conducted at the subject facility on April 4, 2012, by Mr. Michael L. Parker with this Office.
The enclosed report should be self-explanatory. As noted by Mr. Parker during the inspection,
there are specific sections of your Stormwater Pollution Prevention Plan (SPPP) that need to be addressed
and/or updated. These areas were discussed with you during the inspection and are also noted in the
Summary Section in the attached inspection report. As a reminder, please note that your SPPP should be
updated on an annual basis as required by Part H, Section A, No. 7 of your permit.

Inspection Summary:
The Stormwater Pollution Prevention Plan (SPPP) at this facility was generally well maintained and up-to-date with
respect to the requirements of the General Permit (GP). There were a few minor deficiencies noted and these are
detailed below.
1. The general location map should include the latitude and longitude of the points of discharge as required by Part II,
Section A, No. 1(a) of the GP.
2. The existing BMP did not include a description of the existing SW treatment structure (Stormceptor) as required by
Part ll, Section A, No. 2(c) of the GP.
3. The SPPP should identify the specific position responsible for the SPPP as by Part ll, Section A, No. 6 of the GP.
4. The SPPP should be reviewed and updated annually as required by Part II, Section A, No. 7 of the GP.
The inspector was only able to review one (1) Qualitative monitoring report during the inspection. Previously qualitative
monitoring had been performed at this facility by an employee that was no longer at that location and the current facility
contact was unable to locate the records from prior qualitative monitoring events. It should be noted that all qualitative
monitoring should be performed at the location and frequency as defined in the GP.
The existing site (estimated to be over 10+ acres) is almost entirely graveled and borders drainage features on two sides.
At the rear of the property, there is an existing SW outfall that passess off -site through an adjecent property and then
discharges directly into surface waters. The site is generally flat and there are several points along the perimeter of the
property where SW outlets to drainage features or headwater streams via eroded cuts in the bank and ground swales.
There was evidence where fines associated with runoff from the graveled parking area had been transported to these SW
outlets and where deposition of fines had begun to build up in the drainage features. The company had installed a device
called a Stormceptor to intercept some of the SW flow and removed sediment particles and any oil that might escape the
manufacturing operation, however, there were many outlets where SW left the property untreated. Many of these outlets
were in close proximity to the company's storage yard where new product was stored awaiting sale. It was recommended
that the company look at their existing storage practices and see if some type of vegetative buffer could be established on
the perimeter bf the property so as to filter these fines before they migrate off -site. Something as simple as re4ocating the
existing product to allow for the establishment of a vegetative barrier to restrict the off -site transport of gravel fines could
provide a significant benefit to SW discharge quality. Berms and/or other structural devices should also be considered as
a way to divert SW away from areas of potential pollutant sources as noted in Part ll, Section A, No. 2(c) of the GP.

Your facility is currently covered for stormwater discharge under General Permit NCG070000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional
information contact the Central Office Stormwater Staff member listed below or check the Stormwater & General
Permits Unit Web Site at http://h2o.enr.state.nc.us/su/storrnwater.htmi
if the subject stormwater discharge.to waters of the state has been terminated, please complete the enclosed
