Staff with the Division's Mooresville Regional Office has reviewed your request for a determination that
stormwater discharge outfall No. 001 be granted representative outfall status for your facility. Based on a site visit
conducted on April 11, 2013, we are hereby approving this request effective immediately. In accordance with 40
CFR §122.21(g)(7), you are authorized to sample outfall number No. 001 as a representative outfall. Sampling of
outfall No. 001 should begin at your next representative sampling event. We also want to remind you that the
permit still requires Qualitative Monitoring be performed at all SDOs, regardless of representative status unless the
outfall is not captured by the Phase I stormwater program.
Please append this letter to your Stormwater Pollution Prevention Plan or permit to document that
representative outfall status has been approved. If changes in drainage areas, structures, processes, storage
practices, or other activities occur that significantly alter the basis of this approval, representative outfall status may
no longer be valid. You should either resume sampling at all SDOs, or reapply to this Office for representative
outfall status based on updated information. If you have any questions or comments concerning this letter, I can be

REQUEST FORM MOORESVILLE REGIONAL OFFICE
If a facility is required to sample multiple discharge locations with very similar storm water discharges, the
permittee may petition the Director far Representative Outfall Status (ROS). DWQ may grant Representative
Outfall Status if stormwater discharges from a single outfall are representative of discharges from multiple
outfalls. Approved ROS will reduce the number of outfalls where analytical sampling requirements apply.
If Representative Outfall Status is granted, ALL outfalls are still subject to the qualitative monitoring
requirements of the facility's permit —unless otherwise allowed by the permit (such as NCG020000) and DWQ
approval. The approval letter from DWQ must be kept on site with the facility's Stormwater pollution
Prevention Flan. The facility must notify DWQ in writing if any changes affect representative status.

Representative Outfall Status Request
4) Detailed explanation about why the outfalls above should be granted Representative Status:
(Or, attach a letter or narrative to discuss this information.) For example, describe how activities and/or
materials are similar.
The activities at Outfall 001 come from our Trailer repair shop area in which,. there is less product stored thus havinq
less potential contaminants coming across the paved area into the outfall. The activities at Outfall 002 come from our
Tractor repair shop, in which, it has the most exposure coming across the gravel area into the outfall. Since each
outfall have the similar activities the request is to continue monitoring Outfall 002 and discontinue monitoring Outfall
001.
5) Certification:
North Carolina General Statute 143-215.6 B(i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record,
report, plan, or other document filed or required to be maintained under this Article or a rule implementing this
Article; or who knowingly makes a false statement of a material fact in a rulemaking proceeding or contested case
under this Article; or who falsifies, tampers with, or knowingly renders inaccurate any recording or monitoring device
or method required to be operated or maintained under this Article or rules of the [Environmental Management]
Commission implementing this Article shall be guilty of a Class 2 misdemeanor which may include a fine not to exceed
ten thousand dollars ($10,000).
I hereby request Representative Outfall Status for my NPDES Permit. I understand that ALL outfalls are still
subject to the qualitative monitoring requirements of the permit, unless otherwise allowed by the permit
and regional office approval. I must notify DWQ in writing if any changes to the facility or its operations take
place after ROS is granted that may affect this status. If ROS no longer applies, I understand i must resume
monitoring of all outfalls as specified in my NPDES permit.

In accordance with your application for a discharge permit received on August 10, 2005,
we are forwarding herewith the subject certificate of coverage to discharge under the subject
state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other federal or local governmental permit that
may be required.

Inspection Summary:
The Stormwater Pollution Prevention Plan (SPPP) at this facility was generally well maintained and up-to-date with
respect to the requirements of the General Permit. There were a few minor deficiencies noted and these are detailed
below.
1. The general location map should include the latitude and longitude of the points of discharge as required by Part Il,
Section A, No. 1(a) of the GP.
2. Documentation should be added to the SPPP indicating whether or not there have been any spills during the three
previous years as required by Part II, Section A, No. 1(d) of the GP. This Certification can be done at the time of the
annual update.
3. There was no mention of whether or not feasible alternatives to current practices had been evaluated as required by
Part ll, Section A, No. 2(a) of the GP.
4. The existing BMP did not include a description of the existing detention pond as required by Part II, Section A, No. 2(c)
of the GP.
5. The Preventative Maintenance and Good Housekeeping Program did not include a inspections and maintenance
activities of the existing SW control system (detention pond) as required by Part II, Section A, No. 4 of the GP.
6. As required by Part II, Section A, No. 5 of the GP, the person (or team) responsible for implementing the employee
training on proper spill response and clean-up procedures should be indentified in the SPPP.
7, The SPPP should contain an annual certification that the facility has evaluated the site for the presence of
non-Stormwater discharges and signed in accordance with the instructions found in Part II, Section A, No. 1(e) of the GP.
The J.B. Hunt site is actually two properties that have been joined to form one company. There is a SW outfall on each
property, but it appears that a significant amount of the SW runoff from this facility leaves the site through outfall 002,
which is the outfall that discharges_from the existing detention pond. The SW flow to outfall 002 travels across a gravel
parking lot, which allows the SW to pick up small fines and silt from the gravel and deposit them into the pond. As a result
of the silt deposition, the pond is slowly silting in, which has allowed emergent vegetation and trees to become
established both in and around the pond. It is recommended that the vegetative growth within the pond be removed and
the pond brought back to it's original design capacity so as to improve settling efficiency and overall maintenance
capability.

