Compliance Evaluation Reason for Visit: Routine Inspection Summary: Stormwater Pollution Prevention Plan (SPPP) at the concrete block plant facility of Johnson Concrete Company - Piedmont Block Division, was not adhering to the following requirements of the General Permit (GP) NCG070000: 1. As required by Part II, Section A., 6. Employee Training of the GP, training programs shall be developed and training privided at a minimum on an annual basis for facility personnel ... The facility personnel responsible for implementing the training shall be identified, and their annual training shall be documented by the signature of each employee trained. Annual employee training has not been documented with signature of each employee trained at this facility, The last employee training update was on March 17, 2014. 2. As required by Part II, Section A., S. SPPP Amendment and Annual Update of the GP, all aspects of the SPPP shall be reviewed and updated on an annual basis by the permittee. The permittee has not reviewed and updated the SPPP since June 14, 2014, 3. As required by Part II, Section C. The purpose of this site visit was to conduct a multimedia inspection for air and water quality. This facility manufactures concrete blocks for the construction industry. This company operates two shifts from 6 a.m. to 4 p.m. and 4 p.m. to 2 a.m., 10 hours per shift, 5 days per week, 50 weeks per year. Mr. Dale Ludwick, Plant Manager, accompanied me during this inspection. Facility Contact Information: During the inspection I verified the facility contact information in IBEAM with Mr. Dale Ludwick, plant manager. I changed the title of Mr. Dale_ Ludwick to plant manager in IBEAM. Compliance history file review: No problems have been noted in the last five years by DAQ prior to this inspection. From the mixer, the concrete mixture is conveyed into the Besser Super Pac block machine. Columbia central mixer is not connected to the bagfilter or any other filter system. Columbia central mixer and Besser Super Pac block machine lare located inside of the concrete block plant. !Observed: Columbia central mixer (IDNo. ES-4) was in operation during the inspection. I observed no (visible emissions from the Columbia -central mixer that is located inside of the production building. Cement and flyash weigh batch is vented ito the Columbia central mixer (ID No. ES-4). Both the cement and flyash weigh batcher and Columbia central mixer are located inside of the building. Cement and flyash weigh batcher is not connected to the bag -filter (ID No. CD-1) or any other filter system. iObserved: Cement and flyash weigh batcher (ID No. ES-3) was in operation during the inspection. From the silo, the crushed blocks are conveyed to the aggregate batch hopper (ID No. I-ES-6; referred to as the aggregate weigh batch hopper). Then the crushed blocks are conveyed to the aggregate holding hopper above the Columbia central mixer (ID No. ES-4). 3 Observed: I observed that the block crusher was not in operation during the inspection. From each of the three bins, these materials are conveyed via three aggregate belt conveyors into the aggregate batch hopper (ID No. I-ES-6; referred to as the aggregate weigh batch -hopper) and then conveyed to the aggregate holding hopper above the Columbia central mixer (ID No. ES-4). Observed: Three compartment aggregate storage bins (ID No. I-ES-5) were in operation during the inspection. I observed no visible emissions from the three compartment aggregate storage bin. This belt conveyor is located inside of the concrete block plant. This company refers to this belt conveyor as ID No. 9. Observed: Belt conveyor (ID -No. I-ES-19 or 9) was in operation during the inspection, l observed no visible emissions from the belt conveyor that is located inside of the concrete block plant. I-ES-20 - material recycle silo (50 Rv)(E)(i) 0102tons capacity) FNo I Yes Mr. Ludwick stated that the block crusher is no longer being used by this facility, since no waste concrete blocks are used in the making of concrete blocks. If this company resumes the crushing process, then the waste concrete blocks are crushed and then conveyed to the material recycle silo (I-ES- 20; 50 ton capacity; orange silo). From the silo, the crushed blocks are conveyed to the aggregate batch hopper (ID No. I-ES-6; referred to as the aggregate weigh batch hopper). Then the crushed blocks are conveyed to the aggregate holding hopper above the Columbia central mixer (ID No. ES-4). Observed: I observed that the material recycle silo was not in operation during the inspection. Johnson Concrete Company, Piedmont Block Division June 9, 2016 Page 7 6. Observations of air emission sources and control devices not listed on the current permit: a. One natural gas -fired direct -fired steam generator (on front plate: Johnson Cure Pak, direct fired; on side plate: manufactured 2006; model # SP5000; serial 42006EV; natural gas -fired; and input, 5 million Btu/hr; and listed previously listed as I-ES-8 in R06) provides steam for curing the "green." concrete blocks in twelve kilns from sixteen to eighteen hours (one cycle) at temperatures ranging from 140 (usually set at this temperature) to no more than 165 degrees Fahrenheit. This steam generator