Inspection Summary:
On March 8, 2018, a routine stormwater inspection was performed. The following aspects of the Stormwater Pollution and
Prevention Plan (SPPP) were missing:
- The updated site map to include outfall locations.
- Direction of flow in each drainage area and best management practices.
- The updated annual employee training, and the responsible parties for implementing the training
along with the signature of each employee trained.
- The list of significant spills or leaks for the previous three years, or the notation that no
spills have occurred needs to be recorded on the plan.
- Pallets stored outside need to be removed, and all outside storage should be covered or removed.
The permittee has the option of applying for a No Exposure permit for this facility (see attachment). If you are planning on
applying for the no exposure, pallets stored outside need to be removed, and all outside storage need to be covered or
removed.

Enclosed please find a copy of the Compliance Evaluation Inspection Report for the inspection conducted at the
subject facility on July 29, 2011, by Ms. Donna Hood of this Office.
This report is being issued as a Notice of Violation (NOV) because of the violations of the subject NPDES
permit and North Carolina General Statute (G.S.) 143-215.1 as detailed in the Stormwater Pollution Prevention
Plan, Qualitative Monitoring, and Permit and Outfalls Sections of the attached report. Pursuant to G.S. 143-
215.6A, a civil penalty of not more than twenty five -thousand dollars (25,000.00) per violation per day may be
assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of
any pen -nit issued pursuant to G.S. 143-215.1.
It is requested that a written response to the inspection be submitted to the Division of Water
Quality/Mooresville Regional Office discussing whether the permit will be rescinded by "September 7, 2011. In
responding, please address your comments to the attention of Ms. Marcia Aliocco.

1. SITE PLAN AND CERTIFICATION
a. General Location Map — Figure 1.
b. Storage, Loading, Unloading, Manufacturing, and Waste Disposal Practices.
The facility receives and ships all raw materials in bags, bins, boxes, drums or other small
containers. Nothing is currently received or shipped in bulk quantities. All loading,
unloading, and storage is performed under cover in these smaller containers and therefore
is not prone to contribute to stormwater pollution.
All manufacturing operations are performed undercover with suitable dust collection
devices on powder processes and therefore are also not prone to contribute to stormwater
pollution. Wash water from ceramic mixing operations is collected and then processed
through settling tanks and a fabric type filter press. After the solids are removed, wash
water is discharged to a tank and the water re -used. This process water is therefore not
prone to contribute to stormwater pollution.
Manufacturing waste (fired and unfired ceramic articles, filter cake, dust collector fines, and
other manufacturing wastes) is disposed of in a large open dumpster at the rear dock of the
plant. This dumpster is covered and therefore heavy rains will not wash any of this waste
into the parking lot and will not contribute to stormwater pollution.
Waste oil is placed in closed drums and stored in a covered containment dike at the south
end of the plant. It is held for collection and disposal by a professional waste disposal
contractor. This assures that oils are not exposed to stormwater and reduces the risk of
stormwater pollution.
c. Site Map — Figure 2.
d. Significant Spills in the last 3 years -- None.
e. Certification of Inspection of stormwater outfalls.
The following Stormwater outfalls have been evaluated for the presence of non-stormwater
discharges: A natural spring is capped and discharges to the stormwater ditch in front of the
plant along Stitt Street.
2. - STORMWATER MANAGEMENT PLAN
a. Feasibility Study.
Currently, the facility maintains nearly all material operations and storage under cover to
reduce the chance of stormwater contamination due to direct contact of rain water. Of the
operations exposed to stormwater the following review is presented:
To eliminate ceramic washout from the discharge of the Cutting Dust Collector, it is feasible
to raise the barrel up out of the water drainage path and construct a covered area for the
barrel.
b. Secondary Containment Schedule.
The facility maintains used or waste oil in drums which are surrounded by a containment
wall of cement block. The containment area is covered to prevent any water entry. The
primary method for any stormwater removal from the containment area is evaporation. If

