NCG100000 received September 3, 2013, the Division of Energy, Mineral and Land Resources
(DEMLR) is forwarding herewith the reissued Certificate of Coverage and stormwater General Permit.
This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection
Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of the stormwater General Permit
• A copy of a Technical Bulletin for the General Permit
• Copies of the Discharge Monitoring Report (DMR) Form
• . Copies of the Annual Discharge Monitoring Report Form (if applicable)
• Copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with
respect to stormwater discharge controls, management, monitoring, and record keeping. Please review
the new permit to familiarize yourself with all changes in the reissued permit. Significant changes to the
General Permit are outlined in the attached Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DEMLR.
This permit does not affect the legal requirements to obtain other permits which may be required by
DENR, nor does it relieve the permittee from responsibility for compliance with any other applicable
federal, state, or local law, rule, standard, ordinance, order, judgment, or decree.

The Division staff is currentty In the process of renewing this permit. When a draft is available for notice and public comment, It will
be posted on our website at http://portal.ncdenr.org/web/wglws/su/public-notices. Please continue to check this website to review
and comment on proposed changes to the permit Once the permit is reissued, your facility would be eligible for continued coverage
under the reissued permit.
To assure consideration for continued coverage under the general permit, you must apply to the Division of Water Quality (DWQ) for
renewal of your permit. Enclosed you will find a Permit Coverage Renewal Application Form. The application must be completed
and returned by May 4. 2012, betters confirming our receipt of the completed application will not be sent
Failure to request renewal within the time period specified may result in a civil assessment of at least $250.00. Larger penalties may
be assessed depending on the delinquency of the request Discharge of stormwater from your facility without coverage under a valid
stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could result In assessments of civil penalties of up to
$25,000 per day. our records Indicate that your facility has outstanding annual fees in the amount of $80. We cannot
renew permit coverage until you have corrected this situation. We have enclosed copies of the unpaid Invoices) that must be
returned with payment. Failure to pay annual fees is a violation of your permit and may result In revocation of your permit coverage.
"No exposure exclusion" Is now available to industrial facilities in any of the categories of "storm water discharges associated with
Industrial activity," (except construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility
industrial materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional
information contact the Central Office Stormwater Staff member listed below or check the Stormwater Permitting Unit Web Site at
http://portal.ncdenr.org/web/wq/ws/su/npdessw#tab-S.
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed Rescission Request
Form. Mailing Instructions can be found on the bottom of the form. You will be notified when the rescission process has been
completed.

our total revenues are generated through the sale of used parts, the balance being

Dear Mr. Farmer:
Enclosed are the monitoring results from the laboratory analyses performed on the
discharge from Stormwater Outfall 001 at the subject facility. The samples analyzed were grab
samples collected during the sampling investigation performed on October 8, 2008, by Ms. Erin
Hall, Mr. Ron Eubanks, and Mr. Jeff -Mitchell of Charlotte -Mecklenburg Storm Water Services.
This sampling investigation was conducted as part of a cooperative working agreement between
Mecklenburg County and the Division of Water Quality. In addition, the sampling investigation
was conducted on behalf of the City of Charlotte in compliance with the City's NPDES Permit,
NCS000240, Part II Section H.
The enclosed monitoring results indicate that the stormwater discharged on October 8,
2008, contained elevated concentrations of lead, total suspended solids, oil and grease, and pH
compared with the benchmark values contained in Stormwater general permit NCG100000. In
addition, elevated concentrations of BOD, zinc, copper, and chromium were also detected.
Please review the requirements of your stormwater permit and take the required steps/actions to
reduce the concentrations of these pollutants leaving your site. Through the development and
implementation of stormwater management programs it is the Division of Water Quality's
expectation that the stormwater discharges at industrial sites covered by NPDES stormwater
permits do not impact receiving waters.
It is this Ofce's understanding that Lakeview Auto Parts plans to discontinue
automotive salvage activities at the facility and therefore will not be required to have stormwater
permit coverage through the Division of Water Quality. Please be advised that your facility will
still need to meet the requirements of the City of Charlotte Stormwater Ordinance since
stormwater from the facility discharges to the City of Charlotte Municipal Separate Stormwater
System {MS4). Therefore, you should continue measures to reduce the concentrations of the
pollutants noted above in your discharge.'

