Newsome: Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection conducted at Johnson Concrete Company located at 205 Klumac Road, on June 8, 2012, by Ms. Barbara Sifford of this Office. The site drains to Grant Creek, Class C water located in the Yadkin River Basin. The assistance of Mr. Adam Keziah was greatly appreciated during the inspection. Accordingly, the following observations were noted during the inspection: i Stormwater Pollution Prevention Plan SPPP A stormwater pollution prevention plan was available for review. It needs to be updated after the construction of Martin Luther King Boulevard extension and the site drainage has been relocated. The plan describes the facility as a Stone, Clay, Glass and Concrete facility. Z ualitative Monitoring Qualitative monitoring was recorded and evaluated for all the outfalls. 3 Analytical Monitorin Analytical monitoring was performed by the requirements listed in NCG070000. This is the correct classification for the facility, but the facility is permitted as a ready mix facility in NCG140000. Unless the motor oil usage is 55 gallons per month averaged over the calendar year vehicle maintenance does not apply to the discharge. You should review the requirements for no exposure status if you do not have vehicle maintenance to eliminate analytical monitoring. The plant is located approximately one mile north of the intersection of Route 601 and Klumac Road_ Figure 2-1 is a location map showing the general location of the facility and general topography of the local surroundings. Figure 2-2 is an aerial photograph of the property showing the general property boundaries and adjacent structures. A site plan for the facility is provided as Figure 2-3. Overview of Facility Operations The facility consists of 5 acres, all of which are used for industrial activity. Activity at the site is classified within SIC code 3272, Concrete Products, Excluding Block and Brick. Raw materials including sand, aggregate, and Portland cement are transported to the site via truck. These materials are received in bulk and stored in silos at the north end of the property. Sand and aggregate are transferred from the silos by conveyor to a central mixer within the main manufacturing building. The materials are proportioned by weight at the mixer. Cement and fly ash are augured into the mixer from fully enclosed cement and fly ash silos. Transfer of these materials occurs within enclosed systems to minimize exposure to the environment. In the mixer, aggregate is mixed with cement, water, and additives to provide a concrete mix that is formed into concrete pipe. The concrete pipe is manufactured by a mechanized casting process. In this process, concrete is dumped into a form, where it is vibrated and compacted. The raw concrete pipes are removed from the form and transported to a kiln for curing. After the concrete pipe is cured, it is moved outside and stored until shipment. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated December 6, 1983. The stormwater permit you are receiving, NCG140000, does not permit wasting concrete, dumping excess concrete, directly into storm sewer outfalls or into waters of the state. Any facility which either dumps excess concrete or washes excess concrete into storm sewers or waters of the state will be operating in direct violation to both the terms of this permit and the North Carolina General Statutes. Such a discharge shall be considered an illegal discharge and may subject the owner to enforcement actions in accordance with North Carolina General Statutes 143-215.6A If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter.