operates ten hours on natural gas. The steam is introduced for a period of two or three hours and then the concrete blocks have a "humid soak period". The concrete blocks are finally removed from these kilns after a twenty-four hour period. The steam from the generator is vented to the twelve kilns and then the return heat from the twelve kilns exhausts through a single stack on the roof to the outdoor atmosphere. The doors of the twelve kilns also exhausts through a fan on back side of this plant to the outdoor atmosphere. The natural gas -fired steam generator exhausts inside a room that is separate from the concrete block plant. The natural gas -fired steam generator is exempt per 15A NCAC 2Q .0102 (h)(1)(B). During this inspection, the natural gas -fired steam generator and nine steam -heated kilns were in operation. I observed no visible emissions from these sources. b. One Besser Super Pac block machine is used to form the concrete blocks. This block machine is located inside the concrete block plant and exhausts inside of this facility. During this inspection", the block machine was in operation. I observed no visible emissions from this source. C. This company has one 1,000 gallon propane tank at this facility. This propane will be sold as fuel to various customers. This tank is exempt per 15A NCAC 2Q .0102 (g)(4). d. One 275 gallon aboveground tank is used to store the used oil at this facility. This tank is located next to the 500 gallon abovegound diesel fuel storage tank (ID No. I-ES-9). This tank is exempt per 15A NCAC 2Q .0102 (g)(4). Aerosol spray cans containing blue and orange paints are used to label the block. This company uses approximately 600 cans a year of blue paint and 300 cans a year of orange paint (12 oz can; manufacturer, Rust-Oleum; product, gloss protective enamel). The spray painting process should be evaluated during the next permit revision. f. Various admixtures (manufacturer: Euclid Chemical Company) for efflorescence and water repellency are used in the making of the block. Compliance with specific permit conditions and limitations: a. Condition A. 2. Permit Renewal and Emission Inventory Requirement states that at least 90 days prior to the expiration date of this permit, the permittee shall request permit renewal by letter with AA application form and submit the air pollution emission inventory report with certification sheet for 2015 calendar year to MRO DAQ. Observed. The permit was issued on April 13, 2012 and expires on March 31, 2017. Compliance with this stipulation is indicated. Particulate Control Requirement as required by 15A NCAC 2D .0515 "Particulates from Miscellaneous Industrial Processes", states that particulate matter emissions from the emission sources shall not exceed allowable emission rates. Observed. Cement and flyash storage silos are being controlled by a bagfilter. The allowable emission rates from these sources are not being exceeded. Compliance with this stipulation was indicated during the permit application process. C. Condition A. 4. Notification Requirement as required by 15A NCAC 2D .0535 "Excess Emissions Report & Malfunctions" states the permittee of a source of excess emissions that last for more than four hours and that results from a malfunction, a breakdown of process or control equipment or any other abnormal conditions, shall notify the Director or his designee of any such occurrence by 9:00 a.m. Eastern time of the Division's next business day of becoming aware of the occurrence, Observed. Observed. MRO DAQ has not received any fugitive dust emissions complaints regarding this facility. This company has unpaved areas at this facility Mr. Ludwick stated that a forklift with a tank on it could be used to wet the unpaved areas at this facility. During the inspection, I observed no fugitive or visible emissions from this facility. Compliance with this stipulation is indicated. f. Condition A. 7. Fabric Filter Requirements including cartridge filters, baghouses, and other dry filter particulate collection devices as required by 15A NCAC 2D .0611 "Monitoring, Recordkeeping and Reporting" states that the permittee shall perform, at a minimum, an annual internal inspection of the filter system within 12 month period following the initial inspection. In addition, the permittee shall perform periodic inspections and maintenance as recommended by the equipment manufacturer and list corrections made and dates of actions in a filter logbook (in written or electronic format). Filter logbook kept on -site and made available to DAQ personnel upon request. Observed. The logbook provided by Mr. Ludwick indicated that inspection and maintenance activities of the bagfilter (ID No. CD-1) are conducted annually or as needed Johnson Concrete Company,. GACT/MACT Review: This facility has no engines/generators, fire pumps or any other sources that are subject to GACT or MACT. The natural gas -fired steam generator (boiler) is not subject to the National Emission Standards for Hazardous Air Pollutants (NESHAP) for Industrial, Commercial, and Institutional Boilers Area Sources (40 CFR Part 63, Subpart JJJJJJ or 6J), since this rule does not apply to a boiler that is gas -fired. Summary of changes needed to the current permit: a.
