1. On 10"' of January 2013 at approximately 4:15pm as a result of a malfunction of the
fire suppression system in our large hanger (Bldg 5 1 ) a release of AFFF Firefighting
foam was released. Some of the material entered a storm drainage system which was
captured in a concrete spill containment pond. Approximately 30-40 gallons also entered
a storm drainage outfall downhill from the hanger. The firefighting foam is a 3% solution
with water. The foam contained within the concrete pond was allowed to dissipate before
discharge. Site was visited the following day and there was no indication of any residual
loam.
2. This report is a follow-up to the report by phone and should serve as a log of events
for the spill. If you have any questions you can contact our office at 704/391-4327.

The Mooresville Regional Office staff have reviewed your request received on March 16, 2010, for a
determination that stormwater discharge outfall (SDO) 005 be granted representative outfall status for
stormwater outfalls 001, 002, 003, and 004. Based on the information and maps provided and -the site
visit of February 3, 2010, we are approving this request. In accordance with 40 CFR §122.21(g)(7), you
are authorized to sample outfall number 005 as a representative outfall. This approval is effective with
the next sampling. event. We also want to remind you that the permit still requires Qualitative Monitoring
be performed at all SDOs, regardless of representative status.
Please remember that any actions you initiate in response to benchmark exceedances as directed in the
tiered response provisions of your permit must address all drainage areas represented by SDO 005,
where appropriate.
Please append this letter to your Stormwater Pollution Prevention Plan (SPPP) or permit to document
that representative outfall status has been approved. If changes in drainage areas, structures,
processes, storage practices, or other activities occur that significantly alter the basis of this approval,
representative outfall status may no longer be valid. You'should either resume sampling at all SDOs, or
reapply to this office for representative outfall status based on updated information. If you have any

for issuance tomorrow.

perm ittee may petition the Director for Representative Outfall Status (ROS). DWQ may grant Representative

Our facility was visited by Marsha Aliocco from Mooresville Office of DENR and the option
to petition for representative outfall status was recommend. All five gutfalls currentiv being
T sampled are representative -in _-nature to outfall 005. There_bave been no exceedances in required
_ parameters. Qualitative monitoring will continue to be conducted at each of the 5 sampling pararn
outfalls.
5) Certification:
North Carolina General Statute 143-215.6 B(i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record,
report, plan, or other document filed or required to be maintained under this Article or a rule implementing this
Article; or who knowingly makes a false statement of a material fact in a rulemaking proceeding or contested case
under this Article; or who falsifies, tampers with, or knowingly renders inaccurate any recording or monitoring device
or method required to be operated or maintained -under this Article or rules of the [Environmental Management]
Commission implementing this Article shall be guilty of a Class 2 misdemeanor which may include a fine not to exceed
ten thousand dollars ($10,000).

Dear Mr. Ingram:
On February 3, 2010, Ms. Marcia Allocco and Mr. Lon Snider from the Mooresville Regional Office of the
Division of Water Quality conducted a site inspection of the NC Air National Guard facility located at 4930
Minuteman Way in Charlotte, Mecklenburg County, North Carolina. The site drains to Taggert Creek, Class C
waters located in the Catawba River Basin. The site visit and file review revealed that the subject facility is
covered by NPDES General Stormwater Permit — NCG080000.
Accordingly, the following observations were noted during the Division of Water Quality inspection (please see
the attached addendum for information about your permit):
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded, and properly implemented.

However, as noted in the SPPP section of the attached report the secondary containment at Bldg. 52 needs a
locking mechanism added, the tracking sheet for releases from the POL secondary containment area needs
modification to include a description of the water being released, and semi-annual stormwater facility
inspections need to be completed (not annual).
2) Qualitative Monitoring
Qualitative monitoring has been conducted and recorded in accordance with permit requirements.

A review of historical analytical data found that the pH analyses were not completed within the requirements of
40 CFR §136 (15-minute hold time). Pease adjust your procedures to meet the 15-minute hold time. Enclosed
is guidance on completing pH analyses in accordance with 40 CFR §136.

