Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection
conducted at the subject facility on February 22, 2011, by Mr. Michael L. Parker with this Office. During
the inspection, G. L. Wilson Company was represented by Mr. Danny Millsaps.
The enclosed report should be self-explanatory. Please note the comments located in the summary
portion of the inspection report. As noted by Mr. Parker during his inspection, a Stormwater and
Pollution Prevention Plan (SPPP) had not been developed for your facility as required by Part I1, Section
A of your permit. Furthermore, qualitative monitoring has not been performed as required by Part II,
Section B. Once implemented, your facilities SPPP should be updated on an annual basis as required by
Part 11, Section A, No. 7. Such being the case, this inspection report is being issued as a Notice of
Violation for the failure to comply with the terms and conditions of your General Permit. Please be
advised that NCGS 143-215.6A(a)(2) provides for a civil penalty of not more than $25,000 per day per
violation, which may be assessed against any person who fails to act in accordance with the terms,
conditions and requirements of such Permit.

Inspection Summary:
During the inspection, it was noted that the permittee had attempted to put together a Stormwater Pollution Prevention
Plan (SPPP), however, only a few basic components of the plan existed and no qualitative monitoring had been
conducted. The person responsib#e for the SPPP had Left the company and the new facility contact was unaware of the
requirements of the Plan.
The facility's SW outfall was examined and no evidence of off -site contamination was observed. The SW travels through
a BMP prior to leaving the property.

Reference is made to your January 11, 2002 request for rescission of the subject NPDES General
Permit. No specific reason for the permit rescission was noted on the request form. However, in
accordance with your request, NPDES General Permit NCG080057 is rescinded, effective. immediately.
Please be advised that this permit rescission is based wholly upon the premise that the activities
which would require general permit coverage at the site have ceased or been eliminated. Division staff
has not visited the site to obtain independent verification. Operating a treatment facility, discharging
wastewater or discharging specific types of stormwater to waters of the State without a valid NPDES
permit will subject the responsible party to a civil penalty of up to $25,000 per day. Enforcement action
will be certain for persons that have voluntarily relinquished permit coverage when, in fact, continuing
permit coverage was necessary. If, in retrospect, you feel the site still requires permit coverage, you
should notify this office immediately. Furthermore, if in the future you wish to again discharge to the
State's surface waters, you must first apply for and receive a new NPDES permit.
If the facility is in the process of being sold, you will be performing a public service if you would
inform the new or prospective owners of their potential need for NPDES permit coverage.

to receiving waters designated as Beaver Creek in the Yadkin -Pee Dee River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts 1, II, III
and IV of General Permit No. NCG080000 as attached.
This Certificate of Coverage shall become effective May 14, 1993.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