The more significant changes in the General Permit since your last Certificate of. Coverage include the following:
• Part I Section A — A new notation that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part I Section A — A new provision that facilities draining to 303(d) listed waters, or in watersheds with an approved TMDL,
may not be eligible for renewed coverage under the General Permit.
• Part II Section B Table 1 — An increased sampling frequency from once per year to twice per year.
• Part Il Section B Table 3 — Benchmark concentrations now replace cutoff concentrations; the elimination of the option to
avoid sampling based on cutoff concentrations.
• Part 11 Section B — New provisions requiring the permittee to execute Tier One and Tier Two response actions, based on
the first benchmark exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two
requires that the permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive
monitoring events show no benchmark exceedences.
• Part II Section B —A new provision that four exceedences of any particular benchmark will trigger increased DWQ
involvement in the permittee's stormwater management and control actions. DWQ may direct the permittee to apply for
an individual permit, or may direct the implementation or installation of specific stormwater control measures.

Enclosed are the monitoring results from the laboratory analyses performed on the discharge from
Stormwater Outfall 001 at the subject facility. The samples analyzed were grab samples collected during
the sampling investigation performed on July 8, 2008, by Ms. Erin Hall and Ms. Aliyah Turner of
Charlotte -Mecklenburg Storm Water Services. This sampling investigation was conducted as part of a
cooperative working agreement between Mecklenburg County and the Division of Water Quality. In
addition, the sampling investigation was conducted on behalf of the City of Charlotte in compliance with
the City's NPDES Permit, NCS000240, Part II Section H.
The enclosed monitoring results indicate that the stormwater discharged on July 8, 2008, contained
elevated concentrations of lead and total suspended solids compared with the benchmark values contained
in Stormwater general permit NCG100000. In addition, elevated concentrations of fecal colifornl bacteria
and zinc were also detected. Please review the requirements of your stormwater permit and take the
required steps/actions to reduce the concentrations of these pollutants leaving your site. Through the
development and implementation of stormwater management programs it is the Division of Water
Quality's expectation that the storrriwater discharges at industrial sites covered by NPDES stormwater
permits do not impact receiving waters.
The enclosed results should be self-explanatory; however should you have any questions
concerning the sampling results or have any questions regarding your permit, please do not hesitate to

Your facility is currently covered for stormwater discharge under General Permit NCG 100000. This permit expires
on August 31, 2002. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by late summer of 2002, Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit -
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will be expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. The application must he completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties maybe assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10.000 per day -
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the lacilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed

forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit- This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and
the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal

Your facility is currently covered for stormwater discharge under General Permit NCG100000. This permit expires
on August 31, 2002. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by late summer of 2002. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will be expiring. Enclosed you will find a General Permit Coverage Renewal
Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility
without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facilty industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the
inspection conducted at the subject facility on March 19, 2008, by Ms. Erin Hall and Mr. Ron
Eubanks of Charlotte -Mecklenburg Storm Water Services. This inspection was conducted as
part of a cooperative working agreement between Mecklenburg County and the Division of
Water Quality. In addition, the inspection was conducted on behalf of the City of Charlotte in
compliance with the City's NPDES Permit, NCS000240, Part II Section H. Thank you for your
assistance and cooperation during the inspection.
This report is being issued as a Notice of Violation (NOV) due to the permittee's failure
to comply with any of the requirements stated in the facility's general stormwater permit. This is
in violation of the subject Stormwater Permit and North Carolina General Statute (G.S.) 143-
215.1, as detailed in the Stormwater Pollution Prevention Plan, Qualitative Monitoring,
Analytical Monitoring, and Permit and Outfalls sections of the enclosed report. Pursuant to
G.S. 143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may
be assessed against any person who violates or fails to act in accordance with the terms,
conditions, or requirements of any permit issued pursuant to G.S. 143-215.1.

