Dear Mr. Smith:
In order to comply with the requirements of General Permit NCG030000, Hirschfeld Industries
Bridge, LLC- Nashville is hereby providing notification that there have been four benchmark
exceedances for zinc.
As specified in General Permit NCG030000: "During the term of this permit, if the first valid
sampling results required for the permit monitoring periods exceed the benchmark value, or are
outside the benchmark range, for any specific parameter at any specific outfall on four occasions,
the permittee shall notify the DWQ Regional Office Supervisor in writing within 30 days of
receipt of the fourth analytical results."
This letter serves the purpose of this required notification. Hirschfeld Industries Bridge, LLC-
Nashville will continue monthly monitoring, as required by General Permit NCG030000, unless
advised otherwise.

On 27 February 2007, I, Ronald Boone, of the Division of Water Quality's (DWQ) Raleigh Regional Office (RRO),
conducted a stormwater Compliance Evaluation Inspection (CEI) of your Nashville plant. Your assistance and
cooperation was helpful and appreciated. Inspection reports are attached for your records. Inspection findings are listed
below. Please use the permit references listed throughout this report for further details as needed.
SITE DESCRIPTION
Stormwater run-off generated at the site is discharged via two stormwater discharge outfalls (SDO) both of which are
located on the eastern side of the site. According to the current certificate of coverage (COC), these SDOs discharge the
stormwater to receiving waters known as Bear Branch. This is incorrect. Bear Branch is actually located about 5 miles
to the west of your site, near Spring Hope. This should be corrected in your stormwater pollution prevention plan and
all related documentation as well as in your COC. Please ensure you make this critical change in the notice of intent
that you submit during the next renewal cycle for this general permit. The actual receiving stream is an unnamed
tributary (UT) to Little Sapony Creek, this segment of which is classified a class C, nutrient sensitive water (NSW) in
the Tar Pamlico river basin.
INSPECTION SUMMARY
1. Outfall 001 was inspected and no problems were observed. The right-of-way (ROW) to outfall 002 was blocked by
overgrowth and this outfall was not checked. The ROW to both outfalls must be maintained so that they can be
easily accessed at all times.
2. The stormwater pollution prevention plan (SP3) was . found to meet all requirements and has been properly
implemented with the following two exceptions:
a. The site map needs to be updated to show the percent of impervious surface for each of the drainage areas.
b. The general location map needs to be updated to show the following:
1. The correct general area.
2. The correct receiving stream, labeled.
3. The SDOs with accurate latitudes and longitudes for each SDO.
4. Lines of discharge from the SDOs to the receiving stream.

3. Analytical and qualitative monitoring is being accomplished. However, please take special note of the time frames
when the qualitative monitoring must be performed, i.e. spring — April through June (2nd quarter) and Fall —
September through November. Also, please note that there are exceptions to conducting analytical testing every
year. Please review that part of your permit in detail to determine when you must perform the testing and when
there may be exceptions to those requirements. DWQ staff can answer any questions you may have.
No other discrepancies were found during this inspection. No response from you is necessary. If you or your staff has

Comment: Site map and general location map do not meet all requirements. Percent
impervious surface needs to be added to site map and general location map shows
wrong receiving stream and does not show the outfalls with lats/longs and lines to the
receiving stream. Receiving in plan and on current COC is Bear Branch, which is about
5 miles away from plant. General location map was not even the correct general area.
The actual receiving stream would be a UT to Little Sapony Creek.

Mr. Chad Coburn of the Division of Water Quality, Raleigh Regional Office, conducted an insspti n of
Carolina Steel Corporation (NPDES Permit No. NCG030326) on November 21, 2005. Your assistance during
the inspection was greatly appreciated. Findings during the inspection are as follows:
1) The current permit was issued August 31, 2002, and expires on August 30, 2007.
2) The stormwater drainage system and outfalls covered under the Permit were observed. The flow
observed was clear with no odor or evidence of solid material. There was no visible evidence of
spills, overflows or similar incidents within the system or at either outfall.
3) The permit and proper up-to-date monitoring documentation was available on site during the
inspection. The Stormwater Pollution Prevention Plan (SPPP), Stormwater Management Plan
(SMP) and a Spill Prevention and Response Plan (SPRP), were reviewed and found to be in
compliance with the general permit. Leonhardt Environmental P.C. was contracted to do the
stormwater and spill prevention training.
4) It was noted during the inspection that the site map included with the Certificate of Coverage was
incorrect. A corrected map is attached to this report.

Your facility is currently covered for stormwater discharge under General Permit NCG030000. This permit expires
on August 31, 2002. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by late summer of 2002. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will be expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 1 I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

Permits Unit Web Site at http://h2o.enr.state.nc.us/su/stormwater.htmI
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.
