This COC is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection
Agency, dated October 15, 2007 (or as subsequently amended.)
The General Permit authorizes discharges of stormwater and specifies your obligations for discharge controls,
management, monitoring, reporting, and record keeping. Please review the new permit to familiarize yourself
with all of the changes. Parts III and IV contain the Standard Conditions, including Compliance and Liability,
Reporting, Monitoring and Records requirements; Operation and Maintenance obligations; and Definitions.
Your facility has six months from receipt of the COC to update its Stormwater Pollution Prevention Plan
(SPPP) to comply with changes in SPPP requirements. Other changes are effective immediately. Please note
that Tier 3 Actions (if applicable) are triggered by four benchmark exceedances beginning on the effective
date of the renewal permit and do not count prior exceedances. Please visit our website above to review the
new General Permit carefully.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it
relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law,
rule, standard, ordinance, order, judgment, or decree.

Comment: The Stormwater Pollution Prevention Plan (SPPP) records reviewed during
the inspection were organized and well maintained. Employee training was last
performed on 9/14/2011 and the SPPP was last updated on 4/30/2012. The SPPP
coordinator must ensure the following is documented: a history of significant spills for
the previous three years or a notation that no spills occurred (if applicable), annual
re-evaluations of all stormwater outfails/drainage systems for the presence of
non-stormwater discharges, and the observations noted during the semi-annual (at a
minimum) facility stormwater inspections performed in conjuction with the
visual/qualitative monitoring events. Note: The facility stormwater inspections and
qualitative monitoring events are two separate conditions/requirements of the
Stormwater Permit/SPPP.

Comment: The documentation reviewed in the facility's Stormwater Pollution
Prevention Plan (SPPP) was organized and well maintained. Employee training was
last performed on 6/11/10 and the SPPP was last updated on 6/21/10. The facility
stormwater inspections (twice per year) and the recertitications for non-stormwater
discharges (at each outfall) were not documented from January 2009 through June
2010. Note: The on -site staff indicated that any problems associated with the
stormwater runoff system would be observed (and corrected) during the routine
walk-throughs (by 4RC/operations staff) at the VVWTP. 'The facility staff must also
ensure to maintain an updated list of significant spills or leakages of pollutants for the
previous three years, or the notation that no spills have occurred during each applicable
year. Note: No spill history was documented in the SPPP since 2008.

Comment: The facility has designated three stormwater outfalls (SD01, SD03, SD05).
No discharges were observed at these outfalts during the inspection. A shelter had
been constructed over the dumpster for the bar screenings. Overall, the site grounds
were well maintained: however, petroleum stains were observed on the asphalt
driveway adjacent to the solids unloading area located beside the lab building. The
SPPP coordinator must ensure that staff are appropriately trained and equipped to
address any petroleum spills/leakages that may occur at or around this area.

Comment: A copy of the Stormwater Pollution Prevention Plan (SPPP) was reviewed
on -site; however, Hazen & Sawyer are in the process of updating this SPPP. Overall,
the SPPP has been implemented; however, the facility. staff need to address the
potential stormwater runoff at and around the dumpster containing the bar screenings.
Note: The dumpster had leaked onto the concrete pad during the site inspection.

Comment: Outfalls 002 and 003 were not discharging at the time of the inspection;
however, trace deposits of lime were observed in the drainage ditch of Outfall 002. The
facility staff must ensure no chemicals, residuals, etc. are placed in or within close
proximity of any Stormwater drainage systems. The stormwater appeared clear with no
foam from Outfalls 001, 004, and 005.

In accordance with your application for discharge permit received on March 10, 2003, we are forwarding herewith
the subject certificate of coverage to discharge under the subject state - NPDES general permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between
North Carolina and the US Environmental Protection agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to request an individual permit by submitting an individual permit application. Unless such demand is
made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of
coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division
of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

This certificate of coverage shall become effective June 27, 2003
