On February 26, 2013, Ryan Spidel from Charlotte -Mecklenburg Stormwater Services (CMSWS), conducted a
site inspection of the Concord Terrazzo Company, Inc. facility located at 10220 Old Concord Road in Charlotte,
Mecklenburg County, North Carolina. The site drains to Mallard Creek, Class C waters located in the Yadkin
River Basin. The site visit and file review revealed that the subject facility is covered by NPDES General
Stormwater Permit — NCG070000.
This inspection was conducted as part of a cooperative working agreement between Mecklenburg County and
the Division of Water Quality. in addition, the inspection was conducted on behalf Of t{ ie a�.ily U1 %il I I Nile in
compliance with the City's NPDES Permit, NCS000240, Part 11 Section H. The following observations were
noted during the Charlotte -Mecklenburg Stormwater Services inspection (please see the attached addendum
for information about your permit):
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded, and properly implemented.

At the time of inspection no industrial activity was occurring at the permitted facility and therefore no
Stormwater Pollution Prevention Plan (SPPP) had been developed. However, when aggregate is moved on to
the facility a SPPP should be developed and all requirements of the NPDES Permit should be implemented.

Qualitative monitoring has been conducted and recorded in accordance with permit requirements.

ADDENDUM
The National Pollutant Discharge Elimination System (NPDES) program was established under the federal
Clean Water Act and then delegated to the Division of Water Quality for implementation in North Carolina. The
NPDES permitting program for stormwater discharges was established in 1990. Phase I focuses on site and
operations planning to reduce pollutant sources.
Through the NPDES permitting program industrial facilities that fall into one of the subject ten categories are
required to obtain permit coverage under a general permit or an individual permit, depending upon the
facilities SIC code and the industrial activity occurring at the facility. There are currently 19 active general
stormwater permits in North Carolina. One condition that is applicable to both the general permits and
individual stormwater permits is the requirement to develop and implement site -specific comprehensive
Stormwater Pollution Prevention plans (SPPP). These plans are required to include a comprehensive
evaluation of the site and operations to reduce pollutant sources and prevent pollutant discharge.

discharge monitoring forms, and a Notice of Intent [which is the application for coverage under a NPDES
General Stormwater Permit] can be found on this website.
The following items are a subset of permit requirements and are listed to assist you and help bring your
attention to certain permitting issues, process and condition requirements of your permit. Please see the
above mentioned website to obtain a copy of the permit so you may fully comply with all the conditions therein.
Failure to properly comply with conditions of the -permit, like those bulleted below, is a violation(s) of
the permit and subject to civil penalty assessment(s) of up to $25,000.00 per day.
• Industrial facilities that fall into one of the subject ten categories are required to obtain permitcoverage
under a general NPDES stormwater permit or an individual NPDES stormwater permit
All persons desiring to have facilities covered by "a General Permit must register with the Division of
Water Quality (DWQ) by filing of a Notice of Intent (NOI) and Applicable Fees.
• This Notice of Intent (NOI) must be submitted and a Certificate of Coverage issued prior to any
discharge of stormwater associated with the industrial activity, process wastewater, and/or mine
dewatering.
• Once the Notice of Intent is received, DWQ will review and determine whether a Certificate of Coverage
(COC) may be issued. If this Certificate of Coverage is issued the facility is covered by the General
Permit. That is, the permittee is authorized to discharge stormwater, process wastewater, and/or mine
dewatering pursuant to the type of General Permit until the permit expires or is modified or revoked.
• Any other point -source discharge to surface waters of the state is prohibited unless it is an allowable
non-stormwater discharge or is covered by another permit, authorization, or approval.
There are currently 19 active general permits. Permit conditions vary accordingly; however, most of the
permits have the following condition requirements (you must read your entire permit to ensure full
compliance):