4) Permit and Outfalls
The last illicit discharge evaluation was conducted in 2006. Please note that the revised NCG080000 permit
issued in 2007 now requires this evaluation be completed annually (not once per permit cycle).
5) Sum
The summary section of the enclosed inspection report includes suggestions to reduce the potential for
stormwater contamination from the industrial operations and land -use at the facility.
Thank you for the assistance and cooperation during this inspection. Overall, the facility's Stormwater program
was well developed and implemented and this Office commends the permittee's efforts to ensure compliance
with your NPDES permit.

ADDENDUM
The National Pollutant Discharge Elimination System (NPDES) program was established under the federal
Clean Water Act and then delegated to the Division of Water Quality for implementation in North Carolina. The
NPDES permitting program for stormwater discharges was established in 1990. Phase I focuses on site and
operations planning to reduce pollutant sources.
Through the NPDES permitting program industrial facilities that fall into one of the subject ten categories are
required to obtain permit coverage under a general permit or an individual permit, depending upon the
facilities SIC code and the industrial activity occurring at the facility. There are currently 19 active general
stormwater permits in North Carolina. One condition that is applicable to both the general permits and
individual stormwater permits is the requirement to develop and implement site -specific comprehensive
stormwater pollution prevention plans (SPPP). These plans are required to include a comprehensive
evaluation of the site and operations to reduce pollutant sources and prevent pollutant discharge in stormwater
runoff.

General Stormwater Permit], and the Representative Outfall Status application can be found on this website.
The following items are a subset of permit requirements and are listed to assist you and help bring your
attention to certain permitting issues, process, and condition requirements of your permit. Please see the
abovementioned website to obtain a copy of the permit so you may fully comply with the conditions therein.
Failure to properly comply with conditions of the permit, like those bulleted below, constitutes permit
violations and is subject to civil penalty assessment of up to $25,000.00 per day.
• Industrial facilities that fall into one of the subject ten categories are required to obtain permit coverage
under a general NPDES stormwater permit or an individual NPDES stormwater permit
• All persons desiring to have facilities covered by a General Permit must register with the Division of
Water Quality (DWQ) by the filing of a Notice of Intent (NOI) and Applicable Fees.
• This Notice of intent (NOI) must be submitted and a Certificate of Coverage issued prior to any
discharge of stormwater associated with the industrial activity, process wastewater, and/or mine
dewatering.
• Once the Notice of Intent is received, DWQ will review and determine whether a Certificate of Coverage
(COC) may be issued. If this Certificate of Coverage is issued the facility is covered by the General
Permit. That is, the permittee is authorized to discharge stormwater, process wastewater, and/or mine
dewatering (pursuant to the type of.General Permit) until the permit expires or is modified or revoked.
• Any other point -source discharge to surface waters of the state is prohibited unless it is an allowable
non-stormwater discharge or is covered by another permit, authorization, or approval.
• There are currently 19 active general permits. Permit conditions vary accordingly; however, most of the
permits have the following condition requirements (you must read your entire permit to ensure full
compliance):

o Collection, reporting, and recording of Vehicle Maintenance Monitoring
c Records — Visual monitoring shall be documented and records maintained at the facility along
with the Stormwater Pollution Prevention Plan. Copies of the analytical monitoring results shall
also be maintained on site. The Permittee shall retain records of all monitoring information,
including all calibration and maintenance records and all original strip -chart recordings for
continuous monitoring instrumentation, and copies of all reports required by this general permit
for a period of at least 5 years from the date of the sample measurement, report, or application.
o Expiration — The permittee is not authorized to discharge after the expiration date of the general
permit. In order to receive automatic authorization to discharge beyond the expiration date, the
permittee shall submit forms and fees as are required by the agency authorized to issue permits
no later than 180-days prior to expiration date. Any permittee that has not requested renewal at
least 180 days prior to expiration, or any permittee that does not have a permit after the
expiration and has not requested renewal at least 180 days prior to expiration, will be subject to
enforcement. Failure to renew your permit is a violation.
o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is not
transferable to any person expect after notice to and approval by the Director of the Division of
Water Quality. The Director may require modification or revocation and reissuance of the
Certificate of Coverage to change the name and incorporate such other requirements as may be
necessary under the Clean Water Act. The Permit is NOT transferable.
o Name Change, Closure, or Facility Ownership change —The permittee is required to notify the
Division in writing in the event the permitted facility is sold or closed. The Division of Water
Quality views changes of name or ownership as a modification that requires the Director's
approval and reissuance of the permit. Name and ownership changes required you to complete
a Name/Ownership Change Form from the Division and mail it back to the Division of Water
Quality along with all appropriate information.
o Proper Operation and Maintenance is a requirement of the permit.

