Dear Mr. Scott:
On March 17, 2014, I, Ashley Rodgers, of the Raleigh Regional Office, carried out a stormwater
compliance evaluation inspection (CEI) for the above permit. Your availability was appreciated as it .
facilitated the inspection. A synopsis of the inspection is discussed below:
The Stormwater Pollution Prevention Plan (SPPP) was available during the inspection. The
SPPP was reviewed and updated annually during this permit cycle. All components of the plan
were evaluated during the CEI, and the plan was found to be generally complete and being put
into active use. A few suggestions were offered onsite in order to provide better/more
consolidated documentation.
2. The Stormwater Discharge Outfalls (SDO) were inspected during the inspection. The SDO
appear to be adequately capturing all of the stormwater coming from the production area.
3. Qualitative monitoring results were reviewed during the CEI. Analytical monitoring results
were available and up to date, and within benchmarks.

WET CHEMISTRY DATA REPORTING QUALIFIERS
On the appropriate reporting form, under the column labeled "Q" for qualifier, each result is flagged with
the specific data reporting qualifiers listed below, as appropriate. The qualifiers used are:
U : This flag indicates the compound was analyzed for, not detected and is reported as less than the
Method Detection Limit (MDL) (or as defined by the client). The Reporting Limit (RL), or Limit
of Quantitation (LOQ), and the MDL will be adjusted to reflect any dilution or concentration of
the sample and, for soils, the percent moisture.
This flag indicates the reported result is an estimated value. The flag is used when an analyte is
detected and the result is less than the adjusted RL/LOQ but equal to or greater than the MDL.
Q : This flag denotes that one or more quality control criteria have failed (e.g., LCS recovery,
Continuing Calibration Verification, or CCV) and reanalyses can't be performed. The Q flag is
applied to all specific analyte(s) in all samples associated with the failed quality control criteria.
B : This flag is used when the analyte is found in the associated method or calibration blank as well
as in the sample. It indicates probable blank contamination and warns the data user to take
appropriate action. The combination of flags BU or UB is not an allowable policy. Blank
contaminants are flagged B only when they are detected in the sample.
D: This flag is applied to an analyte when the reported result is based on a dilution.
X/Y/Z : Other specific flags may be required to properly define the results. If used, the flags will be fully
described in the SDG Narrative. The laboratory -defined flags are limited to X, Y, and Z.
The extensions: D, S, and SD are added to the end of the Client ID and represent the

In response to your renewal application for continued coverage under stormwater General Permit NCG030000
the Division of Water Quality (DWQ) is forwarding herewith the reissued General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended).
The following information is included with your permit package:

• A copy of the Technical Bulletin for the General Permit
• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself
with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements.
The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable
storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances.
The more significant changes in the General Permit since your last COC was issued are noted either in the
Draft Permit Fact Sheet that accompanied the public notice (http://portal.ncdenr.6rg/web/wq/ws/su/`current-
notices), or in the Response to Comments / Summary of Changes and Technical Bulletin documents that are
posted on the Stormwater Permitting Unit's website with the new General Permit. Please visit

On March 17, 2010, Vicki Webb of the Raleigh Regional Office conducted a stormwater compliance
evaluation inspection of the subject facility.
The following observations were made:
1. The Basic Machinery Co. Incorporated is located at 1220 Harold Andrews Rd., Siler City, NC.
The facility discharges into the receiving waters designated as an unnamed tributary to Rocky
River in the Cape Fear River Basin.
2. The facility had the all of the components needed for the Stormwater Pollution Prevention Plan
(SP3). The SP3 included: a detailed site map with labeled stormwater outfalls, alist of potential
contaminants, a spill and response plan, a narrative describing BMPs, Preventative Maintenance
and Good Housekeeping program, employee training and a contact list of whom to contact in an
emergency.
3. The SP3 is to be reviewed and updated annually. Please refer to Part II, Section A, 7 of your
permit. I
4. Continue to do the analytical and qualitative monitoring as required by the permit.

I have reviewed the Stormwater Pollution Prevention Plan that you submitted. The plan was received March
29, 2007. The plan covers most of the requirements of the General Permit No. 030000. After reviewing the
plan I noted the following:
1. A responsible party list needs to be included with phone numbers in the plan. This information should
also be posted for employees at the facility.
2. Make sure employees are trained at a minimum annually and keep this documentation on file for review.
The facility personnel (or team) responsible for implementing the training must be identified in the plan.
3. Make sure the site map drawn to scale with distance legend indicating locations of industrial activity
include drainage structures, etc. as specified in the general permit in Section A Part 1(c).
4. Their needs to be a certification statement signed as required in the general permit in Section A Part
1(e). This should be included in the plan.
5. Continue to do the analytical and qualitative monitoring as required by the permit

Basic Machinery Co., Inc. is a manufacturing facility specializing in custom machinery
for the brick industry. We purchase raw steel and components, machine and assemble them, and
ship the product by truck to the customer site where we install and start up the machines. The
main concern for contamination would stem from the waste oil and waste paint streams. These
are both shipped off site and recycled, and records are kept on file in the Plant Manager's office.

Basic Machinery Co., Inc. has studied the most reliable way to store and„u-

The paint is stored in a separate small building away from the main structure where there
is minimal activity, so the chance for an accidental spill is reduced. The waste streams for both
products are kept in a separate building that has secondary containment. Both buildings are
inspected monthly for spills or leaks, and the record of those inspections are kept on file by the
Safety Committee Chairman.

