Comment: The Stormwater Pollution Prevention Plan (SPPP) records reviewed during
the inspection were organized and well maintained. The SPPP was last updated in
June 2012 and employee training was last performed in April 2011. The facility staff
must ensure the following is documented: a notation that no spills occurred for each
applicable year, annual SPPP review/update, twice per year facility stormwater
inspections, any implemented BMPs, and feasible atiernatives to current practices.

This letter is in response the the Compliance Evaluation Inspection conducted by
Mr. John Lesley on March 5, 2010 at the Cherryville Wastewater Plant.
Mr. Lesley came out on March 2, 2010 to collect toxicity did a complete walk
through of the plant. John ask to see our Storm water plan. I told him that our
engineer, Recil Wright was still working on it. I called Recil our City Engineer and
he came down and talked to Mr. Lesley in my office. Mr. Lesley stated that he
would be back on Friday March 5, 2010 and that if we had at least a draft in
place that we should be ok.
Recil finished the Draft Stormwater Plan and delivered it to me on March 3, 2010.
Mr. Lesley came back out on March 5, 2010 and looked at it. He suggested a
few minor changes. We made those changes noted below and now have Our
Stormwater Pollution Prevention Plan in place as of March 18, 2010. Since this
time Ellen Huffman came out on March 31, 2010 to do our Annual Sludge
inspection. I showed Ellen our Stormwater Plan and that it was in place.
Please see the response to each finding on page 2 of this letter.

In the Inspection Summary, under Stormwater Pollution Plan all of the questions
that were answered no have been corrected. We do have a Stormwater
Pollution Prevention Plan in place as of March 18, 2010. We made all the
changes that Mr. Lesley requested in our draft plan.
Those modifications included; Name of waters that receive stormwater from your
facility, write a description of how you plan to evaluate the storm drains, come up
with a good housekeeping plan, move the oil storage tank from outside to inside
a building, include Maintenance as in mowing grass in the plan, come up with a
spill prevention plan, employee training including a log sheet in the plan and
identify all storm water ditches and drains in site plan photo.
All of these issues have been corrected and addressed. The plan was
implemented on March 18, 2010.
1 have also included as page 3 documentation where we did our smoke testing
back in 2009. We identified all of our storm water outfalls and drains in 2009.

Here is the link for the stormwater Notice of Intent (NOI) which is the form you need to fill out to get
your permit. Beside the NOI form is the permit text that you will have to follow to comply with your
permit. The technical bulletin has a lot of helpful info that is easier to read than the actual permit text.
You need the permit'NCGI 10000' for treatment works. If you have any questions... just call or email!

We had all of our storm drains at the plant tested with the smoke test and none were tied into our
sewer system.
All of the storm drains were working properly.
They were tested by Kevin Abernathy that is over our Water & Sewer lines and his crew on 9/16/o8. .
I am currently working with Recil Wright our City Engineer on getting our Stormwater Permit ready
to submit and also getting a plan together.

In accordance with your application for a discharge permit received on December 3, 2008, we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement.between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the
Division of Land Resources, Coastal Area Management Act or any other federal or local governmental
permit that may be required.

This Certificate of Coverage shall become effective January 9, 2009.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
