forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon approval of the
Division of Energy, Mineral and Land Resources (DEMLR). DEMLR may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DEMLR, the Coastal Area Management Act, or any other federal or local government permit that
may be required.

to receiving waters designated as UT to Little Sugar Creek, a class C water in the Catawba River
Basin, in accordance with the effluent limitations, monitoring requirements, and other conditions
set forth in Parts I, II, III, and IV of General Permit No. NCG080000 as attached.
This certificate of coverage shall become effective November 17, 2017.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Hi Zahid,
I am familiar with the subject facility. They received a Notice of Violation from us approximately 3 months ago, but they
do not have any outstanding stormwater complaints/violations. This facility will be inspected by our office.
I see that they didn't completely fill out the receiving water portion of the NOI and I'm not sure that they actually have a
SWPPP or structural stormwater BMPs, but everything else looks fine.
We would recommend this permit be issued.

A General Permit application or additional information was received on 11/3/2017 for in Mecklenburg County. The
assigned Permit Number is NCG080959.

Please review the attached NO] application and respond to me with any recommendations for the permit. Your
recommendation now to issue the permit serves as your acknowledgement that (1) the facility is located in your region,
(2) that there are no current complaints outstanding about the facility that have not been dealt with, and (3) that the facility
may ultimately be inspected by your office.
If you need any more info, give me a call. If you could send me a recommendation to issue the permit by around 11/20/17
or so, I'd appreciate it, so we can issue their COC.

1) All storage tanks and outdoor materials will be moved inside and away from any doors to make
sure no oil gets spilled on any 'soil.
2) All out of service trucks will only be worked on inside. We are adding more covered service bays
to the shop and extending the length of the concrete pad so that all work will be performed
under cover and on a concrete pad with proper.
3) All oil containers will be stored indoors. All oily parts will be stored inside or covered. All oil spills
will be cleaned up immediately using oil-dri and properly disposed of.
4) Containment matt has been placed next to fuel tank to catch any spills.
5) We have also invested in an oil pump consisting of specialized fittings and hoses to hook up to
the trucks to drain the oil and pump it out without any drips or spills.

There is nothing in our system where they have submitted a NOI. I agree they need coverage under NCG080000. I will
hand delivery a NCG080000 NOI form next week.

inspected hack in Augtist.'Ne facility has removed the contaminated soil from the site and made some significant
improvements in housekeeping and operations to prevent discharges of'oil and other pollutants. I have attached copies of

Attached is a facility inspection report for MigWay, a trucking company located at 9349 China Grove Church Road in
Charlotte (Pineville ZIP code). As a result of this inspection, this facility received a Notice of Violation for multiple
discharges of motor oil that have caused significant pavement and soil staining. Overall housekeeping in the truck
maintenance areas and waste disposal areas was very poor. We also observed significant erosion caused by recent grading
and fill activities on the property. In addition, the City of Charlotte has also issued another NOV to the facility for
exceeding impervious area limits under the City's Post -Construction Stormwater Ordinance.
MigWay does not have an NPDES stormwater permit. I do not know the SIC code. The facility has at least one outfall;
diesel fuel, new oil, and used oil are stored on -site; and there is a significant amount of exposed processing area at this
time (virtually all truck maintenance is being performed outdoors). The facility manager claims that they will be covering
their service areas with a roof in the near future. With significant upgrades to the site, the facility could possibly qualify
for a No Exposure Exclusion, but as it stands now, it may be a good candidate for coverage under the NCG08000 general
stormwater permit. I asked the company president, Mr. David Voronin, to contact your office about applying for a permit.

Hi James,
Please find attached the report from my August 23`d inspection of independent Beverage Corporation (IBC), located at
3936 Corporation Circle in Charlotte. The facility is currently non -permitted, but it seems like they may fall under the
NCG06 permit based on nature of operations and the SIC equivalent of the NAICS code they provided to me. The facility
produces and packages carbonated beverages, and site operations include forming plastic bottles, receiving/offloading
high fructose corn syrup (HFCS) and proprietary liquids, and processes related to beverage production. The following
items are exposed or partially exposed to stormwater: a BOD/waste tank, HFCS tanks, a crusher used to crush cans of
off -spec product (the subject of a previous NOV for product discharge), 55-gallon used oil drums, waste containers, and
numerous plastic crates and containers.