Inspection Summary:
A site walk-through was conducted as part of the stormwater permit inspection. The following observations were noted.
An air compressor behind Bldg. 7 had a discharge pipe to the ground surface and could be a source of stormwater
contamination. Please investigate the discharge from this pipe and if it could contain oil the discharge should be
contained and properly disposed.
There was erosion in the drainage swale that runs along Saber Drive and discharges just upstream of Outfall 005. This is
adding sediment to Outfall 005 and Taggert Creek, sediment is the number pollutant of NC surface waters. The permittee
should add rfprap or some other velocity dissipater to reduce the erosive, velocity of stormwater runoff in this swale.
Bldg. 46 is located along the northern portion of the site and adjacent to a stormwater inlet that discharges to Outfall 005,
The building is not a permanent structure and does not provide any containment for spilled materials. The building serves
as storage for outdoor maintenance equipment and also the fuels used to power this equipment. The permittee should
provide a spill kit for this area (not evident during the inspection), place the small containers of fuel within a secondary
containment structure (e.g. plastic trays), and investigate measures to prevent the discharge of petroleum products or
other materials directly to Outfafl 005. Suggestions include the permanent installation of absorbent booms/socks around
the inlet structure, providing a drain cover during dry weather, or constructing a berm to provide increased detention time
of any spilled material.
During the inspection multiple generators were stored just outside of Bldg. 22 (Generator shop) directly over a stormwater
drainage structure. Although, there was no evidence of leaks from the generators the permittee should refrain from
storing equipment over stormwater drains that have the potential to contaminate stormwater.

Comment: Qualitative monitoring records were reviewed from June 2006, March
2008, November 2008, July 2009, and November 2009. The monitoring noted erosion
near Outfall 001. However, through discussion with site personnel it was determined
that the monitoring is performed off site and the erosion cannot be controlled by the Air
National Guard. A monitoring point within the facility was highlighted during the
inspection (inlet to a stormwater pond located between Morris Field Drive and Airport
Drive) but it had limited access (within a locked and fenced -in area). Movement of the
monitoring location for Outfall 001 is at the discretion of the Air National Guard.

Comment: The Air National Guard has conducted analytical monitoring at the five
designated stormwater outfalls on site. A discussion occurred during the inspection
regarding representative outfall status (ROS) for the analytical monitoring portion of
NCG080000. The permittee should apply for ROS if interested using the application
form available on the DWQ website,

During a review of the analytical data it was noted that pH analyses are not being
conducted as per 40 CFR §136. As per 40 CFR §136 the hold time for pH analysis is
only 15 minutes and usually requires on -site analysis. The Chain -of -Custodies for
historical stormwater sampling reviewed during the inspection found that the 15-minute
hold time was not met. This was attributed to the shipment of samples to an off -site
laboratory for analysis. Please see the enclosed guidance on conducting pH analyses
in conformance with 40 CFR §136 using the available on -site pH meter.

Comment: The permit requires secondary containment devices be controlled by
manually activated valves or other similar devices (which shall be secured closed with a
locking mechanism). The Bldg. 52 secondary containment structure is manually
activated but is not "locked shut" with a locking mechanism (lock). The permit requires
that accumulated stormwater in the containment area be "visually observed for color,
foam, outfall staining, visible sheens and dry weather flow, prior to release. Records
documenting the individual making the observation, the description of the accumulated
stormwater, and the date and time of the release shall be kept for a period of five
years." The secondary containment release records for the POL area do not note the
description of the accumulated stormwater (color, foam, sheens, etc.).
The permit requires semi-annual stormwater facility inspections; only one inspection of
the stormwater facilities was completed.