Spill prevention is accomplished by having only certain trained personnel handling the
waste paint and oil streams as this is where the largest possibility of a spill. Response to a spill
would be the responsibly of the floor supervisors who would gather whatever help necessary to
contain a spill and then contact the appropriate response agency.
Section 4, a
Analytical monitoring shall be done annually in April and shall be the responsibility of
the Plant Manager to contract this work to a lab.
Section 5, a
Qualitative monitoring shall be the responsibly of the Safety Committee and will be
conducted monthly. Records of this inspection shall be kept by the Safety Committee Chairman.

On January 24, 2007, Ms. Stephanie Brixey of the Raleigh Regional Office conducted a stormwater evaluation
inspection of Basic Machinery Company, Inc. with the assistance of Mr. Billy Scott, general manager of the
facility. All help was appreciated as it eased the inspection process.
The following observations were made:
There is one outfall that discharges stormwater for the Basic Machinery facility and for the Basic Metal
facility located next door. This outfall discharges to a forested area with minimal potential of reaching
surface water.
2. The majority of the paperwork required by the permit and Stormwater Pollution Prevention Plan (SPPP)
was not present during the inspection. The missing items included: a copy of the Certificate of
Coverage, stormwater management plan and the spill prevention and pollution plan. A site plan/map of
the outfall was submitted to the State, but copies were not kept on -site. All facets of the permit/SPPP
were thoroughly discussed during the inspection.
3. CompuChem, a Division of Liberty Analytical, conducted analytical monitoring in 2006.
4. No qualitative monitoring was conducted as required by the permit. Qualitative monitoring is required
to be conducted twice per year (once in the spring and fall). All data are to be documented on Division
forms (available on-line) and submitted to Central Files.
Within 30 days of your receipt of this letter, please provide a copy of an updated Stormwater Pollution
Prevention Plan (SPPP). The components of the SPPP are detailed in the NCG030000 general permit which

On October 24, 2005 Mr. Chad Coburn and Mr. Christopher Wu of the Raleigh Regional Office
conducted a stormwater evaluation inspection of Basic Metal Products, Inc. and Basic Machinery
Products, Inc. with your assistance. On November 3, 2005 Mr. Wu conducted another inspection
with the assistance of Mr. Billy Scott, general manger of the facilities. All help was appreciated
as it eased the inspection processes.
The following observations were made:
1. Basic Metal Products, Inc. and Basic Machinery Products, Inc. were issued permits
NCG030525 and NCG030526, respectively, because they fabricate structural steel and
also manufacture machinery for the brick industry. Neither facility uses more than 55
gallons of new oil per month (and, therefore, are exempt from on -site vehicle
maintenance monitoring requirements), but their list of potential pollutants of concern
include: water and oil based paints, paint thinner, waste coolant, and some motor oil.
2. While Basic Metal Products, Inc. and Basic Machinery Products, Inc. have the same
owner and share the same lot of land, they were each issued separate stormwater permits
in 2004 due to different outfalls. Since permit issuance, however, both facilities have re-
routed their stormwater to uniformly discharge from 1 new outfall. This outfall will
discharge to a forested area with no potential of reaching surface water. During the
inspection, Mr. Scott was told to inform the State Stormwater Permitting Group of the
new outfall location and to perhaps eliminate one of the permits.
3. None of the paperwork required by the permit and stormwater pollution prevention plan
(SPPP) was present during the inspection. The missing items included: a copy of both

The current conditions of both facilities are adequately prepared to handle large storm events
with little threat of stormwater contamination. Riprap has been placed along the path of
discharge, waste containers and potential pollutants of concern are stored indoors without
potential of reaching storm drains, and employee training is conducted annually to prepare staff
for emergency spills. This NOV is written to help document and establish the infrastructure
necessary for maintaining an efficient stormwater program.

Failure to monitor semi-annually per permit terms may result in the Division requiring monthly
monitoring for all parameters for a specified time period. "No discharge" from an outfall or
inability to collect a sample because of adverse weather conditions during a monitoring period, for
example.does not constitute failure to monitor, as long as it is properly reported.
The permittee shall compare monitoring results to the benchmark values in Table 3. The benchmark
values in Table 3 are not permit limits but should be used as guidelines for the implementation of the
permittee's Stormwater Pollution Prevention Plan (SPPP). Exceedances of benchmark values require
the permittee to increase monitoring, increase management actions, increase record keeping, and/or
install stormwater Best Management Practices (BMPs) in a tiered program. See below the descriptions
of Tier One, Tier Two, and Tier Three response actions. In the event that DWQ releases the permittee
from continued monthly monitoring under Tier Three, DWQ's release letter remains in effect through
the subsequent reissuance of this permit, unless the release letter provides for other conditions or
duration.

SECTION B: ANALYTICAL MONITORING REQUIREMENTS
Analytical monitoring of stormwater discharges shall be performed as specified in Table 1. All
analytical monitoring shall be performed during a measureable storm event at each stormwater
discharge outfall (SDO). Only SDOs discharging stormwater associated with industrial activity must
be sampled (See Definitions).
A measurable storm event.is a storm event that results in an actual discharge from the
permitted site outfall. The previous measurable storm event must have been at least 72 hours
prior. The 72-hour storm interval does not apply if the permittee is able to document that a shorter
interval is representative for local storm events during the sampling period, and the permittee
obtains approval from the local DWQ Regional Office. See Definitions.

time, a local rain gauge reading may be substituted for an on -site reading.
The permittee shall complete the analytical samplings in accordance with the schedule specified
below in Table 2, unless adverse weather conditions prevent sample collection (see Adverse
Weather in Definitions). A minimum of 60 days must separate Period 1 and Period 2 sample
dates, unless monthly monitoring has been instituted as part of other requirements of this permit.
Inability to sample because of adverse weather conditions must be documented in the SPPP and
recorded on the DMR. The permittee must report the results from each sample taken within the
monitoring period (see Part II1, Section E).