1. The general location map should include the latitude and longitude of the points
of discharge as required by Part II, Section A, No. 1(a) of the GP.

• REVISION — I reviewed this and discussed with the Environmental VP at
Marshall Miller & Associates, the firm that helped me create the Plan and make
the revisions. Our understanding is this is a "once" one time requirement. I can
do'this certification on an annual schedule if you prefer. Let me know and I will
create an annual form and have this certification done annually if this is what
you want. Copy of certification is enclosed for reference.

2 Significant Materials and Spill Record
2.1 Significant Material Description
J.B Hunt has evaluated alternatives to the products used and the method and activities
completed at the facility and have adopted the most environmentally sound options as
required by Part I. Section A, No. 2 (a) of the General Permit.
Table 2-1 summarizes the primary significant materials used at the Facility, including
method of storage, storage and dispensing location, likelihood of contacting storm water
and the Best Management Practices (BMPs) that are employed for protecting storm
water. Table 2-1 also includes any equipment, parts or significant materials that are
stored outdoors in a manner exposed to storm water runoff. Generally, storm water does
not receive any treatment and is routed via overland flow to an on -site retention pond and
a intermittent drainage channel. The Facility maintains emergency response equipment
and utilizes BMPs in the event of an accidental release of a significant material.
2.2 Spill Summary
On April 12, 2005, the National Response Center (NRC) database was queried to
determine if any oil and/or chemical incidents have occurred and been reported at the
Facility. There have been no reports of discharges deemed as a harmful quantity (i.e.,
producing a visible sheen on receiving waters) since November 1, 1995 (see Appendix B
for NRC database report).
2.3 Sampling Data
Per the Storm Water General Permit (provided in Appendix A), the J.B. Hunt Terminal is
required to conduct sampling for pollutants of concern. The following requirements are

Samples must be taken at the nearest accessible point after final treatment, but prior to actual
discharge or mixing with receiving waters. Grab samples shall be taken during the first 30
minutes of discharge, if possible. If conditions are not safe for sampling due to local
flooding, high winds, electrical storms, etc., the permittee shall collect a sample once
dangerous conditions have passed and must provide documentation as to why timely

3.2 Preventive Maintenance (Baseline)
A preventive maintenance program identifies areas and equipment that need to be
maintained to prevent the possibility of storm water impact. The Team Leader, or
designee, of the Pollution Prevention Team (refer to Section 5) must ensure that
preventive maintenance practices are utilized. Maintenance activities applicable to this
Facility include the following:
• Replace or repair waste collection bins (especially oil filter containers) if damaged or
leaking,
• If indications of leaks, spills, or drips are evident, find, report, and correct the
problem: and
• Whenever possible, shield all significant materials from exposure to storm water.
3.3 Visual Inspections (Baseline)
A routine Facility inspection program is a regulatory requirement. The inspection shall
be conducted as described in Section 6 by the Pollution Prevention Team Leader, or
designee, who shall be familiar with the intent and goals of this SWPPP. A Facility
inspection checklist is provided in Appendix C of this SWPPP, If conditions are
discovered which could impact storm water quality, the Team Leader shall arrange for
corrective action or designate an individual to make such arrangements. The complete
checklist, along with any records of corrective actions, must be maintained for a
minimum of six years. It is recommended that these inspection records be maintained
with the SWPPP in Appendix D.
3.4 Spill Prevention and Response (Baseline)
All employees should receive initial and refresher training in spill response and related
environmental issues. All employees are responsible for the clean up of any spill. The
Facility's Spill Prevention, Control, and Countermeasure (SPCC) Plan (located on -site) is
used to prevent, manage, and mitigate oil -related spills. Small, routine drips and spills
may be cleaned by applying loose absorbent to the spill and then disposing of the
absorbent properly. J.B. Hunt also has a spill kit on hand in case of emergency.
Should a spill have the potential to leave the Facility or be discharged from the Facility,
the spill nest be reported immediately to:

5 Pollution Prevention Team
The Pollution Prevention Team is a designated group of J.B. Hunt Transport employees
or contractor employees that are responsible for ensuring the proper development and
implementation of the SWPPP. Members of the team include management level
personnel and those who are familiar with the Facility and its operations.. The Team is
responsible for defining an appropriate set of goals for the Facility storm water
management program. In addition, the team must be aware of any changes to the Facility
and implement necessary changes to the SWPPP.
5.1 Team Position Descriptions
The following presents descriptions of the positions and responsibilities of the Pollution
Prevention Team:
• Team Leader - The leader is responsible for ensuring the proper implementation
of' the SWPPP at the Facility level. The Team Leader must be aware of any
significant changes to the Facility and is also responsible for ensuring the SWPPP
is reviewed annually for accuracy and applicability as described in Section 6.3.
The Team Leader is also responsible for ensuring that facility personnel receive
the appropriate SWPPP training. The Team Leader will also conduct training for
personnel correct spill response and cleanup procedures.
• Team Members - The members are responsible for the proper implementation of
the SWPPP at each department level. Team members are to communicate with
the 'learn Leader on SWPPP implementation issues and significant Facility
changes.
