Please see the attached revised Certificate of Coverage (COC) for Permit No. NCG050150 that reflects the new company
and facility name. Also attached is a report from our database showing these changes, as well as the contact
changes. (Note that because of the background programming, our database will not capitalize the "c" in "AC"— sorry
about that! The COC shows it correctly.)
Please verify that all the information is correct, including contact details. I noticed that the email submitted for Walter
Drew does not match the email we had in the system for Owner Affiliation, and "walter.drewPtc.tc" looked like it may
be a typo. Could you verify Mr. Drew's correct email address?
Also, with this email I am copying my contact at Mecklenburg County. (Chad, this serves as the County's copy of the
revised Certificate of Coverage for this facility.)

N.C. DWQ has reissued NPDES Stormwater General Permit NCG050000, which becomes effective June 1, 2013. Your Certificate of
Coverage (COC) is ready for renewal and download from our website. Please follow the instructions below and be sure to print your
new COC for your records when you complete the renewal step.
1.Go to the following URL link: http://Portal.ncdenr.ors/web/wq/ws/su/permit-renewal
2. Enter your COC Permit Number and unique PIN:

3. Scroll Down and update the name and e-mail address information if necessary for a renewal confirmation. NOTE: These fields will
be pre -filled with information we have on file about the Owner. "However, if the confirmation should be e-mailed to you instead
of the email in the box, please revise it accordingly." You are unable to change the information in several fields. Please contact us if
something else is incorrect.
4. Click "SUBMIT." A confirmation screen should follow.
5. On this screen, you can click the button "DISPLAY CONFIRMATION DETAILS." Clicking this button will automatically generate an e-
mail to the address you provided on the previous screen. It will also send you to a screen that will display details of your renewal.
6. IMPORTANT: PRINT YOUR COC. On this screen, you must print a copy of the Certificate of Coverage for your facility by clicking on
"Print COC"

ADDENDUM
The National Pollutant Discharge Elimination System (NPDES) program was established under the federal
Clean Water Act and then delegated to the Division of Water Quality for implementation in North Carolina. The
NPDES permitting program for stormwater discharges was established in 1990. Phase I focuses on site and
operations planning to reduce pollutant sources.
Through the NPDES permitting program industrial facilities that fall into one of the subject ten categories are
required to obtain permit coverage under a general permit or an individual permit, depending upon the
facilities SIC code and the industrial activity occurring at the facility. There are currently 19 active general
stormwater permits in North Carolina. One condition that is applicable to both the general permits and
individual stormwater permits is the requirement to develop and implement site -specific comprehensive
stormwater pollution prevention plans (SPPP). These plans are required to include a comprehensive
evaluation of the site and operations to reduce pollutant sources and prevent pollutant discharge in stormwater
runoff.

The following items are a subset of permit requirements and are listed to assist you and help bring your
attention to certain permitting issues, process, and condition requirements of your permit. Please see the
abovementioned website to obtain a copy of the permit so you may fully comply with the conditions therein.
Failure to properly comply with conditions of the permit, like those bulleted below, constitutes permit
violations and is subject to civil penalty assessment of up to $26,000.00 per day.
• Industrial facilities that fall into one of the subject ten categories are required to obtain permit coverage
under a general NPDES stormwater permit or an individual NPDES stormwater permit
• All persons desiring to have facilities covered by a General Permit must register with the Division of
Water Quality (DWQ) by the filing of a Notice of Intent (NOI) and Applicable Fees.
• This Notice of Intent (NOI) must be submitted and a Certificate of Coverage issued prior to any
discharge of stormwater associated with the industrial activity, process wastewater, and/or mine
dewatering.
• Once the Notice of Intent is received, DWQ will review and determine whether a Certificate of Coverage
(COC) may be issued_ If this Certificate of Coverage is issued the facility is covered by the General
Permit. That is, the permittee is authorized to discharge stormwater, process wastewater, and/or mine
dewatering (pursuant to the type of General Permit) until the permit expires or is modified or revoked.
• Any other point -source discharge to surface waters of the state is prohibited unless it is an allowable
non-stormwater discharge or is covered by another permit, authorization, or approval.
• There are currently 19 active general permits. Permit conditions vary accordingly; however, most of the
permits have the following condition requirements (you must read your entire permit to ensure full
compliance):