o Collection, reporting, and recording of Qualitative Monitoring
o Collection, reporting, and recording of Vehicle Maintenance Monitoring (if applicable)
o Records -- Visual monitoring shall be documented and records maintained at the facility along
with the Stormwater Pollution Prevention Plan. Copies of the analytical monitoring results shall
also be maintained on site. The Permittee shall retain records of all monitoring information,
including all calibration and maintenance records and all original strip -chart recordings for
continuous monitoring instrumentation, and copies of all reports required by this general permit
for a period of at least 5 years from the date of the sample measurement, report, or application.
o Expiration — The permittee is not authorized to discharge after the expiration date of the general
permit. In order to -receive automatic authorization to discharge beyond the expiration date, the
permittee shall submit forms and fees as are required by the agency authorized to issue permits
no later than 180-days prior to the expiration date. Any permittee that has not requested
renewal at least 180 days prior to expiration, or any permittee that does not have a permit after
the expiration and has not requested renewal at least 180 days prior to expiration, will be
subject to enforcement. Failure to renew your permit is a violation.
o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is not
transferable to any person expect after notice to and approval by the Director. The. Director may
require modification or revocation and reissuance of the Certificate of Coverage to change the
name and incorporate such other requirements as may be necessary under the Clean Water
Act. The Permit is NOT transferable.
o Name Change, Closure, or Facility Ownership change -- Permittee is required to notify the
Division in writing in the event the permitted facility is sold or closed. The Division of Water
Quality views changes of name or ownership as a modification that requires the Directors
approval and reissuance of the permit. Name and ownership changes require you to complete a
Name/Ownership Change Form from the Division and mail it'back to the Division of Water
Quality along with all appropriate information.
o Proper Operation and Maintenance is a requirement of the permit.

are forwarding herewith the subject certificate of coverage to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina

Based on what I see in the geoportal, there is no flow to Back Creek from the property.
And it makes sense that everything west of the railroad bed flows to the west toward Mallard Creek.

I am confident that the portion of the property that is to be used for the project (and that was used for
the ready mix operation) drains to the storm sewer. It would appear, though, from the lay of the
land on the north side of Old Concord Rd., that the storm drainage may be carried to a UT of Mallard
Creek. Of course, I would have to get with the City in order to verify that. It appears that the unused
portion of the property, between the old plant site and the railroad mainline, may drain to Back Creek.
Do I need to contact the City and see if they can tell us where the storm sewer on this end of Old
Concord Rd. outfalls??

We've received an NOI from Concord Terrazzo in Charlotte (Mecklenburg County) for coverage of their
stone crushing facility under.NCG07 (stone, clay and glass permit). The site discharges stormwater into
the MS4 and then into what I am assuming is Black Creek (C) through settling basins. The NOI is
attached.
Does the Mooresville Regional Office have any concerns about issuing this facility a COC for this general
permit? If we don't receive any objections, we'll issue the COC in 30 days.

On behalf of Concord Terrazzo, Inc, we are pleased to submit an NOI for coverage
under General Stormwater Permit NCG070000 for a very small proposed stone screening
and crushing operation that they wish to install on company owned property located at
10220 Old Concord Rd. which was formerly occupied by a ready -mixed concrete
operation.
The purpose of the operation would be to screen and crush 89M stone products
(basically 1/2" top size, though only a small percentage of 1/2"x3/8" is allowable) obtained
from local aggregate producers into the more exact sizes, as well as the quantities of
these sizes, needed for their terrazzo tile flooring operations. The plant would have a
maximum throughput of about 20 tons per hour with a crushing capacity of only about 5
tons per hour. Basic sizes to be produced would be 3/8¢x1/4`, 1/4"x1/8A, 1/80x1/16", and a
sand product that is-1116". Currently most of these products are dolomitic limestone;
however, some granite products may be crushed. In the future, an attempt may be made
to crush some recycled glass.
The plant would be fed by a front end loader (expected size to be about a 2'/2 cubic
yard machine) which would load imported material into a +/-304on hopper. A small
variable speed belt feeder would remove material from the hopper and place it onto a 24"
conveyor feeding a 5'x10', 2-deck, scalping screen. Material that passes through this
screen would be fed directly onto a 5'x10', 3-deck, sizing screen for screening into the
product sizes required. Oversized material from the scalping screen would be transferred
by a 24" conveyor to a 10-15 ton hopper. A second small variable speed belt feeder would
feed the material from this hopper into a small 10HP roll crusher (actual size to be
determined) for size reduction. Material that has passed through the crusher would be
carried by conveyor back to the 24" conveyor feeding the scalping screen. The four
products from the sizing screen would drop into approximately 1.5 — 2 ton steel containers
for direct transport to offsite tile production facilities or to be moved inside a proposed
prefabricated metal building on this site for dry storage and/or filling 14on bags for
transport to the tile production facilities. The proposed screening/crushing plant and
40'x100' metal building are shown on the attached General Site Plan drawing.
