Attached please find the Stormwater Discharge Monitoring Report (DMR) and analytical
report for the sampling conducted at the above -referenced site. A stormwater sample was
collected from Outfall 001 on December 20, 2018 and analyzed for total suspended solids
(TSS), pH, total recoverable lead, oil and grease and ethylene glycol. As indicated on the
attached DMR, all parameters were reported below or within their respective benchmark

MONITORING REPORT
Facilities that incorporate a solvent management plan Into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my Inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."

Attached please find the Stormwater Discharge Monitoring Report (DMR) and analytical
report for the sampling conducted at the above -referenced site. A stormwater sample was
collected from Outfall 001 on June 14, 2018 and analyzed for total suspended solids
(TSS), pH, total recoverable lead, oil and grease and ethylene glycol..As indicated on the

MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."

Attached please find the Stormwater Discharge Monitoring Report and analytical report
for the sampling conducted at the above -referenced site. A stormwater sample was
collected from Outfall 001 on December 20, 2017 and analyzed for total suspended solids
(TSS), pH, total recoverable lead, oil and grease and ethylene glycol. With the exception
of TSS, all other parameters were reported below or within their respective benchmark
values. Based on the results of this sampling event, a Tier One Response has been
implemented at the facility. If you have any questions or need additional information,

STORMWATER DISCHARGE OUTFALL (SDO)
MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds Used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (770), 1
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."

STORMWATER DISCHARGE OUTFALL (SDO)
MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or stormwater
runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent management plan
included in the Stormwater pollution Prevention Plan."

MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver, the
discharger shall sign the following certification statement:

Attached please find the Stormwater Discharge Monitoring Report and analytical report
for the sampling conducted at the above -referenced site. A stormwater sample was
collected from Outfall 001 on June 22, 2015 and analyzed for total suspended solids
(TSS), pH, total recoverable lead, oil and grease and ethylene glycol. With the exception
of TSS, all other parameters were reported below or within their respective benchmark
values. Based on the results of this sampling event, a Tier One Response has been

STORMWATER POLLUTION PREVENTION PLAN SECTION cont'd:
Vehicle parts are washed indoors and the washwater is recycled for future use. The antifreeze and used motor oil are
collected and disposed by a contracted company (Liquid Gator). Used motor oil is also reused in the indoor heating
system. Liquid Gator has also been contracted for the removal and disposal of the recycle system's sludge/wastewater
on an annual basis. Gas is either reused in the on -site equipment or sold to ernployees.
The facility staff must ensure that oil spills/dischwges are properly cleanted up and dlsposed as part of the SPPP's Spill
Response Procedures_ Pleases note that the failure to properly clean up oil spills/discharges can subject the responsible
party to potential civil penalties.

Comment: The facility had contracted a company (James Environmental) to develop a
Stormwater Pollution Prevention Plan (SPPP). The contracted company also provides
the on -site staff with the annual SPPP review/updates, employee training, and one of
the semi-annual facility stormwater inspections. All of these activities were last
performed in April 2012_ The records reviewed during the inspection were organized
and well maintained. The facility staff must ensure the following is documented in the
SPPP: three year history of significant spills or a notation that'no spills occurred (if
applicable), the July - December facility stormwater inspection, and the re-evaluation of
the stormwater outfall for the presence of non-stormwater discharges. See "Summary"
Section for additional comments.

Comment: Analytical monitoring events were performed in April 2012 and December
2012. The facility staff must ensure the stormwater is collected directly into the oil &
grease container and the pH analyses are not performed by pH strips. The on -site
representative was emailed a listing of NC certified commercial labs and a pH
meter/analysis guidance document.