Inspection Summary:
Facility Description:
Lakeview Auto Parts is located at 5500 Wilkinson Blvd., Charlotte, NC. Lakeview Auto Parts is an automotive salvage
facility that buys new and used automobiles from the public !t operates from 8:00 a.m. to 5:30 p.m., Monday through
Friday and is closed Saturday and Sunday. The facility buys approximately 900 salvaged automobiles per year and
specialize in Volvo and Volkswagen, but will accept other types.
Lakeview Auto Parts is located on approximately 49,400 sq ft of property, which includes an office building. a shop
building, approximately six separate covered areas, and a gravel lot at the rear of the property where salvaged
automobiles are stored. The office building is located at the front of the property along Wilkinson Blvd. To the west of
the office building is a paved parking area, The property is fenced on all sides beyond the office building and parking
area. A paved driveway extends north from the parking area to the shop building. To the east of the office building is
a gravel driveway that extends north to the rear of the property The rear of the property is a gravel lot. The shop
building. covered areas, and salvaged automobiles are located in this area.
Compliance History:
The permittee/facility does not have any prior compliance issues with the NC Division of Water
Quality or the Charlotte -Mecklenburg Storm Water Services.
Lakeview Auto Parts was permitted under a general stormwater permit from 1997 to 2002. The facility's permit has
been expired since 2002. Ms. Keel with Lakeview Auto Parts was instructed to contact Ms. Sarah Young with the
NCDWQ Stormwater permitting group to obtain coverage under the general permit.
Industrial ODerationslCity of Charlotte Stormwater Ordinance Review:
Salvaged Automobiles
Salvaged automobiles are bought from the public and upon arrival at the facility gasoline, oil, and antifreeze are
drained to the facility's on site ASTs. After this process, specific parts (See Salvaged Automobile Parts below) as well
as engines (See Engines below) are dismantled from the automobile. Volvos and Volkswagens are stored on one side

facility for a longer length of time to allow customers more automobile part options. All other automobile types are
stored on the other side of the gravel lot and wait to go to the car crusher. Facility personnel hauled off approximately
three automobiles per day to Southern Metals for crushing. Moderate staining was observed in the gravel lot.
Salvacied Automobile Parts
Dismantled catalytic converters, alternators, starters, and batteries are boxed in industrial -grade boxes and shipped to
different retailers. Boxes containing parts are stored under cover and kept until they are shipped. No staining was
observed in this area. Batteries that are removed from vehicles are tested for a charge. If a battery contains enough
charge, then it will stay at the facility for re -sale. Re -sale batteries were elevated off the ground and stored under
cover. After gas tanks have been drained they are removed and stored under cover before they are taken to Southern
Metals for disposal.
Engines
Engines are dismantled from automobiles as they arrive at the facility. They are split and transmissions are removed.
Several engines were observed being stored elevated off the ground and under cover in the same location as the
removed gas tanks. Per facility personnel, this area is set up to store all engines until they are taken to Southern
Metals for disposal. However, approximately 10 engines were observed being stored on a concrete pad exposed to
stormwater. Only a few of the engines were covered with a tarp. Per facility personnel, some engines stay onsite for
longer periods for customers to buy parts. After several months to no longer than a year, these engines are also taken
to Southern Metals for disposal. A significant amount of staining and oil residue was observed on the concrete pad in
this area. Used oil -dry was observed on the concrete pad as well.

Engines
The facility should store engines elevated off the ground and under cover to minimize exposure to stormwater. Used
oil -dry should be property disposed of as soon as the discharged material has soaked in completely.
Material Storage
Different types of automotive fluids (<1 quart in volume) were observed being stored outside on a shelf exposed to
stormwater. Approximately ten, empty 55-gallon drums were located throughout the facility grounds which were not
labeled and were exposed to stormwater.
Automotive fluids should be stored under cover. It is recommended that the facility properly dispose of empty 55-
gallon drums.
Trash Dumoster
The facility's trash dumpster is owned by Allied Waste Management. It appeared to be in good condition with rain lids
and a rain plug in place.
Aboveground Storage Tanks (ASTs)
The facility has two ASTs and two 55-gallon drums, which are used for storage of fluids drained from vehicles. The
tanks include: one 275-gallon used oil tank, one 275-gallon gasoline tank, and two 55-gallon drums for antifreeze.
M&M Waste Oil picks up the used oil, gasoline, and antifreeze.
Both ASTs appeared to be in poor condition with a significant amount of rust. The ASTs and antifreeze drums are
located within a concrete secondary containment that is under cover. A hole was observed in the secondary
containment on the north side at the bottom.
It is recommended the facility address the condition of the ASTs such as removing rust and re -painting. The facility
must plug the hole in the secondary containment and inspect stormwater prior to discharge as required by the permit.
Permit and Outfalls (cdnt.)
The stormwater runoff on site is predominately in the form of sheet flow. A topographic low point on the property
enabled stormwater to puddle up in this area.
However, facility personnel created an outfall area at this point allowing stormwater to runoff into a drainage ditch
located to the east of the facility.
The outfall area is constructed of broken bricks and cinder blocks around the outside edge while the center area is fine
sediment with no filter structure. Two check dams have been constructed and grass seed has been planted in the
drainage ditch. Stormwater from the drainage ditch eventually flows into a catch basin located on Wilkinson Blvd_
Analytical and qualitative monitoring should be conducted at the created outfail area. It is recommended the facility
place some type of filter structure, such as gravel or grass, in the center of the outfall area.

