Subject: NSRR — Charlotte Engine Terminal 2019 - First of two annual stormwater monitoring (sampling) results Date: March 4, 2019 Attached is the completed DMR, tab results and field qualitative (visual) inspection reports for the first of two, annual analytical stormwater monitoring and qualitative inspections that was done in January. This was the first of two required analyses per year. The DMR report (the original plus one copy) will need to be mailed to the state at the address on the second page of the form. This should be mailed by April 3, 2019. On the first page of the report form, there is a space at the top for completing the date the report is submitted. There is also a space on the first page for listing the amount of new motor oil usage. This will need to be completed by someone with knowledge of the on site activities. The amount to enter should be the average monthly usage over the course of the calendar year. This report needs to be signed and dated on the second page by the responsible corporate officer or his authorized representative. If you have any questions or need any assistance on this matter, please call me and we can discuss. This DMR is all that needs to be submitted to the state. The lab results and the field qualitative (visual) inspection reports do not need to be submitted. A copy of the reports and the supporting lab results should be kept on site for at least five (5) years. All that you need to do is complete the date submitted and list the amount of oil used on page 1 of the DMR and have page 2 of the report form signed (and dated.) Then submit the original plus one copy of the signed report to the state at the address on the second page. I was not sure how NSRR has been handling these signatures, but if you have any questions or need any assistance on this matter, please call me and we can discuss. If there are other forms that NSRR would also like completed, if someone will provide these forms I will try to use them as well (although I consider this to be redundant.) Subject: NSRR — Charlotte Engine Terminal 2017 — first of two annual stormwater monitoring (sampling) results Date: March 13, 2017 Leonard, Attached is the completed DMR, lab results and qualitative (visual) inspection report for the twice annual analytical stormwater monitoring that was done in February. This was the first of two required analyses per year. The DMR report (the original plus one copy) will need to be mailed to the state at the address on the second page of the form. This should be mailed by April 9, 2017. On the first page of the DMR report form, there is a space at the top for completing the date the report is submitted. There is also a space on the first page for listing the amount of new motor oil usage. This will need to be completed by someone with knowledge of the on site activities. The amount to enter should be the average monthly usage over the course of the calendar year. This DMR report needs to be signed and dated on the second page by the responsible corporate officer or his authorized representative. I was not sure how NSRR has been handling these signatures, but if you have any questions or need any assistance on this matter, please call me and we can discuss. This DMR is all that needs to be submitted to the state. The lab results and qualitative visual inspection report do not need to be submitted. A copy of the report and the supporting lab results and qualitative (visual) inspection report should be kept on site for at least five (5) years. All that you need to do is complete the date submitted and list the amount of oil used on page 1 and have page 2 of the report form signed (and dated.) Then submit the original plus one copy of the signed report to the state at the address on the second page.
