However, if the condition worsens. more intense investigation may be warranted at that time. Subject: NSRR — Charlotte Roadway Shop 2019 — first of two annual stormwater monitoring (sampling) results Date: February 19, 2019 Attached is the completed DMR, lab results and field qualitative (visual) inspection reports for the twice annual analytical stormwater monitoring that was done in January. This was the first of two required analyses per year. The DMR report (the original plus one copy) will need to be mailed to the state at the address on the second page of the form. This should be mailed by March 15, 2019. On the first page of the report form, there is a space at the top for completing the date the report is submitted. There is also a space on the first page for listing the amount of new motor oil usage. This will need to be completed by someone with knowledge of the on site activities. The amount to enter should be the average monthly usage over the course of the calendar year. This report needs to be signed and dated on the second page by the responsible corporate officer or his authorized representative. If you have any questions or need any assistance on this matter, please call me and we can discuss. This DMR is all that needs to be submitted to the state. The lab results and the -field qualitative (visual) inspection reports, do not need to be submitted. A copy of the reports and the supporting lab results should be kept on site for at least five (5) years. All that you need to do is complete the date submitted and list the amount of oil used on page 1 of the DMR and have page 2 of the report form signed (and dated.) Then submit the original plus one copy of the signed report to the state at the address on the second page. As required by the permit, the report must be submitted by March 15, 2019 to be within thirty days of receiving the laboratory results. I have also included a copy of the completed "in-house" inspection report forms that I was asked to complete. Again, these are not required to be submitted to the state. Subject: NSRR — Charlotte Roadway Shop 2018 —second of two annual stormwater monitoring (sampling) results Date: November 27, 2018 Attached is the completed DMR, lab results and field qualitative (visual) inspection reports for the twice annual analytical stormwater monitoring that was done in October. This was the second of two required analyses per year. The DMR report (the original plus one copy) will need to be mailed to the state at the address on the second page of the form. This should be mailed by December 26, 2018. On the first page of the report form, there is a space at the top for completing the date the report is submitted. There is also a space on the first page for listing the amount of new motor oil usage. This will need to be completed by someone with knowledge of the on site activities. The amount to enter should be the average monthly usage over the course of the calendar year. This report needs to be signed and dated on the second page by the responsible corporate officer or his authorized representative. If you have any questions or need any assistance on this matter, please call me and we can discuss. This DMR is all that needs to be submitted to the state. The lab results and_the_field qualitative (visual) inspection reports, do not need to be submitted. A copy of the reports and the supporting lab results should be kept on site for at least five (5) years. All that you need to do is complete the date submitted and list the amount of oil used on page 1 of the DMR and have page 2 of the report form signed (and dated.) Then submit the original plus one copy of the signed report to the state at the address on the second page. As required by the permit, the report must be submitted by December 26, 2018 to be within thirty days of receiving the laboratory results. This visual inspection must be completed twice per year, once in the spring and once in the fall. This is the spring inspection for this outfall for this year. I will plan to do the fall inspection later this year (2017.). These reports must be kept on site in case requested for review by a state inspector. Your permit does not require that these visual inspection reports be submitted unless requested to do so by the state. I have also included a copy of the completed "in-house" inspection report forms that you a ;ked me to complete. The samples analyzed were grab samples collected during the sampling investigation performed on June 20, 2008, by Ms. Heather Sorensen and Mr. Chris Elmore of Charlotte -Mecklenburg Storm Water Services (CMSWS). This sampling investigation was conducted as part of a cooperative working agreement between Mecklenburg County and the Division of Water Quality. In addition, the sampling investigation was conducted on behalf of the City of Charlotte in compliance with the City's NPDES Permit, NCS000240, Part II Section H. The enclosed monitoring results indicate that the stormwater discharged on June 20, 2008, contained an elevated concentration of total suspended solids compared with the benchmark concentration contained in Stormwater General Permit NCG680000 and your Certificate of Coverage under this permit. Please review the requirements of your stormwater permit and take the required steps/actions to reduce the concentration of total suspended solids leaving your site. Through the development and implementation of stormwater management programs it is the Division of Water Quality's expectation that stormwater discharges are not impacted by the industrial activities of the NPDES permit holders. In addition, through discussions with the CMSWS inspectors it is the understanding of this Office that the primary industrial activity that occurs at the Liddell Street facility is the manufacturing of railroad track maintenance equipment.
