Per Pan 1, Section D of the attached NPDES Permit (Appendix A), the Team Leader, or his designee, shall visually
inspect a sample of the storm water discharges associated with industrial activities from each permitted outfall. Samples
should be taken during a qualifying storm event (>0, i inches) within the first 30 minutes of when runoff or snowmelt

2019 — first of two annual stormwater monitoring (sampling) results
Date: February 19, 2019
Attached is the completed DMR, lab results and field qualitative (visual) inspection reports for the twice
annual analytical stormwater monitoring that was done in January. This was the first of two required
analyses per year. The DMR report (the original plus one copy) will need to be mailed to the state at the
address on the second page of the form. This should be mailed by March 15, 2019.
On the first page of the report form, there is a space at the top for completing the date the report is
submitted. There is also a space on the first page for listing the amount of new motor oil usage. This will
need to be completed by someone with knowledge of the on site activities. The amount to enter should be
the average monthly usage over the course of the calendar year.
This report needs to be signed and dated on the second page by the responsible corporate officer or his

This DMR is all that needs to be submitted to the state. The lab results and the -field qualitative
(visual) inspection reports, do not need to be submitted. A copy of the reports and the supporting lab

DMR and have page 2 of the report form signed (and dated.) Then submit the original plus one copy of
the signed report to the state at the address on the second page. As required by the permit, the report must
be submitted by March 15, 2019 to be within thirty days of receiving the laboratory results.
I have also included a copy of the completed "in-house" inspection report forms that I was asked to

annual analytical stormwater monitoring that was done in October. This was the second of two required
analyses per year. The DMR report (the original plus one copy) will need to be mailed to the state at the
address on the second page of the form. This should be mailed by December 26, 2018.
On the first page of the report form, there is a space at the top for completing the date the report is
submitted. There is also a space on the first page for listing the amount of new motor oil usage. This will
need to be completed by someone with knowledge of the on site activities. The amount to enter should be
the average monthly usage over the course of the calendar year.

Per Part I, Section D of the attached NPDES Permit (Appendix A), the Team Leader, or his designee, shall visually
inspect a sample of the storm water discharges associated with industrial activities from each permitted outfall. Samples
should be taken during a qualifying storm event (>O.l inches) within the first 30 minutes of when runoff or snowmelt
begins discharging. Samples may only be collected during daylight hours and must be taken in a well lit area. The sample
may not be collected less than 72 hours following a previous measurable storm event. Each collected sample must be
visually inspected for the items listed below. Record the condition/quality of each item and note any adverse conditions.
Samples may be returned to the collection point following visual examination.

Attached is the completed "qualitative" inspection reports for the stonmwater outfalls 001 and 002. This
visual inspection must be completed twice per year, once in the spring and once in the fall. This is the
spring inspection for this outfall for this year. I will plan to do the fall inspection later this year (2017.).
These reports must be kept on site in case requested for review by a state inspector.
Your permit does not require that these visual inspection reports be submitted unless requested to
do so by the state.
I have also included a copy of the completed "in-house" inspection report forms that you a ;ked me to
complete.
The last copy of the SWP3 that I received only indicates one stormwater discharge outfall. This is shown
as outfall 001 on the site plan. As we discussed, you have advised me that a second outfall was found and
that NSRR wants both outfalls to have twice annual visual and quantitative (analytical) sampling and
analysis. I have done this and 1 am designating the second outfall as 002. I highly recommend that the
SWP3 he revised and that the site plan be updated to show the location of outfall 002. Otherwise, 1
suspect that this may create some confusion and possible compliance issues during the next state or
county stormwater inspection.
I have also collected the "quantitative" stormwater sample for outfalls 001 and 002. I am also forwarding
those results to you along with the completed report form.

Per Part I, Section D of the attached NPDPS Permit (Appendix A), the Team Leader, or his designee, shall visually

Enclosed are the monitoring results from the laboratory analyses performed on the
discharge from Stormwater Outfall 001 at the subject facility. The samples analyzed were grab
samples collected during the sampling investigation performed on June 20, 2008, by Ms. Heather
Sorensen and Mr. Chris Elmore of Charlotte -Mecklenburg Storm Water Services (CMSWS).
This sampling investigation was conducted as part of a cooperative working agreement between
Mecklenburg County and the Division of Water Quality. In addition, the sampling investigation
was conducted on behalf of the City of Charlotte in compliance with the City's NPDES Permit,
NCS000240, Part II Section H.
The enclosed monitoring results indicate that the stormwater discharged on June 20,
2008, contained an elevated concentration of total suspended solids compared with the
benchmark concentration contained in Stormwater General Permit NCG680000 and your
Certificate of Coverage under this permit. Please review the requirements of your stormwater
permit and take the required steps/actions to reduce the concentration of total suspended solids
leaving your site. Through the development and implementation of stormwater management
programs it is the Division of Water Quality's expectation that stormwater discharges are not
impacted by the industrial activities of the NPDES permit holders.
In addition, through discussions with the CMSWS inspectors it is the understanding of
this Office that the primary industrial activity that occurs at the Liddell Street facility is the
manufacturing of railroad track maintenance equipment. This industrial activity (manufacturing
of industrial and commercial machinery) is covered by Stormwater General Permit NCGO30000 -
Metal Fabrication and would not be covered as an approved activity under the current stormwater
permit at the site, NCG080000 — Transit and Transportation. Please contact the stormwater
permitting group in Raleigh to discuss the appropriate stormwater permit coverageTor the Liddell
Street facility.
