Laura, thanks so much for sending along the information, and the "permit owner affiliation form." As I mentioned in a
voice mail to you this morning, I know that Waste Management (Shawn Carroll) submitted a "permit name/ownership
change request" form for our Fayetteville Hauling Company in a letter addressed to you, dated March 27, 2015. Our

intent in that correspondence was to simply change the responsible contact within our organization. I believe that the
form we submitted will accomplish that, but please let me know if you would prefer that we submit the "permit owner

Reference is made to your January 14, 2002 request for rescission of the subject NPDES General
Permit. No specific reason for the permit rescission was noted on the request form. However, in
accordance with your request, NPDES General Permit NCG080406 is rescinded, effective immediately.
Please be advised that this permit rescission is based wholly upon the premise that the activities
which would require general permit coverage at the site have ceased or been eliminated. Division staff
has not visited the site to obtain independent verification. Operating a treatment facility, discharging
wastewater or discharging specific types of stormwater to waters of the State without a valid NPDES
permit will subject the responsible party to a civil penalty of up to $25,000 per day. Enforcement action
will be certain for persons that have voluntarily relinquished permit coverage when, in fact, continuing
permit coverage was necessary. If, in retrospect, you feel the site still requires permit coverage, you
should notify this office immediately. Furthermore, if in the future you wish to again discharge to the
State's surface waters, you must first apply for and receive a new NPDES permit.
If the facility is in the process of being sold, you will be performing a public service if you would
inform the new or prospective owners of their potential need for NPDES permit coverage.

on August 31, 2002. The Division statT is currently in the process of wwriting this permit and is scheduled to have
the permit reissued by late summer of 2002. Once the permit is rciSSued, your facility would be eligible [or
continued coverage under the reissued permit.
In order to assure your continual coverage under the general hermit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit will he expiring, l ncloscd you will find a General Permit Coverage Renewal
Application Form. The application must be completed and returned by March 4, 2002 in order to assure continued
coverage under the general permit.
Failure to request renewal within this time period may result in it civil assessment of at least $250.00. Larger

If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit

Please take notice that this certificate of coverage is not transferable except after notice to the Division