Enclosed are the monitoring results from the laboratory analyses performed on the discharge from
Stormwater Outfall 001 at the subject facility. The samples analyzed were grab samples collected during
the sampling investigation performed on July 8, 2008, by Ms. Erin Hall and Ms. Aliyah Turner of
Charlotte -Mecklenburg Storm Water Services. This sampling investigation was conducted as part of a
cooperative working agreement between Mecklenburg County and the Division of Water Quality. In
addition, the sampling investigation was conducted on behalf of the City of Charlotte in compliance with
the City's NPDES Permit, NCS000240, Part II Section H.
The enclosed monitoring results indicate that the stormwater discharged on July 8, 2008, contained
elevated concentrations of lead and total suspended solids compared with the benchmark values contained
in Stormwater general permit NCGi 00000. In addition, elevated concentrations of fecal coliform bacteria
and zinc were also detected. Please review the requirements of your stormwater permit and take the
required steps/actions to reduce the concentrations of these pollutants leaving your site. Through the
development and implementation of stormwater management programs it is the Division of Water
Quality's expectation that the stormwater discharges at industrial sites covered by NPDES stormwater :
permits do not impact receiving waters.

In response to your renewal application for continued coverage under stormwater General Permit NCG100000, the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG100000
• A copy of a Techni cat Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since your last Certificate of Coverage include the following:
• Part I Section A — A new notation that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part I Section A — A new provision that facilities draining to 303(d) listed waters, or in watersheds with an approved TMDL,
may not be eligible for renewed coverage under the General Permit.
• Part 11 Section B Table 1 — An increased sampling frequency from once per year to twice per year.
• Part II Section B Table 3 — Benchmark concentrations now replace cutoff concentrations; the elimination of the option to
avoid sampling based on cutoff concentrations.
• Part II Section B — New provisions requiring the permittee to execute Tier One and Tier Two response actions, based on
the first benchmark exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two
requires that the permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive
monitoring events show no benchmark exceedences,
• Part 11 Section B — A new provision that four exceedences of any particular benchmark will trigger increased DWQ
involvement in the permittee's stormwater management and control actions. DWQ may direct the permittee to apply for
an individual permit, or may direct the implementation or installation of specific stormwater control measures.

On April 13, 2010, Erin Hall from Charlotte -Mecklenburg Stormwater Services (CMSWS) conducted a site
inspection of the Lakeview Auto Parts facility located at 5500 Wilkinson Blvd. in Charlotte, Mecklenburg
County, North Carolina. The site drains to Taggart Creek, Class C waters located in the Catawba River Basin.
The site visit and file review revealed that the subject facility is covered by NPDES General Stormwater Permit
— NCG100000.
This inspection was conducted as part of a cooperative working agreement between Mecklenburg County and
the Division of Water Quality. In addition, the inspection was conducted on behalf of the City of Charlotte in
compliance with the City's NPDES Permit, NCS000240, Part II Section H. The following observations and/or
permit condition violations were noted during the CMSWS inspection (please see the attached addendum for
information about your permit):

ADDENDUM
The National Pollutant Discharge Elimination System (NPDES) program was established under the federal
Clean Water Act and then delegated to the Division of Water Quality for implementation in North Carolina. The
NPDES permitting program for stormwater discharges was established in 1990. Phase I focuses on site and
operations planning to reduce pollutant sources.
Through the NPDES permitting program industrial facilities that fall into one of the subject ten categories are
required to obtain permit coverage under a general permit or an individual permit, depending upon the
facilities SIC code and the industrial activity occurring at the facility, There are currently 19 active general
stormwater permits in North Carolina. One condition that is applicable to both the general permits and
individual stormwater permits is the requirement to develop and implement site -specific comprehensive
Stormwater Pollution Prevention plans (SPPP). These plans are required to include a comprehensive
evaluation of the site and operations to reduce pollutant sources and prevent pollutant discharge.

