Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection conducted at
the subject facility on February 23, 2011, by Ms. Barbara Sifford of this Office. The site drains into an
unnamed tributary of Draft Branch to Grants Creek in the Yadkin River Basin. The site visit and file review
revealed that the subject facility is covered by NPDES Stormwater General Permit NCG070000. The assistance
of Mr. David Frame and Mr. Chris Blackwell and yourself was greatly appreciated during the inspection.
Accordingly; the following observations were noted during the inspection:

section would help insure continued compliance.

3) Other Observations
The outfalls from the property were observed and no visible evidence of solids appeared to be leaving
the property. The sediment should be removed regularly from the drainage basin to prevent washing out
to the RR tracks_ The vegetation on the slope below the storage area should be replaced with grass and
not the shrubs since those require a wet stream bank area for survival.
The report should be self-explanatory. Please be advised that violations of the \PDES General Permit are
subject to a civil penalty assessment of up to $25,000.00 per day for each violation. If you have any questions,
comments, or need assistance with understanding any aspect of your permit, please do not hesitate to contact

Division of Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15,
2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Pen -nit NCG070000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• . Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligation' s.with respect.to
stormwater discharge controls, management, monitoring, and record keeping. Please review the new permit to
familiarize yourself with all the changes in the reissued permit.
The more significant changes in the General Permit since your Iast Certificate of Coverage include the following:
• Permif cover page — Coverage is expanded to include industrial activities similar to the categorically identified
industries.
• Part I Section A — This part explains that the No o Exposure Exclusion from permitting option may be available to
the permittee.

• Part II Section A — Several minor expansions and clarifications of the required content of the Stormwater
Pollution Prevention Plan have been added. Please be sure that your plan is updated to comply with the new
requirements.
• Part II Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part It Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part II Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil
per month, new provisions require that discharges from the vehicle maintenance areas be sampled twice per year,
and that the sampling results be compared to new numerical benchmark values. (The previous cutoff
concentrations have been eliminated.)
• Part II Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil
per month, new provisions require the permittee to execute Tier One and Tier Two response actions, based on the
first benchmark exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two
requires that the permittee institute monthly monitoring instead of twice -per -year monitoring, until three
consecutive monitoring events show no benchmark exceedences.
• Part II Section C — This part contains a new provision that four exceedences of any particular benchmark will
trigger increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may
direct the permittee to apply for an individual permit, or may direct the implementation or installation of specific
stormwater control measures.
Your coverage under the General Permit is transferable only through the specific action of DWQ.
This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does
it relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule,
standard, ordinance, order, judgment, or decree.

Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the
inspection conducted at the subject facility on April 30, 2008, by Mr. Michael L. Parker with this
Office.
The enclosed report should be self-explanatory. Please note the comments located in the
summary portion of the inspection report. As noted by Mr. Parker during his inspection, a
Stormwater and Pollution Prevention Plan (SPPP) has not been developed for your facility as
required by Part 11, Section A of your permit. Furthermore, qualitative monitoring has not been
performed as required by Part 11, Section B. Once implemented, your facilities SPPP should be
updated on an annual basis as required by Part II, Section A, No. 7. Such being the case, this
inspection report is being issued as a Notice of Violation for the failure to comply with the terms
and conditions of the subject General Permit. Please be advised that NCGS 143-215.6A(a)(2)
provides for a civil penalty of not more than $25,000 per day per violation, which may be
assessed against any person who fails to act in accordance with the terms, conditions and
requirements of such Permit.

Your facility is currently covered for stormwater discharge under General Permit NCG070000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Forms. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your "
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 1 i categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional
information contact the Central Office Stormwater Staff member listed below or check the Stormwater & General

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