evaporation does not suffice to dry the containment area, stormwater in the containment
area is to be drained ONLY after the water has been visually observed for absence of color,
foam, staining, and a high degree of sheen indicating high levels of oil in the water. Any time
that the containment area must be drained, the individual making the observations must
not them in the appropriate log sheet (Figure 3). These records must be maintained in the
master stormwater file for a minimum of 5 years.
c. BMP Summary --- none.
3. SPILL PREVENTION AND RESPONSE PLAN
Liquids are to be placed ONLY in clearly labeled containers. Liquid containers are to be handled
with care. Only approved lifting devices in good operating condition are to be used to move, lift,
or dump liquid materials. Before moving any liquid container, it must closed completely by
replacing plugs, bungs, or lids and fastening securely. The extra effort required to properly
handle liquids and avoid spills is well spent in avoiding a costly spill clean up. Liquid containers
are to be transported via an indoor route if at all possible to avoid potential stormwater
contamination. If a spill occurs, the material shall be handled and disposed as directed on the
MSDS. If a spill occurs, the following actions will be taken:
a. Containment.
If it is safe to do so, stop the flow of liquid from the container first. The spill area should
immediately be surrounded with absorbent material to reduce the spread of the spill. Loose
Sawdust (for smaller spills) or bags of clay material may be used to build dams around the
spill. Take precautions to keep material from entering storm drains by building dams
completely around the drain(s).
b. Notification.
Once the spill has been contained, immediately notify the Plant Manager of the material
and the quantity spilled. The Plant Manager will help to direct efforts through the cleanup of
the spill. The Plant Manager will also be responsible for notifying Monroe City authorities
immediately if the spill has reached the storm drains, and the NC DEHNR within 24 hours
verbally, and in writing within 5 days:
c. Removal.
Based on the information on the MSDS, the spilled material should be collected and
disposed. Absorbent materials such as sawdust and clay may be used to help collect
materials for disposal.
d. Clean up.
Any residue of the spill that may remain must be finished cleaning to a point that
stormwater will not be significantly contaminated. Special cleaning agents or methods may
be required for some materials.

4. PREVENTATIVE MAINTENANCE AND GOOD HOUSEKEEPING
For stormwater prevention, the following general tasks shall be performed.

These inspections shall be logged on the appropriate log sheets (Figure 3) and maintained in the
master stormwater file for a minimum of 5 years.
5. EMPLOYEE TRAINING
All supervisors, maintenance, and mixing operators are to be re-trained annually in spill
response and cleanup procedures. This training is to be documented by the HR Director. The
Plant Manager is responsible for coordinating this training.
6. RESPONSIBLE PARTY
The Plant Manager is responsible for the overall coordination, development, implementation
and revisions to the plan.
7. PLAN AMENDMENT -
This plan should be amended whenever changes in the operation require additional procedures
to prevent contamination of stormwater. The plan will be reviewed and updated on an annual
basis by the Plant Manager. This review will be logged in the main facility inspection log (Figure
4).
There are currently no illicit (Non-stormwater) discharges to the stormwater system. This will
be reviewed and logged annually with the plan review.
8. FACILITY INSPECTION
The stormwater systems of the facility should be inspected at a minimum of twice a year, once
in April —June and once in September- November. The Inspection should be logged on the main
facility inspection log (Figure 4) and records are to be maintained in the master stormwater file
for a minimum of 5 years.
Qualitative monitoring of the stormwater will be done and logged during all facility inspections.

Dear Mr. Crytzer:
Enclosed please find a copy of the Compliance Evaluation Inspection Report for the inspection conducted at the
subject facility on July 29, 2011, by Ms. Donna Hood of this Office.
This report is being issued as a Notice of Violation (NOV) because of the violations of the subject NPDES
permit and North Carolina General Statute (G.S.) 143-215.1 as detailed in the Stormwater Pollution Prevention
Plan, Qualitative Monitoring, and Pertnit and Outfalls Sections of the attached report. Pursuant to G.S. 143-
215.6A, a civil penalty of not more than twenty five -thousand dollars (25,000.00) per violation per day may be
assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of
any permit issued pursuant to G.S. 143-215.1.
It is requested that a written response to the inspection be submitted to the Division of Water
Quality/Mooresville Regional Office discussing whether the permit will be. rescinded by September 7, 2011. In
responding, please address your comments to the attention of Ms. Marcia Allocco.

Your facility is currently covered for stormwater discharge under General Permit NCGO70000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 11 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.

continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the l 1 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.

DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.