discharge monitoring forms, and a Notice of Intent [which is the application for coverage under a NPDES
General Stormwater Permit] can be found on this website.
The following items are a subset of permit requirements and are listed to assist you and help bring your
attention to certain permitting issues, process and condition requirements of your permit. Please see the
above mentioned website to obtain a copy of the permit so you may fully comply with all the conditions therein.
Failure to property comply with conditions of the permit, like those bulleted below, is a violation(s) of
the permit and subject to civil penalty assessment(s) of up to $25,000.00 per day.
• Industrial facilities that fall into one of the subject ten categories are required to obtain permit coverage
under a general NPDES stormwater permit or an individual NPDES stormwater permit
• All persons desiring to have facilities covered by a General Permit must register with the Division of
Water Quality (DWQ) by filing of a Notice of Intent (NOI) and Applicable Fees.
40 This Notice of Intent (NO]) must be submitted and a Certificate of Coverage issued prior to any
discharge of stormwater associated with the industrial activity, process wastewater, and/or mine
dewatering.
• Once the Notice of Intent is received, DWQ will review and determine whether a Certificate of Coverage
(COC) may be issued. If this Certificate of Coverage is issued the facility is covered by the General
Permit. That is, the permittee is authorized to discharge stormwater, process wastewater, and/or mine
dewatering pursuant to the type of General Permit until the permit expires or is modified or revoked.
• Any other point -source discharge to surface waters of the state is prohibited unless it is an allowable
non-stormwater discharge or is covered by another permit, authorization, or approval.
• There are currently 19 active general permits. Permit conditions vary accordingly; however, most of the
permits have the following condition requirements (you must read your entire permit to ensure full
compliance):

Expiration — The permittee is not authorized to discharge after the expiration date of the general
permit. In order to receive automatic authorization to discharge beyond the expiration ,-late, the
permittee shall submit forms and fees as are required by the agency authorized to issue permits
no later than 180-days prior to the expiration date. Any permittee that has not requested
renewal at least 180 days prier to expiration, or ally permittee tilat does not have a permit after
the expiration and has not requested renewal at least 180 days prior to expiration, will be
subject to enforcement. Failure to renew your permit is a violation.
o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is not
transferable to any person expect after notice to and approval by the Director. The Director may
require modification or revocation and reissuance of the Certificate of Coverage to change the
name and incorporate such other requirements as may be necessary under the Clean Water
Act. The Permit is NOT transferable.
o Name Change, Closure, or Facility Ownership change — Permittee is required to notify the
Division in writing in the event the permitted facility is sold or closed. The Division of Water
Quality views changes of name or ownership as a modification that requires the Director's
approval and reissuance of the permit. Name and ownership changes require you to complete a
Name/Ownership Change Form from the Division and mail it back to the Division 01 vvvater
Quality along with all appropriate information.
o Proper Operation and Maintenance is a requirement of the permit.

Inspection Summary:
Facility Description:
Lakeview Auto Parts is located at 5500 Wilkinson Blvd., Charlotte. NC, Lakeview Auto Parts is an automotive salvage
facility that buys new and used automobiles from the public. It operates from 8:00 a m to 5:30 p.m., Monda through
Friday and is closed Saturday and Sunday. The facility buys approximately 900 salvaged auturnobiles per year and
specialize in Volvo and Volkswagen, but will accept other types.
Lakeview Auto Parts is located on approximately 49,400 sq ft of property, which includes an office building, a shop
building, approximately six separate covered areas, and a gravel lot at the rear of the property where salvaged
automobiles are stored The office building is located at the front of the property along Wilkinson Blvd. To the west of
the office building is a paved parking area. The property is fenced on all sides beyond the office building and parking
area. A paved driveway extends north From the parking area to the shop building. To the east of the office building is
a gravel driveway that extends north to the rear of the property. The rear of the property is a gravel lot. The shop
building, covered areas, and salvaged automobiles are located in this area.
Compliance History:
A stormwater inspection was conducted by Charlotte -Mecklenburg Stormwater Services (CMSWS) on March 19, 2008
A Notice of Violation was issued to the facility by NCDENR on April 10, 2008.
Two stormwater monitoring events were conducted by CMSWS: The first monitoring event was conducted on July 8,
2008. NCDENR issued a report to the facility for this monitoring event on September 17, 2008. The second was
conducted on October 8, 2008 as a follow-up to the first monitoring event. NCDENR issued a report to the facility for
this monitoring event on March 4, 2009,
During the March 19, 2008 inspection, Lakeview Auto Parts had an expired stormwater permit. After the inspection the
permittee submitted required paperwork to DWQ to renew its permit. DWQ issued a Certificate of Coverage to
Lakeview Auto Parts effective December 17, 2008. The facility is currently permitted under general stormwater permit
NCG100000.
The permittee stated its intention of discontinuing automotive salvage activities at this facility. The permittee was
instructed to contact DWQ when the permittee had discontinued all activities required by general stormwater permit
NCG100000. At that time an inspection would be conducted to verify the cessation of automotive salvage activities. If
these activities had ceased, then the permittee could apply to have its permit rescinded. However, the permittee never
ceased these activities. Therefore the inspection conducted by CMSWS on April 13, 2010, was a follow-up to the
previous inspection conducted on March 19, 2008.
Industrial Ooerations/City of Charlotte Stormwater Ordinance Review:
Engines
Engines are dismantled from automobiles as they arrive to the facility. Engines are split and transmissions are
removed. The majority of engines are stored outside on a concrete pad under cover while other engines are stored
elevated off the ground on metal racks under cover. Several engines were observed lying on the ground under cover.
Southern Metals picks up engines for disposal. However, some engines stay onsite for customers to buy parts. After
several months to no longer than a year, Southern Metals will pick these up for disposal. Used oil -dry was observed
on the concrete pads where engines are stored. Per facility personnel, oil -dry is swept up on a daily basis. It is
recommended the facility store engines elevated off the ground to minimize exposure to stormwater.
An inspection conducted on April 20, 2010 confirmed that the engines previously observed lying on the ground had
been relocated to an area where they were being stored off the ground and under cover.

