This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.) The following information is included with your permit package: • A new Certificate of Coverage • A copy of the Stormwater General Permit • A copy of a Technical Bulletin for the General Permit • Copies of the Discharge Monitoring Report (DMR) Form • Copies of the Annual Discharge Monitoring Report Form (if applicable) • Copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all changes in the reissued permit. Significant changes to the General Permit are outlined in the attached Technical Bulletin. Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or decree. A review of the certificate of coverage has indicated that an error was inadvertently made. Accordingly, we are forwarding herewith modifications to the subject certificate of coverage to correct the error. This modification is issued to change the receiving waters to Clark Creek. Please find enclosed an amended certificate of coverage page which should be inserted in your issued document. The old certificate of coverage page should be discarded. All other terms and conditions contained in the certificate of coverage remain unchanged and in full effect. This modification is issued pursuant to the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency.