o Collection, reporting, and recording of Vehicle Maintenance Monitoring
o Records — Visual monitoring shall be documented and records maintained at the facility along
with the Stormwater Pollution Prevention Plan. Copies of the analytical monitoring results shall
also be maintained on site. The Permittee shall retain records of all monitoring information,
including all calibration and maintenance records and all original strip -chart recordings for
continuous monitoring instrumentation, and copies of all reports required by this general permit
for a period of at least 5 years from the date of the sample measurement, report, or application.
o Expiration — The permittee is not authorized to discharge after the expiration date of the general
permit. In order to receive automatic authorization to discharge beyond the expiration date, the
permittee shall submit forms and fees as are required by the agency authorized to issue permits
no later than 180-days prior to expiration date. Any permittee that has not requested renewal at
least 180 days prior to expiration, or any permittee that does not have a permit after the
expiration and has not requested renewal at least 180 days prior to expiration, will be subject to
enforcement. Failure to renew your permit is a violation.
o Transfer — The Certificate of Coverage (COC) issued pursuant to this general permit is not
transferable to any person expect after notice to and approval by the Director of the Division of
Water Quality, The Director may require modification or revocation and reissuance of the
Certificate of Coverage to change the name and incorporate such other requirements as may be
necessary under the Clean Water Act. The Permit is NOT transferable.
o Name Change, Closure, or Facility Ownership change — The permittee is required to notify the
Division in writing in the event the permitted facility is sold or closed. The Division of Water
Quality views changes of name or ownership as a modification that requires the Director's
approval and reissuance of the permit. Name and ownership changes required you to complete
a Name/Ownership Change Form from the Division and mail it back to the Division of Water
Quality along with all appropriate information.

Comment: The permittee has designated two outfails at the facility. During the
walkthrough the location of Outfall 001 was not clear. It appeared that Outfall 001 may
be the stream that starts on site. Once new ES&H staff is hired please review the
location of Outfall 001 to ensure Exopack is monitoring the quality of the stormwater
runoff from the site before it commingles with surface waters (UT to Fourmile Creek).
The illicit discharge evaluation was last completed in 2010. This review should be

i'll copy this note and place it in the SPU file folder for NCG050150.
We expect that the personnel in plant positions change pretty regularly, so this is not an unusual circumstance. When
the NCGOS mailer goes out (Jan? Feb?) there will be a place for them to correct the information so that we can update
BIMS. If they are worried that our renewal mailer won't get to them, then they should submit a Name/Ownership
change form to us. However, typically a pretty good fraction of our mailers get through to the facilities, even if the
personnel have changed and we are working with outdated addressees.
So, our sort of game plan for staving in touch with folks is this:
1. First: Folks are obligated by the permit boilerplate to send us a Name/Ownership change form when those
changes occur. However, it's a regular occurrence that some fraction don't.
2. When we renew the permit, of course we use the BIMS information that we have, whether current or outdated.
A large number of our renewal mailers get through to the facilities, even with outdated personnel addressees.
Upon receipt of the mis-addressed mailer the permittees then have a second chance to provide us with proper
mailing addresses. However, for a smaller fraction sometimes the bad address information prevents effective
delivery.
3. When there is no effective delivery, a fraction of the mailers are returned to us 'Undeliverable'; another fraction
presumably reach their destination but are trashed when no one at the facility responds. However,
4. Typically Fran still has a proper billing address for this small subset of our permittees, and we are successful
sending the bills, which presents a third opportunity for the permittees to provide us with proper addresses.
S. When Fran gets stymied by bad addresses, then every few years we can mount a collections/renewal initiative
(Like the one earlier this year) and hope to pull those folks back into proper status, if they are still in business.
6. And finally, as a sort of safety -net procedure, SPU staff can break out of this scripted approach, and try to chase
down individual permittees and get the correct information. But, typically I view that as a labor intensive, back-
up kind of approach to be employed only if there is time, and if there is a special reason to do so.

informed Ms. Nancy Summe that DWQ will send out a letter and that she does not need to initiate permit
renewal. She note that Mr. Lerch who is listed as the owner is no longer there and that the plant manager
position is currently open. I believe permitting sends out the renewal letters based on the owner so I thought I

In accordance with your request received July 2, 2002, the Division is forwarding the subject permit
modification. This modification documents die change in the ownership of the subject facility. All other terms and
conditions in the original permit remain unchanged and in full effect. This permit modification is issued under the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U. S. Environmental Protection Agency dated December 6, 1983.
This permit modification does not affect the legal requirement to obtain other permits which may be
required by the Division of Water Quality, the Division of Land Resources, Coastal Area Management Act, or any

Dear Permitice:
Your facility is currently covered for stormwater discharge under General Permit NCG050000. This permit expires
on March 31, 2003_ The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would he eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring_ Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00,
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215,1
and could result in assessments of civil penalties of up to $10.000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 1 1 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion_ For additional
information contact the Central Office Stormwater Staff member listed below or check the Stormwater & General
Permits Unit Web Site at http://h2o.enr.state.nc.us/su/stormwatcr.html
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.
