Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection
conducted at the subject facility on January 13, 2009, by Ms. Heather Sorensen of Charlotte -
Mecklenburg Storm Water Services. This inspection was conducted as part of a cooperative working
agreement between Mecklenburg County and the Division of Water Quality. In addition, the
inspection was conducted on behalf of the City of Charlotte in compliance with the City's NPDES
Permit, NCS000240, Part 11 Section H. Thank you for your assistance and cooperation during the
inspection.
Overall, the facility's Stormwater program was well developed and implemented and this
Office commends the permittee's efforts to ensure compliance with your NPDES permit.
The enclosed report should be self-explanatory; however should you have any questions
concerning this report or have any questions regarding your permit, please do not hesitate to contact

Inspection Summary:
Facility Description;
This facility is primarily involved in cutting and printing board for consumer product packaging. This facility operates five
days a week, 24 hours a day, weekends as necessary. No vehicle maintenance or washing occurs on site. Only
non -hazardous materials are handled on site and UNiVAR handles all waste disposal. Waste includes waste ink and used
oil. A vortex unit is located on the roof of the facility and handles paper trim from production, which is sent to bailers.
Compliance History:
The permitteelfacility does not have any prior compliance issues with the NC Division of Water Quality or with
Charlotte -Mecklenburg Storm Water Services (CMSWS).
Industrial Operations/City of Charlotte Stormwater Ordinance Review:
Trash Compactor —
Hydraulic fluid was observed on the ground around the trash compactor. The facility should conduct proper maintenance
on the trash compactor to prevent stormwater contamination and pollution discharges. The facility shall properly remove
and dispose of all spilled oil.

Comment: The General Location Map within the SPPP needs to be updated to include
the points of discharge and the latitude and longitude coordinates for points of
discharge. The Site Map needs to be updated to include the property boundaries,
points of discharge, site topography, and percent of each drainage area that is
impervious. The SPPP did not identify the facility personnel responsible for
implementing employee training or the Spill Prevention and Response Plan and did not
contain a feasibility study. Employee Training conducted in each of the last five years.
The annual review and update dates were listed in the SPPP but no signatures were
included s required by the permit.

Comment: Storm Drain Outfall to Granite Street —
One of the three drains that discharge to the outfall was approximately 45% blocked
with sediment.
Storm Drain Outfall to Railroad Tracks —
This storm drain outfall was 98% blocked with sediment. Staff commented that the City
of Charlotte has been contacted about cleaning out the sediment and debris from the
ditch where this outfall is located.
The permittee is required by Part II, Section A, Item #4 and Part 11, Section C, Item #1 to
properly operate and maintain all stormwater facilities (drains, pipes, etc.) that are under
their control.

Your facility is currently covered for stormwater discharge under General Permit NCG050000. This permit expires
on March 31, 2003- The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form- The application must he completed and returned by October 2. 2002 in order to assure
continued covera-e under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending, on the delinquency of the request- Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.

Memorandum of Agreement between North Carolina and the U. S. Environmental Protection Agency
dated December 6, 1983.

If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit

of Environmental Management. The Division of Environmental Management may require modification or