Material Stora-ge
Several empty 55-gallon drums without lids or caps were located throughout the facility grounds. It is recommended
the facility properly dispose of empty 55-gallon drums or store them under cover to prevent rain wafer accumulation
inside the drum.
An inspection on April 20, 2010 confirmed that excess 55-gallon drums had been removed from the facility grounds.
Approximately five 55-gallon drums were left onsite. drums were stored upside-down and under cover.
Batteries
The inspection on April 20, 2010, confirmed that batteries were no longer being stored on the concrete pad. Facility
personnel are now storing batteries on a wooden pallet under cover. Once the wooden pallet is filled with batteries, the
batteries are wrapped in bulk with plastic and await pickup by the retailer. Excess batteries are being stored in a large
metal container with the sealed aid.

Comment: The facility has one outfall on site. Stormwater is in the form of sheet flow
across facility grounds and then converges at a low point on the east side of the facility's
property creating the facility's outfall. The outfall area is constructed of broken bricks
and cinder blocks in the middle and at the pipe opening. Straw bales have been placed
around the broken bricks and cinder blocks to filter possible pollutants from entering
stormwater before it is discharged through the pipe and eventually to a storm drain inlet
located along Wilkinson Blvd. The outfall appeared to be in good condition with no
signs staining, blockage, or erosion.

Enclosed are the monitoring results from the laboratory analyses performed on the
discharge from Stormwater Outfall 001 at the subject facility. The samples analyzed were grab
samples collected during the sampling investigation performed on October 8, 2008, by Ms. Erin
Hall, Mr. Ron Eubanks, and Mr. Jeff Mitchell of Charlotte -Mecklenburg Storm Water Services.
This sampling investigation was conducted as part of a cooperative working agreement between
Mecklenburg County and the Division of Water Quality. In addition, the sampling investigation
was conducted on behalf of the City of Charlotte in compliance with the City's NPDES Permit,
NCS000240, Part II Section H.
The enclosed monitoring results indicate that the stormwater discharged on October 8,
2008, contained elevated concentrations of lead, total suspended solids, oil and grease, and pH
compared with the benchmark values contained in Stormwater general permit NCG 100000. In
addition, elevated concentrations of BOD, zinc, copper, and chromium were also detected.
Please review the requirements of your stormwater permit and take the required steps/actions to
reduce the concentrations of these pollutants leaving your site. Through the development and
implementation of stormwater management programs it is the Division of Water Quality's
expectation that the stormwater discharges at industrial sites covered by NPDES stormwater
permits do not impact receiving waters.
It is this Office's understanding that Lakeview Auto Parts plans to discontinue
automotive salvage activities at the facility and therefore will not be required to have stormwater
permit coverage through the Division of Water Quality. Please be advised that your facility will
still need to meet the requirements of the City of Charlotte Stormwater Ordinance since
stormwater from the facility discharges to the City of Charlotte Municipal Separate Stormwater
System (MS4). Therefore, you should continue measures to reduce the concentrations of the
pollutants noted above in your discharge.