TECHNICAL ASSISTANCE FOR FIELD ANALYSIS OF pH
This document is intended to provide technical assistance and guidance for the measurement of pH. This is
not meant to be a substitute for the method, but is offered as a guide to the method.
HOLDING TIME:
• Samples must be analyzed within 15 minutes of collection.
METER CALIBRATION:
• The meter must be calibrated prior to the analysis of samples.
• if compliance samples are analyzed throughout the day, a post calibration check (e.g., 7 buffer) must be
analyzed mid -day and at the end of the run (i.e., roughly every4 hours based on an eight hour day).
For routine work, use a pH meter accurate and reproducible 10 0.1 pH unit with a range of 0 to 14,
equipped with a temperature compensation device. Follow all manufacturers' recommendations for the
calibration of the meter each analysis day. In all cases, the meter must be calibrated with at least two
buffers. Calibrating with the pH 4 and pH 10 buffers meets standard methods requirements and brackets
the pH range for most monitoring (with the exception of sludge) scenarios. The calibration; however,
must bracket the range of the samples being analyzed. In addition to the calibration standards, the meter
must be verified with a third calibration standard (e.g., pH 7 buffer).
In order to meet the above -specified criteria, the standard must read within a range of pH 6.9 to 7.1 to be
acceptable.
General Information:
• Samples shall be gently stirred during measurement.
•. The electrode must be rinsed and blotted dry between measurements.
All thermometers and temperature measuring devices must be checked every 12_months against a
NIST certified or NIST traceable thermometer and the process documented even if the instrument is not
used for reporting temperature results. To check a thermometer or the temperature sensor of a meter,
read the temperature of the thermometer/meter against a NIST certified or NIST traceable thermometer
and record the two temperatures. The thermcmeter/meter readings must be less than or equal to 1°C
from the NIST certified or NIST traceable thermometer reading, The documentation must include the
serial number of the NIST certified thermometer or NIST traceable thermometer that was used in the
comparison, Also document any correction that applies on both the thermometer/meter and on a
separate sheet to be filed. (NOTE: Other certified laboratories may provide assistance in meeting this
requirement.)
DOCUMENTATION:
The following must be documented whenever sample analysis is performed.
1. Time and date of sample collection
2. Time of calibration
3. Time and date of analysis (to verify 15 minute holding time is met)
4. Collector's/analyst's name
5. Value of buffers used for calibration
6. Value obtained for check buffer (e.g., pH 7 buffer)
7. Sample location/site
B. Post calibration checks (if applicable)
9. Report all data values to the nearest 0.1 pH unit.
10. The temperature correction (even if it is zero) must be posted on the meter as well as in hard copy
format (to be retained for 6 years).

Our facility was visited by Marsha Allocco from Mooresville Office of D£NR and the option
to petition for representative outfall status was recommend. All five outfalls currently being
sampled are representative in nature to outfall 005. There have been no exceedances in required__
sampling parameters. Qualitative monitoring will continue to be conducted at each of the 5
outfalls.

