Dear Ms. Allocco:
The City of Belmont is in receipt of the above NOD and as required by your letter, our
responses to these deficiencies are as follows:
List of significant spills occurring during the past three years- The City of Belmont Waste
Water plant, permit # NCG110023, had no occurrence of spills during the past three years. We
have updated worksheet #4 and added this worksheet to Appendix C of WWTP SWPPP, to
reflect the non-occurrence of spills during this three year period.
Employee Trainina Documentation- The documentation for SWPPP employee training conducted for
2011 has been added to Appendix C.
SWPP reviewed and updated annually- The City of Belmont has completed a review and updated our
SWPPP. A list of the updates is as follow;
• Appendix B Figure 3 - Stormwater System Map Added and Updated to reflect current status.
• 2.2 Site Map - paragraph 3 Updated Outfall No. 1 information,
• 2.2 Site Map -- paragraph 4 Updated Outfall No. 2 Information.
• 2.3.1 Storage Practices - paragraph 3 Updated 250 gallon diesel fuel tank to say that it is
covered to prevent contact with stormwater.
• 2.3.1 Storage Practices - paragraph 4 Updated to say that Dumpster Lid stays closed unless in
use or being emptied by solid waste disposal contractor.
• 2.3.2 Loading and Unloading Activities - paragraph 2 Updated to indicate drainage of Sludge
Transfer Station catch basin to head-works/influent.
• 2.3.5 Waste Disposal Practices - paragraph 1 Updated to indicate cans and dumpster are hauled
off by a solid waste contractor, not the City of Belmont. Also added that not only the dumpster
lid but the can lids are closed when not in use.

Enclosed please find a copy of the Compliance Evaluation Inspection Report for the inspection
conducted at the subject facility on June 22, 2012, by Ms. Donna Hood of the Office. Please inform the
facility's Operator -in -Responsible Charge of our findings by forwarding a copy of the enclosed report.
Several deficiencies were noted during the inspection as detailed in the Stormwater Pollution
Prevention Plan and Permit & Outfalls Sections of the attached report. It is requested that a written
response be submitted to this Office by August 20, 2012, addressing the deficiencies noted in the
Stormwater Pollution Prevention Plan and Permit & Outfalls Sections of the attached report. In
responding, please address your comments to the attention of Ms. Marcia Allocco.
This report should be self-explanatory; however, should you have any questions concerning the

This letter is in response to your report concerning the Storm water Compliance Inspection
Report for the Town of Cleveland conducted by Mr. John Lesley on March 6, 2012.
The report noted a deficiency for not updating the Storm water Pollution Prevention Plan for
2011, however, the plan was updated on May 17, 2011 and a copy was given to Statesville Analytical,
Inc. A copy of this update was in the plan copy that Statesville Analytical has. A copy had not been
entered into the ORds copy that was available during inspection time. The ORds copy now includes a
copy of the update. A copy of the updated SPPP page is included for your review.
Another deficiency noted concerned employee training. Employee training was not
documented for 2011. Employee training is scheduled now.
The report also noted that the storm water facility inspection was not documented. The
inspection was performed; however it was not noted in the SPPP's log sheet. The log sheet has been
updated to include the inspection date. A copy of the 2011 inspection log sheet is included for your
review.
