you for your time and assistance during this inspection. Stormwater from the facility drains to
an unnamed tributary to Crane Creek, a Class C, Sw water located in the Cape Fear River Basin. The
site visit and file review revealed that the subject facility is covered by NPDES Stormwater General
Permit-NCG060336.
Accordingly, the following observations were noted during the Division of Water Quality
inspection (please see the attached addendum for information about your permit):

The Stormwater Pollution Prevention Plan has been developed and implemented and the plan appears to be complete
and includes all of the necessary components. Qualitative monitoring is being performed at all outfalls, as required. The
analytical monitoring is being performed at the required frequency. As a reminder for analytical monitoring from vehicle
maintenance areas, semi-annual documentation of new motor oil usage (gallons/month) needs to be recorded. A review
of the latest analytical results showed benchmark exceedances for Total Suspended. Solids (TSS) and Chemical Oxygen
Demand (COD) at several of the outfalls. As a reminder, these benchmark exceedances requires the Permittee to take
additional actions. The required actions are outlined in the stormwater permit in a tiered program. The first valid sampling
results that are above a benchmark value or out of a benchmark range require the permittee to take the actions outlined in
Tier One. Be sure to review the descriptions of the Tier One, Tier Two, and Tier Three response actions outlined in the
permit and take the necessary actions that are required.

In accordance with your application for a discharge permit received on May 10, 2012 we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject
state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal.or local governmental permit that may be required.
Please note that the attached permit expires on October 31, 2012. Your COC will be
automatically renewed .to cover the full 5 yeat term of the soon to be reissued permit. In the
interim you must abide by the terms of the expiring permit. You can review the proposed

Thank you for your time and assistance during my site visit at the feed mill in Clinton on April 10, 2012. As
discussed, coverage under a General Stormwater Permit is required for certain industrial facilities, including
feed mills, which have point source discharges of stormwater. During the site visit, observations were made of
the facility in Clinton, which has several outfalls conveying stormwater runoff from the property. It was relayed
by you that a similar facility with stormwater outfalls is located in the nearby community of Moultonville. The
General Stormwater Permit that covers these types of facilities is the NCG060000. A copy of the Notice of
Intent application form to obtain coverage under the NCG060000 permit was left with you to be completed and
submitted to the Stormwater Permitting Unit at the address on the form. You are asked to submit a completed
form for each facility within 30 calendar days from receipt of this letter.
