Dear Mr. McKenzie:
Enclosed is a copy of the Compliance Evaluation Inspection (CEI) report for the inspection conducted at
the subject facility on January 19, 2011, by Ms. Barbara Sifford of this Office. The site drains into an unnamed -
tributary of Church Creek in the Yadkin River Basin. The site visit and file review revealed that the subject
facility is covered by NPDES Stormwater General Permit NCG050000. The assistance of Mrs. Wanda Mills
was greatly appreciated during the inspection.
Accordingly, the following observations were noted during the inspection:
1) Stormwater Pollution Prevention Plan (SPPP) Yes X No
A SPPP has been developed the plan needs to be updated to reflect the additional buildings that have
been added to the complex. This plan is to be reviewed and updated on an annual basis. The update is
to include a recertification that the stormwater outfalls have been evaluated for the presence of non-
stormwater discharges.
2) Qualitative Monitoring Yes X No
Qualitative monitoring should be conducted on a semi annual basis. All outfalls should be evaluated
during representative rain events and document the discharge characteristics. A qualitative form -is
included with this report.

3) Other Observations
The new storrnwater pond will help to reduce the turbidiri from he g-avel dock area. To retain the
capacity of the pond it will need sediment to be removed when the capacity is reduced by 50 %. Otter
outfalls noted during the inspection could use some rock to stop erosion.
The facility will have 90 days from the date of this letter to come into compliance wish the rioted
observations. A follow --up evaluation will be conducted after July 1.2011 to ensure compliance with the
facility's Stormwater Permit NCO050019 Certificate of Coverage_
The report should be self-explanatory. Please be advised that violations of the NPDES General
Permit are subject to a civil penalty assessment of up to $25,000.00 per day for each violation. If you have
any questions. comments, or need assistance with understanding any aspect of your permit, please do not

Comment: The SWPPP needs to be updated to include the additional buildings and
drainage areas from those basins. Maintaining the construction stormwater retention
pond would reduce the suspended solids from the gravel dock area. The plan shold
also include the training rosters from the monthly training meeting on spill prevention
and any training that is included for the plan. Plan should be updated by July 1, 2011for
review.

discharge stormwater associated with industrial activities performed at your facility. The purpose of
this letter is to advise you that a representative with the Division's Mooresville Regional Office may
be contacting you in the near future for the purpose of conducting an inspection at your facility.
If an inspection is conducted at your facility, you should be prepared to demonstrate .
compliance with all terms and conditions included in the GP. During the inspection, the following
areas (if applicable) will be evaluated: Stormwater Pollution Prevention Plan, stormwater outfall
locations, a review of any qualitative and/or analytical monitoring data that may be required by the

Your facility is currently covered for stormwater discharge under Genera[ Permit NCG05{X — This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage_ To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal 4
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit_ Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may he assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the 1 1 categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed

If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take hotice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

to receiving waters designated as an unnamed tributary to Church Creek in the Yadkin -Pee Dee River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III
and IV of General Permit No. NCGO50000 as attached.
This certificate of coverage shall become effective April 23, 1993.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
