In accordance with your application for a discharge permit received on December 15. 2010, we
are forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES
general permit. This permit is issued pursuant to the requirements of worth Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of "later Quality or permits required by the Division of Land Resources, Coastal
Area Management Act or any other federal or local governmental permit that may be required.

Dear Mr. Hendrix:
Enclosed is a copy of the Compliance Evaluation inspection Report for the inspection
conducted at the subject facility on November 5, 2010 by Mr. Wes Bell of this Office. Please
inform the facility'; Operatoran-Responsible Charge of our findings by forwarding a copy of the
enclosed report.
This Office has determined that the facility will need to be covered under the General
Stormwater Permit No. NCG060000 due the on -site industrial activity (General Warehousing)
and the stormwater discharge into waters of the State. Potential exposures to the stormwater
runoff as a result of the facility's industrial activities wete observed during this inspection. A
completed permit application and supporting documentation (including fee) should be mailed to
the attention of Bradley Bennett, Supervisor, DWQ/Stormwater Permitting Unit, 1617 Mail
Service Center, Raleigh, N.C. 27699-1617. The permit application and supporting information
can be found at the following website address:

Please note that facilities with stormwater discharges "associated with industrial activity"
may be excluded from permit coverage if industrial materials and operations are not exposed to
stormwater. The facility must maintain this condition of no exposure to qualify for the
pern3.itting exclusion. If the condition changes, the facility must seek permit coverage for
industrial stormwater discharge under the general permit. The Facility.is responsible for self re-
certifying annually that the no exposure condition is being maintained. The N.C. No Exposure
Application form can also be found at the above -noted website address and should be mailed to
the address listed on the last page of the application.
