This letter requests permission for outfall 7 to represent outfalls 1, 2, 3, 4, 5, and 6, outfall 9 to represent outfall 8, and outfall 10 to represent outfall 11. The Mooresville Regional Office staff inspected the subject facility on September 28, 2016 and met with Mr. Chris Harrington, the Complex Environmental Manager. During the site visit we viewed all outfalis on site and determined that outfalls 7, 9, and 10 most effectively captured stormwater representative of all activities on site. Please use these outfalis as sampling points for analytical monitoring per the permit requirements. Please retain a copy of this letter in the Stormwater Pollution Prevention Plan for your records. And just to put on paper what we discussed. The city would like for you to run your pump, which is set up to automatically pump the discharge from your storm water system of this one outfall, for the first 2 hours of storm discharge into your sanitary treatment system. From there, the discharge from your treatment system would go into the city sanitary sewer system. Your question was if there was any problem according to your coverage under NCG060000 concerning sampling. After discussing the situation with Ken Pickle, of our office, we decided that NCDEMLR did not have any problem with that proposal. The following is a summary of possible sampling: • If during the monitoring period, you have no stormwater discharge from this outfall because of using the above method, then just record "No flow" on your DMR. • If during a monitoring period, you still have a discharge after the 2 hour pumping, then sample that discharge and flow normal sampling procedure. • Or if the automatic pump does not work for some reason during discharge event, then follow normal sampling protocol. Let me know if anything is different than discussed or if you have any further questions. I will include a copy of this email in the file. This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended). The following information is included with your permit package: • A new Certificate of Coverage (COC) • A copy of General Permit NCG060000 • A copy of the Technical Bulletin for the General Permit • Two copies of the Discharge Monitoring Report (DMR) Form • Two copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all the changes in the reissued permit. Your facility has six (6) months from the time of receipt of the permit to update your current SPPP to reflect all new permit requirements. The first sample period of your permit begins January 1, 2013. Your facility must sample a "measureable storm event" beginning during the periods beginning January 1 and July 1 of every year (or, if applicable, report "No Flow," as outlined in Part III, Section E). Also, please note that Tier 3 Actions in Part 11 of your permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this permit and do not count prior exceedances. On Friday, April 20, 2007 (approximately 7:30 am), a check valve that connects the hatchery drain and line from the processing plant going to the wastewater treatment plant failed. This caused the sump to overflow into a truck bay at the Hatchery. Some of the water flowed outside the truck bay across the parking lot and into a grassy area on the Hatchery parking lot. Approximately 50 gallons did flow into a storm water drain which leads to an unnamed tributary of Richardson Creek. The storm water drained was covered as soon as it was realized that water was flowing into it. All flow was stopped and contained by 8:30 am. A pH reading was taken of the water in the affected area of the tributary. The pH was 7.2. The affected area of the tributary did not show any evidence of fish kill. The affected area of the tributary was contained and cleaned at this time also. Ms. Marcia Allocco (NCDENR — Surface Water Protection) was contacted and notified of the incident at approximately 8:30 am. The water in the sump was pumped to the wastewater treatment plant and the check valve was replaced. An inflow cover for the grate style manhole cover is being ordered. This should prevent a majority of the water from entering the unnamed tributary should a similar situation occur. Most of the water would flow into the grassy area on the Hatchery property. The hatchery employees have also been retrained to contact management should there be an alarm at the sump indicating a high level condition. They have also been instructed to berm the storm water drain area to further prevent water from entering the drain should there be a release.