PERMIT:
NC Air National Guard holds a general NPDES Storm Water Permit, NCG080607. The
permit became effective on September 1, 2002 and will expire on August 31, 2007. The permit
authorizes the permittee to discharge storm water associated with industrial activity to the surface
waters of the State or to the Municipal Separate Storm Sewer System, providing that the
permittee controls, limits and monitors the discharge as specified in the permit.
RECORDS/REPORTS:
All documents maintained by NC Air National Guard were organized and available for
inspection upon request. The documents reviewed included NC Air National Guard's Storm
Water Pollution Prevention Plan, Spill Prevention, analytical monitoring, facility inspection
records, and employee training records. The documentation maintained by NC Air National
Guard in part satisfies their NPDES permit obligations.
FACILITY SITE REVIEW:
The NC Air National Guard base is located at the Charlotte -Douglas lnferna'tional Airport
in Mecklenburg County, North Carolina, approximately five miles west of downtown Charlotte.
The base is located within the northwest quadrant of the intersection of Morris Field Drive and
Airport Drive. There are five drainage basins that contain the industrial activities of the base.
These basins slope generally from west to east.
The State of North Carolina General Permit requires coverage for all point source discharges
of storm water associated with industrial activity to the waters of the State of North Carolina. The
base is considered to have the following activities that can be classified industrial in nature:
• runoff from areas where aircraft fluid changes, mechanical repairs, parts cleaning, sanding,
refinishing, painting, and washing occur. Areas where these activities are conducted include
the Aircraft Engine Maintenance facility, aircraft maintenance hangars, Aerospace Systems
Maintenance Hangar, Fuel Cell Maintenance, and occasionally the Aircraft Parking Apron.
• runoff from areas associated with aircraft refueling operations. Areas where these activities
occur include the Petroleum, Oils, and Lubricant (POL) Facility and the Aircraft Parking
Apron.
• runoff from areas associated with aircraft deicing operations. This activity occurs (typically
during winter months) on the Aircraft Parking Apron.
• runoff from areas where Aerospace Ground Equipment (AGE) and vehicle (i.e., non -aircraft)
fluid changes, mechanical repairs, parts cleaning, sanding, refinishing, painting and washing
occur, as well as, storage of vehicles and equipment waiting for repair or maintenance, and
storage of the related materials and waste materials such as oil, fuel, batteries, tires, or oil and
fuel filters. Areas where these activities occur include AGE Maintenance facilities, Vehicle
STORMWATER:
The permit requires the permittee to develop a Storm Water Pollution Prevention Plan, as
detailed in Part 11, Section A. Stormwater Pollution Prevention Plan. Storm water pollution
prevention (e.g., retention pond, secondary containment systems, and oil/water separators)
employed at this facility appeared to be effective. Also, the facility's outdoor housekeeping
observed during the inspection appeared to be excellent. Stored materials were out of the path of
traffic and containers were closed or covered where possible. Good housekeeping procedures are
applied throughout the base.
POLLUTION PREVENTION
The permittee has successfully developed and -implemented a plan that is adequate and
innovative. Staff is required to complete training on an annual basis on proper spill response and
cleanup procedures. Training includes the completion of worksheets, as well as, the viewing of an
internet-based presentation. Documentation appeared accurate and up-to-date.

SUBJECT: Report of Diesel Fuel Spill at NC Air National Guard (New London Site)
1. At approximately 7:20 am on August 5, 2003 a spill of JP$ was discovered at our
facility. A rain event the night before on the 41h of August flushed an oil water separator
causing some product to be released to a storm water basin just north of our POL tank
farm. An estimate of 100 gallons may have been spilled. It caused a slight broken up
sheen on the water in the storm basin. This product was removed using an oil absorbent
boom. Also the oil water separator was pumped out by US Filter Recovery Services.
Attached with this letter is a copy of their manifest.
2. This was reported to your office on the 5`h of August as well as to Mecklenburg
County Environmental Protection. This report is a follow-up to our phone conversation
and your request for a log of events for the spill. If you have any questions you can

SUBJECT: Report of Diesel Fuel Spill at NC Air National Guard (Charlotte Site)
1. On the 25'h of October 2002 a diesel fuel spill occurred at our Charlotte location. The
spill occurred at approximately 1:00pm in the afternoon when a truck owned by Southern
Concrete truck a pole which caused a rupture of the saddle tank. Approximately 25
gallons of diesel fuel spilled on the ground. This fuel was cleaned.up using spill
absorbent pads. None of the fuel entered any storm drain or sanitary sewer system. Nor
did any of the fuel enter any other body of water. The fuel spill was reported by Troy
Johnson of the Charlotte Fire Department on the 251h of October.
2. This report is a follow-up to the report by phone and should serve as a log of events

SUBJECT: NPDES REQUIREMENT FOR OILIWATER SEPARATOR DISCHARGE
1. Per our conversation by phone on November 9, 2000 here is a follow-up memo summarizing our discussion
about the requirement for a NPDES permit for an oil/water separator discharge currently Iocated at our fuel tank
farm (Bldg. 39). You stated that since this area is included as an identified process discharging into outfall #5 in our
stormwater permit that this would fulfill the requirement for the NPDES permit. Under stormwater permit
#NCG080000 (Certificate of coverage NCG080607) this fuel tank farm is identified as a process with potential to
discharge into outfall 45.
2. Please advise if additional requirements are necessary due to this permitted discharge. If you have questions

In response to your renewal application for continued coverage under general permit NCG080000, the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
A new Certificate of Coverage
A copy of General Stormwater Permit NCGO80000
A copy of a Technical Bulletin for the general permit
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.
